Elements of Informed Consent

The client has received the following information: Diagnosis Name of procedure, test, or medication Explanation of procedure, test, or medication Reasons for recommending the procedure, test, or medication Anticipated benefits Major risks of the procedure, test, or medication Alternative treatments Prognosis if treatment is refused The nurse has performed the following duties: Assessed barriers, such as hearing impediments, transcultural factors, pain, anxiety, and other factors that could influence the clients understanding of the information Assessed the influence of education, age, developmental level, and emotional status on the clients ability to understand the information Ensured that information was provided in a manner that facilitated understanding Determined that the client is voluntarily giving informed consent
