INVITATION TO BID

The City Government of Olongapo through the Bids and Awards Committee (BAC)
invites suppliers/manufacturers/distributors/contractors to apply for the eligibility and to bid for
the hereunder project:

Name of Project : Procurement of Printed Forms

Location : PUD Collection Section, Olongapo City
Contract No. : OC-74-(CTO-04) -09 GD
Brief Description : Purchase of (240 boxes) computerized receipt in continous form with
numbering and amienda

Approved Budget : Php 1,080,000.00

Prospective bidders should have experience in undertaking a similar project within the
last two years with an amount of at least 50% of the proposed project for bidding. The
Eligibility Check/Screening as well as the Preliminary Examination of Bids shall use non-
discretionary “pass/fail” criteria. Post-qualification of the lowest calculated bid shall be
conducted.

All particulars relative to the Eligibility Statement and Screening, Bid Security,
Performance Security, Pre-bidding Conference, Evaluation of Bids, Post Qualification and
Award of Contract shall be governed by the pertinent provisions of R.A. 9184 and its
Implementing Rules and Regulation (IRR).

Bid Documents will be available to prospective bidders up to Nov. 4, 2009, 8:00 am to

12:00nn only upon payment of non– refundable documents fee of P 3,000.00 to the Treasury
Dept. Olongapo City .

The BAC reserves the right to accept and reject any bid and to cancel all bids at any time prior to
award of contract, waive any defects therein and accept the offer most advantageous to the City
Government without thereby incurring any liability whatsoever to offset expenses or losses the
bidder may incur in the preparation of the bids and obligation to inform the bidder(s) of the
ground(s) for the basis of BAC’s action.
