Objective
Name-based tracking of
pregnant women - for ANCs, Delivery & PNCs children - for immunisation

To facilitate
Closer monitoring of regular check-ups of pregnant women and reduce avoidable complications Complete immunisation of children

For closer monitoring of mortality indicators (IMR and MMR)

Pregnant Women
All pregnant women since 1st April, 2009 In the first instance from 1st December, 2009 Emphasis on ALL pregnancies

Sensitise the District CMOs on the urgency and need for the name based tracking of pregnant women and immunization of children

Hold discussions with the Block Medical Officers to sensitise them

Block Level

To sensitise the ANMs/ASHAs/Block Level Officials/ Block Program Management Unit/LHW Develop modalities for getting the primary information on name based tracking Report the services delivered to the pregnant women and children based on the stipulated dates as per the ANC schedule and immunization schedule
