is a young lady that will be entering middle school during the next school year cycle. Her physical age is 10 years old making her a fifth grader by age however she functions at the level of a kindergartener. Diagnosed with severe mental retardation and cerebral palsy, Brittany spends most of her time in the extended resource room due to her physical and academic needs. Currently Brittany needs no assistive technology. IEP Goal One General Education Content Standard Brittany will be able to count by one to 75 and be able to county by 10s to one hundred. Idaho Common Core Standard (K.CC) Present Level of Performance Brittany can currently count to 20 and can write her numbers. Annual Goal Brittany will be able to count by ones to 75 and by tens to 100 with 95-100% accuracy Evaluation Procedures/Assessment Methods Bi-weekly verbal assessments by her teacher will be done where Brittany will have to be ble to county by both ones and tens. 3 month goal: Brittany will be able to count 35 with 75% accuracy and count to 45 with 50% accuracy. She will also be able to count to 100 by tens to 50 with 50% accuracy and to 100 with 25% accuracy 6 month goal: Brittany will be able to count to 35 with 95%-100% accuracy, to 45 with 75% accuracy, and to 60 with 50% accuracy. Brittany will be able to count by 10's to 50 with 75% accuracy and to 100 with 50% accuracy

IEP Final 2 9 month goal: Brittany will be able to count to 45 with 95-100% accuracy, to 60 with 75% accuracy and to 75 with 50% accuracy. Brittany will be able to count by 10's to 50 with 95-100% accuracy and to 100 with 75% accuracy. IEP Goal Two General Education Content Standard CC Health Standards: Injury Prevention & Safety; Mental, Emotional and Social health: knowledge about prevention of intentional/unintentional injuries and understanding of the consequences of ones decisions to prevent injuries. Also work on the ability to communicate needs/emotions with others to enhance personal health and control emotional outburst. CC Language: Being able to communicate with the support of teachers/adults the needs in a clear manner. Present Level of Performance Brittany can care for herself but has participated in inappropriate acts of sensory stimulation, self harm and at times is unwilling to take instruction from teachers. Brittany can currently care for her basic needs and is able to dress herself. Annual Goal Brittany will be able to communicate her needs without getting upset with those who are providing instruction or assistance. She will also learn ways to refocus her emotions to prevent self-harm and inappropriate acts of sensory stimulation. Brittany will be expected to participate with her peers in an appropriate way. Evaluation Procedure/Assessment Method Monthly evaluations will be done with her speech and occupational therapy staff to modify or change methods of instruction to make sure that Brittany is learning how to take control of her emotions with communications limiting her outburst and issues of self-harm while building communication skills. 3 month goal: Brittany will learn to communicate needs to her teachers and occupational/speech therapy team to help maintain her health and emotional needs improving her behavior in general education and special needs classes. Outburst and acts of self-harm will be less than 6 times and Brittany will participate in class 2 times with her peers.

Outburst and acts of self-harm will be less than 4 times and Brittany will participate in class 4 times with her peers. 9 month goal: Brittany will learn to communicate needs to her teachers and occupational/speech therapy team to help maintain her health and emotional needs improving her behavior in general education and special needs classes. Outburst and acts of self-harm will be less than 2 times and Brittany will participate in class 6 times with her peers. IEP Goal Three General Education Content Standard Common Core: L.K.1, L.K.2 - Be able to read emergent readers text with purpose and understanding. Use a combination of drawing, dictating and writing to narrate a single event or tell about events in an order. Present Level of Performance Brittany can read and spell kindergarten site words. She is able to answer WH questions from a story that is read to her. She is able to write her letters and spell kindergarten level words. Annual Goal Brittany will be able to read kindergarten level books and use a combination of pictures and writing to tell about the events of a story. Evaluation Procedure/Assessment Method Monthly Brittany will be evaluated for her reading progress and her ability to use pictures/basic sentences to tell about an event or story. 3 month goal: Brittany will be able to read 15 words in one minutes with 50% accuracy. She will be able to draw a picture about a character in a story that was read to her. 6 month goal: Brittany will be able to read 15 words with 75% accuracy and an additional 10 words with 25% accuracy in one minute. Brittany will be able to write

IEP Final 4 one sentence (4 words) using her site words to tell about a part of a story read to her. (Example: The dog is big.) 9 month goal: Brittany will be able to read 25 words with 75% accuracy and an additional 10 words with 25% accuracy in one minute. Brittany will be able to write two sentences of at least 4 words from her site words, to tell about part of a story read to her. Remaining Elements Brittany will continue to ride the bus to and from school. This is an activity she finds enjoyable as she is able to be with her friends and brothers. At school, Brittany will be in the least restrictive environment possible spending her school time in both the special education room with experiences in a traditional class room as appropriate for her needs. Occupational services are providing assistance to Brittany through other resources so no further services are needed at this time. As Brittany gets ready to transition to middle school, IEP teams from each school will meet to assure that Brittanys learning, social and physical needs are met giving Brittany the best chance at success. IEPs are built on a 12 calendar month evaluation program starting the first day of school. In the event Brittany reaches her goals sooner, the goal will be modified to include additional learning goals. When there is minor or no progress to completing a goal, the IEP will be reevaluated to ensure that the goal is appropriate to Brittanys level of ability. Brittany will not participate in assessments provide by the state or district as she is being evaluated and tested based on her IEP goals.
