GORDONS 11 FUNCTIONAL PATTERN 1.

HEALTH PERCEPTION AND HEALTH MANAGEMENT PATTERN Before hospitalization Ang kalusugan ay mahalaga at hindi ito dapat pabayaan. The patient considered herself as healthy and strong woman because she can do all her activities of daily living. Ako nga yung taong madalang magkasakit eh, she added. She doesnt believe in quack doctors and doesnt use herbal medicines and herbal plants for management. NUTRITIONAL AND METABOLIC PATTERN Before Hospitalization The patient eats three to six times a day (3-6x/day) with snacks between meals. Her breakfast is usually composed of two (2) cups of rice, one (1) piece of fried egg and pieces of fried dried fish. For lunch, she consumes two to three (2-3x/day) cups of rice, vegetables such as pinakbet, bitswelas and nilagang isda. For dinner, she consumes two to three (2-3x/day) cups of rice, vegetables and fish. She drinks six to eight (6-8) glasses of water a day and seldom drinks soft drinks or other beverages. She is fund of eating sour foods. She also takes snack at any time of the day, which is usually composed of bakery bread and a glass of juice, usually after work. She also eats fruits and vegetables such as apples and oranges for once a week. She didnt take any vitamin or food supplement. She has no difficulty in eating or swallowing foods or loss of appetite. She never experienced nausea and vomiting during eating or drinking. She experienced having constipation and diarrhea. As mentioned by the patient, her weight last January 2014 is 71 kilograms and her height is 55. Her source was from the weighing scale at the junkshop. Her BMI is ____________. During Hospitalization Upon receiving the patient, she has an ongoing IVF of ________________________________.

The patient is on diet as tolerated. She consumes half cup of rice and seldom eats the whole meal being served by the hospital. She verbalized, ngayon hindi ko na nauubos ang pagkain ko kasi nag-iiba yung lasa, parang matabang baka dahil sa gamot. She drinks 1L of water in a day. She didnt experience nausea and vomiting. Currently, her weight is _____________. 3. ELIMINATION PATTERN

Before Hospitalization The patient usually urinates five (5) times a day with clear to yellow, amber, whereas she defecates five to six (5-6) times a day with dark, yellow in semi-solid form. She doesnt have any discomfort in urinating and defecating. She didnt use any laxatives, suppositories or enema. She never had a bowel surgery. During Hospitalization The patient only defecated once upon admission up to the day handling the patient with dark, tarry, semi-solid stool. The patient has no Indwelling Folley Catheter. She usually urinates five to six (5-6) times a day with yellow color amounting to _________L/day. 4. ACTIVITY-EXERCISE PATTERN Before Hospitalization The patient can easily do all her activities. She does household chore, laundry, collecting bottles and selling corn. The patient stated that she never had activity intolerance except for what recently happened to her. After her work, she rests and watches television. The patients exnercise is walking and considersherb work. The patient has no Indwelling Folley Catheter. She usually urinates five to six (5-6) times a day with yellow color amounting to _________L/day.
