PROVISIONAL INTERVIEW CALL LETTER

Subject:-

RECRUITMENT TO 2000 (INCLUDING 47 POSTS RESERVED FOR


DISABLED PERSONS AND 131 POSTS RESERVED FOR MINORITIES)
POSTS OF MEDICAL OFFICER (BS-17) ON TEMPORARY BUT
LIKELY TO BECOME PERMANENT BASIS IN THE PUNJAB HEALTH
DEPARTMENT.

With reference to your application for the above post, you are requested to attend the
office of the Punjab Public Service Commission for interview at 2-Agha Khan (Davis) Road, Lahore
on 19-07-2013 at 08:00 A.M. sharp.
Important Note:a)

You are being called provisionally for interview based on the data provided by you on the
application form.

b)

Information provided by you in filled form will be cross checked with the original
documents before the interview and if found correct, you will be allowed to participate in
the interview.
In case any false information or variation is observed between information provided on
the form and the original documents presented by you on the interview day, your
candidature will be rejected, as undertaken by you on the application form.
You are required to produce the following DOCUMENTS IN ORIGINAL at the
time of interview failing which you will not be interviewed and your application will
be rejected:-

Note.

i)

Matriculation Certificate or in case of O Level, equivalent certificate


from Inter Board Committee of Chairmen (IBCC), Islamabad showing
total/obtained marks, date of birth. Or school leaving certificate showing
date of Birth.

ii)

Intermediate Certificate / A level along with equivalent qualification


from (IBCC) Islamabad, showing total/obtained marks.

iii)

Degree/DMC of MBBS (1st to final profs) duly issued by Controller of exam


showing total/obtained marks and percentages.

iv)

Post graduate degree/higher diplomas duly registered / recognized by P.M.D.C

v)

Valid P.M.D.C Registration Certificate showing all medical degrees therein.

vi)

Domicile certificate of district of Province of Punjab.

vii)

Departmental Permission Certificate duly signed/stamped by the Appointing


Authority or authorized officer on behalf of appointing authority i.e. Secretary
Health, Government of the Punjab even if you joined any Govt. or Semi Govt.
service after submitting your application.

viii)

Experience Certificates/ House Job Certificate duly signed/stamped by the


Appointing Authority. Current charge / Adhoc / contract experience certificate
must be signed by the competent authority of Health Department.

Foreign qualification and experience must be recognized/registered by H.E.C/

One attested photograph.


Application fee / Treasury Receipt for Rs.300/- in original.
Valid C.N.I.C.
Disability certificate duly issued by competent authority of Social Welfare
Department (For disabled candidates only).
