What to Submit:
Except where specified, all documentation should be submitted in digital / scanned copy.

- A digital copy (on USB Stick or CD) of the filled-in, official application form and
Annexe 1 (Project Budget) in an electronic folder named Kreattiv Application
Form.

- A digital copy of any additional documentation (on the same USB Stick or CD)
presented in one electronic folder named Kreattiv Additional Information.
This folder should contain:
o CV/Portfolio of the creative practitioner/s.
o Official documentation (quotations/estimates) in support of the costs
claimed under the project. These could be either electronic documents
or scanned copies of paper-based originals.

The documentation mentioned above should be submitted in the following
format:
- either as a Word Document,
- or as a PDF File


- A printout of Section 3 with all necessary signatures.

The Application is to be submitted in a sealed envelope to either of the following
addresses, and should be received by not later than Friday 16
