-FACILITIES AGREEMENT & WAIVER-

Hereinafter referred to as Participant, and Double T Stables, LLC.

SECTION ONE – PAYMENT


Participant agrees to pay $ n/a per horse for use as a “facilities fee” for access to
equestrian facilities at Double T Stables, LLC. Waiver of such facilities fee shall not constitute a
waiver of any other provision of this agreement.

SECTION TWO – OBLIGATIONS


Participant agrees to abide by the following guidelines and that failure to do so
will constitute a breach of this agreement.
1. Follow all Double T Stables, LLC rules and regulations;
2. Only access stable facilities during posted stable hours;
3. Safety is paramount and safe riding practices must be observed and utilized. When using
arenas, training areas, and trails, Participant must exercise extreme caution when
approaching or departing other riders. Unsafe riding practices on the part of Participant
may result in immediate termination of this agreement. Participant agrees to use ordinare
care in the use of the horses and not abuse, mistreat, or ride any horse in a reckless or
careless manner.

SECTION THREE – WAIVER & RELEASE


In consideration of receiving permission to enter Double T Stables, LLC property,
Participant, for himself/herself, and for his/her heirs, distributes, next of kin, executors,
administrators, and personal representatives releases Double T Stables, LLC, its agents,
officers, servants, and employees of and from any and all liability, claims, demands, actions,
and causes whatsoever, in law or in equity, arising out of or related to any loss, damage, or
injury, including death, that may be sustained by Participant or any property of Participant
while in or on the Double T Stables, LLC premises.

The Participant, being duly aware of the risks and hazards inherent to entering the Double T
Stables, LLC premises and/or participating in any activities on or around horses, voluntariliy
elects to enter in to the Double T Stables, LLC premises. Participant voluntarily assumes all
risks of loss, damage, or injury, including death, that may be sustained by Participant while
on the Double T Stables, LLC premises.
By signing this agreement, Participant acknowledges and represents that he/she
(1) has read this entire agreement; and (2) is eighteen years of age or older and of sound
mind.

WARNING:
Under Colorado Law, an equine professional is not liable for
injury or the death of a participant in equine activities resulting from the
inherent risks of equine activities, pursuant to Section 13-21-199,
Colorado Revised Statutes

SECTION FOUR – AGREEMENT


This agreement may be terminated by either party at any time for any reason.
This agreement shall be governed, construed, and enforced in accordance with the laws of the
State of Colorado. Participant represents that no oral representations have been made on behalf
of Double T Stables, LLC and this agreement constitutes the complete understanding between
the parties hereto. The terms of this agreements shall constitute the entire agreement and may
not be altered, amended or modified except in writing and signed by both parties to the
agreement. All agreements and covenants contained herein are severable. In the event that any
Section, or portion of a Section, were not contained herein. All attorneys fees and cost incurred
in enforcing this agreement shall be awarded to the prevailing party.

STATEMENT OF ACKNOLWEDGMENT BY MINOR

is above specified remise, release, and discharge Double T Stables, LLC their representatives,
officers, agents, and/or any other employees/volunteers from all actions, causes of action, claims
or demands or actions which may arise, in any way, as a result of my ward’s presence in, on, or
about any said stable vehicles or other conveyances, pursuant to the privilege hereby granted.
