Description of work to be performed and services to be provided:

Furnish labor and materials to replace the roofing at the above mentioned address as follows:
Two layer tear off
Up to two sheets of plywood replaced at no additional charge if there is any bad wood ( 64 square feet )
All new drip edge
Ice and water on lower six feet of roof
Ice and water on each valley, as well as 90lb roll in each valley
Elk starter shigles on all lower edges
15 lb felt paper
30 yr Dimensional shingles
New soil pipe covers
All new ridge vent type 2 with matching caps
One day installation
Re-flash any chimneys
Clean gutters, grounds and haul away all debris
Solar seal caulk
Note: Dan, I would be willing to match a competitors estimate if the same quality products are being used.
My Corp is fully licensed and insured as well as workmans comp.
Any changes to this proposal must be in written form.

We propose hereby to furnish material and labor, complete in accordance with the above specifications, for the sum of:
TEN THOUSAND SEVEN HUNDRED AND FIFTY DOLLARS (10,750.00)

Payments to be made as follow:

50% OF TOTAL COST AT START OF PROJECT AND 50% UPON COMPLETION

All material is guaranteed to be as specified. All work to be completed in a workmanship like manner according to standard practices. If the client requests deviations from the above specifications that
represent added costs beyond the time and materials quoted, a separate written addendum will be required before the alterations can be executed. The changes requested by the client will become an
additional charge over and above the originally quoted price. The client agrees that M&J General Contracting will not be held liable for delays caused by labor strikes, accidents, adverse weather
conditions, or other factors beyond the control of M&J General Contracting. The employees and sub-contractors of M&J General Contracting are fully covered by Workman's Compensation Insurance.

Acceptance of Proposal
The above prices, specifications, and conditions are satisfactory and are hereby accepted. Signatures below authorize M&J General Contracting to commence work on the project. Payment for the
project will be made as detailed above. Client acknowledges that they carry adequate levels of hazard insurance on the location/premises in which the work will be performed.
