Summer Explorers Camp
2014 Financial Assistance Form
Dear Parents/Guardians:
We would like to ensure that every child who desires to attend the WCSU Summer Explorers
Camp is able to do so. We have a limited amount of resources set aside for the purpose of
offering financial assistance to anyone who needs it. We are basing the assistance on the sliding
scale guidelines below. In order to be considered for the assistance please see below.

of camp for your child(ren).

If no, please fill out and return the enclosed 2013-2014 Application for Free and Reduced Price for
School Meals to determine the cost of camp for your child(ren).
If you are applying for financial assistance, please return this form and the Application for Free
and Reduced if applicable to the attention and address below by May 30, 2014.
You will receive an invoice for the cost of camp once your application is reviewed and full
payment will be due upon receipt of the invoice. Payment must be received before the start of
camp on July 7th, 2014.
