1-OBJETIVOS
Los objetivos del presente taller se han centrado en el estudio de anticuerpos
anticitoplasma de neutrfilo (ANCA) en dos aspectos:

1.1-El abordaje de los anticuerpos asociados a enfermedad inflamatoria intestinal,
concretamente a la colitis ulcerosa. Por un lado se ha tratado el tema de los ANCA con
patrn atpico, que constituyen un aspecto no muy bien definido por los consensos
internacionales (1,2). Presentan adems la dificultad de su distincin de los distintos
anticuerpos antinucleares o anticitoplsmicos concomitantes y han sido objeto de distintas
aproximaciones (3,4,5).
Recientes publicaciones han caracterizado sus antgenos diana, especialmente en
hepatopatas autoinmunes y colitis ulcerosa (3,6,7). Por otro lado, tambin recientemente,
se ha descrito la asociacin de colitis ulcerosa a anticuerpos especficos de vasculitis
asociados a ANCA (8,9). Ello puede contribuir al diagnstico diferencial entre colitis
ulcerosa y enfermedad de Crohn, en el que la determinacin de ANCA juega un papel
importante y por otro lado, puede afectar a la especificidad de los ANCA para el
diagnstico de vasculitis.

2.1-La comparacin de distintas tcnicas, en general cuantitativas, para la determinacin
de anticuerpos antimieloperoxidasa y antiproteinasa 3.
La distinta sensibilidad y especificidad de dichas tcnicas puede determinar su valor
diagnstico y su aplicacin en el seguimiento de las vasculitis asociadas a ANCA.

Se pretende con este taller, por un lado, conocer qu tipos de tcnicas se utilizan en los
laboratorios que determinan ANCA y por otro, comparar los resultados en una serie de
casos concretos que pueden ser interesantes por sus caractersticas clnicas, de
laboratorio o ambas. Una comparacin estadstica exhaustiva de los mtodos no es el
objeto de este taller, por el nmero reducido de pacientes estudiado para cada tipo de
patologa y por la falta de una tcnica gold standard, que nos impide una comparacin
objetiva. No obstante, creemos que de este estudio descriptivo puede obtenerse una
buena fotografa de lo que es actualmente la determinacin de ANCA en Espaa y
pueden discutirse casos interesantes y el comportamiento de las distintas tcnicas en
cada uno de ellos.

5-INTRODUCCIN
5.1-Patrones de inmunofluorescencia indirecta (IFI) de anticuerpos anticitoplasma
de neutrfilo (ANCA)

5.1.1-Consenso Internacional
El consenso internacional sobre ANCA acordado en 1999 (1) y su adenda de 2003 (2)
establecen la siguiente nomenclatura para los patrones IFI de ANCA:

-Se distingue entre patrones C y C atpico y no entre P y P atpico para evitar que un
patrn C se atribuya, por el clnico, a vasculitis sistmica, si se emite el informe antes de
la realizacin del ELISA para antiproteinasa 3 (a-PR3) o antimieloperoxidasa (a-MPO).

-La extensin nuclear en los P-ANCA no es siempre til para distinguir los que
corresponden a a-MPO o no.

-Se considera que la fluorescencia debe graduarse como negativa, positiva dbil o
positiva intensa, slo sobre la base de la dilucin de cribaje.

-Para seguimiento se recomienda no utilizar los ttulos de IFI sino las tcnicas
cuantitativas para a-MPO y a-PR3.

-El informe debe indicar si el laboratorio no puede confirmar un P-ANCA , debido a un
ANA concomitante y que sern realizadas tcnicas cuantitativas para a-MPO y a-PR3.

-Las muestras positivas deben ser estudiadas por tcnicas cuantitativas para a-MPO y
a-PR3.
C-ANCA: patrn clsico de fluorescencia granular con acentuacin central o
interlobular .
C-ANCA atpico: patrn difuso flat, sin acentuacin interlobular
P-ANCA: patrn perinuclear, con o sin extensin nuclear, incluye ANA
especficos de granulocito
ANCA atpico: incluye todos los otros patrones IFI de reactividad especfica
para neutrfilos o monocitos. Con frecuencia se trata de una combinacin de
fluorescencia citoplsmica y perinuclear.

Respecto a los patrones atpicos de ANCA:

-Se considera que no existe an una prueba satisfactoria para detectar ANCA en
enfermedad inflamatoria intestinal o hepatopatas autoinmunes (colangitis esclerosante o
hepatitis autoinmune).

-En general, sern aquellos en los que se obtiene positividad para cualquiera de los 4
patrones descritos en el recuadro pero que no correspondan a a-MPO o a-PR3. La IFI
obtenida en etanol desaparece, en la mayora de casos, en los portas fijados con
formalina, aunque pueden obtenerse resultados poco consistentes en muestras
individuales. Por ello el uso de portas fijados con formalina se deja a la eleccin del
laboratorio.
-No se recomienda, por el momento, la utilizacin de tcnicas ELISA para antgenos
distintos de MPO y PR3.

5.1.2-Otras propuestas de definicin de patrones ANCA atpicos

Algunos autores han propuesto patrones distintivos para los patrones ANCA atpicos:

-El patrn very p , sin extensin nuclear, aunque, como ya se ha dicho no es aceptado
por el consenso internacional por su inconsistencia para distinguir entre sueros a-MPO
positivos o negativos (1,2).

-El patrn mixto , por combinacin de C-ANCA y P-ANCA, denominado snow-drift like
(5).

-Por ltimo, Terjung et al (3) propusieron en 2001 la siguiente clasificacin de patrones
ANCA, basada en la presencia o no de puntos intranucleares y en los patrones IFI
obtenidos en portas fijados con etanol o formalina. Este mismo grupo y otros, en
sucesivas publicaciones (6,7), han caracterizado a los antgenos que dan lugar a este
patrn p atpico, asociado a colitis ulcerosa y hepatitis autoinmune, como constituyentes
de la membrana nuclear.

5.2-Mtodos para la determinacin de anticuerpos antiproteinasa 3 (a-PR3) y
antimieloperoxidasa (a-MPO)

5.2.1.-Recomendaciones internacionales y del Grupo Espaol de Autoinmunidad
(GEAI)
El consenso internacional sobre determinacin de ANCA aconseja realizar, en aquellas
muestras positivas por IFI, una confirmacin por tcnica ELISA para anticuerpos
antiproteinasa 3 (a-PR3) y antimieloperoxidasa (a-MPO). Para el seguimiento de
pacientes positivos se recomienda usar los niveles obtenidos por ELISA y no los ttulos de
IFI, ya que la correlacin no es buena entre los resultados obtenidos por ambas tcnicas
(1,2).
El protocolo del GEAI indica tambin la necesidad de realizar determinacin de
anticuerpos a-PR3 y a-MPO en muestras positivas por IFI y para el seguimiento se
recomienda mtodo cuantitativo (10).

5.2.2.-Mtodos
La sensibilidad y especificidad de las tcnicas ELISA para a-MPO y a-PR3 ha ido
aumentando desde los ELISA directos, tcnicas de primera generacin en las que los
antgenos se fijan directamente a la placa, a los ELISA de captura, en los que se evitan
cambios conformacionales, en PR3 o MPO, utilizando un anticuerpo fijado a la placa que
une el antgeno por eptopos distintos a los de los autoanticuerpos (tcnicas de segunda
generacin).
Los ELISA de tercera generacin o de alta sensibilidad (hsELISA: highsensitivity ELISA),
utilizan una protena como anchor o puente entre el antgeno y la placa. stos ltimos se
postulan como un sistema que an preserva mejor las caractersticas antignicas de PR3
y MPO (11,12), aunque existen estudios que les atribuyen una sensibilidad similar a los
ELISA de captura (13). Algunos trabajos han detectado incluso una mejor capacidad
diagnstica de las tcnicas de primera generacin para a-MPO, mientras que las de
segunda o tercera generacin siguen siendo consideradas las ms adecuadas para a-
PR3 (13).
Recientemente han aparecido tcnicas de quimioluminiscencia que, en algunos casos,
combinan tcnicas con antgenos unidos directamente a la placa con otras tipo anchor
(14). Estas tcnicas se han descrito equiparables a las tcnicas ELISA de ltima
generacin y presentan, junto a los ELISA fluorescente de pocillo individual, la ventaja de
la rapidez en la obtencin de resultados y de permitir carga contnua de muestras. De
todas formas, su implantacin es muy reciente y se requieren ms estudios para confirmar
su utilidad.
Existen tambin en el mercado distintos mtodos cualitativos como dot-blot o
inmunofluorescencia para el cribaje de a-MPO y a-PR3 .

7-RESUMEN DE HISTORIA CLNICA DE
LOS PACIENTES

SUERO 1: RVM
Varn de 71 aos, afecto de hipertensin arterial, diabetes mellitus tipo II, hiperuricemia y
cardiopata isqumica. Tras ingreso en urgencias por ITU (06-11-2012) y empeoramiento
general, ingresa de forma programada (07-11) y se diagnostica, por TAC abdominal y
colonoscopia, de colitis ulcerosa. Durante el ingreso, 15 das antes de la toma de muestra
para el taller, se produce cuadro de bacteriemia por Klebsiella pneumoniae, que se trata
con antibitico de amplio espectro. Recibe,adems, tratamiento con Linagliptina, Micardis,
Alopurinol, Sintrom, Pantoprazol y Dacortin.

SUERO 2: AMZF
Mujer de 33 aos. Diagnosticada de Lupus eritematoso sistmico, nefritis lpica y
gromerulonefritis rpidamente progresiva por inmunocomplejos, demostrada por biopsia
renal percutnea. Antecedente de toxemia gravdica y rinitis alrgica. Presenta el debut
de LES en el post-parto (08/2012). Recibe bolus de corticoides y rituximab. En el
momento actual se encuentra en tratamiento por hemodilisis.

SUERO 3: MSR
Mujer de 68 aos diagnosticada de enfermedad de Wegener con afectacin pulmonar,
con biopsia compatible. Adems presenta hipertensin arterial, alergia a Trimetoprim-
Sulfametoxazol, DM tipo II, dislipemia y sndrome ansioso-depresivo. En el momento de la
toma de muestra se encuentra ingresada por ITU debida a Klebsiella pneumoniae, que
se resuelve con tratamiento con amoxicilina-clavulmico.
Su tratamiento habitual incluye Dacortin, esomeprazol, Novomix 30/70 Flexpen,
Metformina, AAS, Candesartan, Simvastatina, Fenofibrato, Tramadol, Bromazepam,
Duloxetina, calcio Vit D y bifosfonatos.

SUERO 4: JAPH
Mujer de 32 aos, diagnosticada de colitis ulcerosa, con colonoscopia compatible.
Presenta antecedente previo de infeccin por Dientaomeba fragilis (1 mes antes de la
toma de muestra del taller), ya erradicada. Actualmente en tratamiento con Dacortn,
Pentasa, Kilor y Omeprazol.

SUERO 5: BJM
Mujer de 33 aos, diagnosticada de colitis ulcerosa, con colonoscopia compatible. En
Marzo de 2012 sufre hepatitis aguda con serologas virales negativas. Actualmente en
tratamiento con Mesalazina.

SUERO 6: APSO
Mujer de 39 aos que acude a urgencias (24/04/12) por cuadro de pielonefritis aguda
tratada con clavulmico. Tras la suspensin del mismo comienza con tos no productiva y
dolor torcico de caractersticas pleurticas, disnea de esfuerzo y fiebre. En Rx de trax se
aprecia infiltrado en base izquierda con derrame pleural e imgenes nodulares en
hemitrax derecho.
En la analtica se constata VSG 120, Hb: 10.8, leucocitosis (15300) con eosinofilia
(10.5%), PCR 18 mg/dl y sedimento de orina patolgico con hematuria y leucocituria, sin
sntomas miccionales. IgE:318, IgA:944, IgE frente a Aspergillus negativa. Estudio de
parsitos en heces: negativo. Serologa HIV y estudio microbiolgico en orina: negativo.
El TAC torcico indica afectacin parenquimatosa con ndulos y masas bilaterales.
Presenta tambin derrame pleural, y adenopatas mediastnicas y parahiliares inferiores a
10 mm. Se realiza fibrobroncoscopia, lavado broncoalveolar (BAL) y biopsia transtorcica
y pleural, obtenindose resultados negativos para PCR y cultivos de micobacterias,
bacterias y hongos. En BAL se obtiene cociente CD4/CD8 > 3.5 y eosinofilia de 1%.
El estudio de Anatoma Patolgica objetiva fibrosis pulmonar, granulomatosis
necrotizante con granulomas en empalizada y en uno de los informes se objetiva
granuloma de aspecto necrtico claramente caseoso. En todos los casos las tinciones
para bacterias y hongos son negativas y no hay signos de vasculitis.

SUERO 7 : JCP
Varn de 79 aos. Diagnosticado de Enfermedad de Wegener en 2003, tras estudio de
cuadro txico en el que se evidenci patrn radiolgico compatible con ndulos
pulmonares bilaterales y bronquiectasias. Presenta adems: enfermedad renal crnica,
estenosis bilateral de cartidas, dislipemia, calcificacin de aorta abdominal, tiroidopata y
bradicardia sinusal. En tratamiento con Doxazosina , Dacortin, Omeprazol, Esertia, Adiro,
Plusvent, Atorvastatina, Kilor, Duphalac.

SUERO 8: CCB
Mujer de 57 aos. Diagnosticada de L.E.S. en 1993, con sintomatologa cutnea, articular
y renal. La biopsia renal indic nefritis lpica proliferativa focal tipo III. En 2010 se
evidencian, por RMN, lesiones en sustancia blanca y presencia de anticuerpos
antifosfolpido a ttulo moderado, por lo que se diagnostica de SAP secundario a LES.
Actualmente en tratamiento con Omeprazol, calcio, AAS, amlodipinio, aliskiren,
ezetimiba, micofenolato, cido risedrnico, lormetazepam, citalopram, prednisona.

SUERO 9: FBS
Mujer de 65 aos. Diagnosticada de colitis crnica. En el momento actual, por clnica y
biopsia de muestra endoscpica, se diagnostica de colitis ulcerosa severa con marcada
actividad. Presenta adems: varices, obesidad, necrosis avascular de cabeza de fmur e
insuficiencia mitral. Recibe tratamiento con: ramipril, bisoprolol, torasemida, sintrom,
sinvastatina.

SUERO 10: MMM
Paciente con esclerosis sistmica difusa y vasculitis necrotizante, con afectacin del
sistema nerviosos perifrico y vascular confirmada por biopsia del nervio sural y muscular.

8-MTODOS
En primer lugar se mostrarn todos los datos sobre las tcnicas utilizadas por cada
laboratorio participante. El nmero de laboratorios participantes es de 48, no todos
participan en todas las determinaciones y algunos presentan ms de una determinacin
por parmetro y suero, con distintos fabricantes, de aqu que el nmero de informes
emitidos por parmetro y suero sea diferente del nmero de participantes.
Se mostrarn de manera resumida, las tcnicas con las que se han informados resultados
en este taller, con las siguientes excepciones:

IFI-METANOL: El escaso nmero (4) y la diversidad de resultados hace que no se
considere esta tcnica en el anlisis de resultados.

ANTI-MEMBRANA BASAL GLOMERULAR: Todos los sueros son negativos para los
11 laboratorios que informan esta tcnica.

ASCA: Slo hay un resultado por suero, excepto para el n 5 que hay dos (uno
positivo y otro negativo). No se considera esta tcnica en el anlisis de resultados.

ANA: Se ha alcanzado consenso en todos los sueros analizados. Se mostrarn los
resultados de cada participante as como en las tabas de resumen por tcnicas y por
sueros, pero dado que los anticuerpos antinucleares no son el objeto de este taller,
sino una herramienta para resolver las posibles interferencias con los ANCAs, no se
considerar esta tcnica en el anlisis de resultados.

PANEL ANCA: Se mostrarn los resultados de cada participante as como en las
tablas de resumen por tcnicas y por sueros. A pesar de que se ha alcanzado
consenso en todos los sueros analizados no se considerar esta tcnica en el anlisis
de resultados dado el bajo nmero de laboratorios que informan y la falta de
concrecin de los antgenos que incluye cada fabricante en este panel.

10-CONCLUSIONES

10.1.-Patrones IFI
1-La nomenclatura de los participantes para sueros P-ANCA positivos en etanol y
negativos en formalina vara, prcticament al 50%, entre A-ANCA y P-ANCA. Sera
conveniente unificar criterios en cuanto a este tema.

2-La positividad, tanto en portas fijados en etanol como en portas fijados en formalina y el
tipo de patrn informado, segn la IFI en portas etanol, no dependen del origen de los
portas (tipo de fabricante) sino de los laboratorios participantes.

3-El hecho de utilizar una dilucin de cribaje de 1/10, 1/20 1/40 tampoco condiciona la
positividad o negatividad en etanol y formalina.

4- Los participantes no han descrito la presencia o ausencia de puntos intranucleares en
sueros P-ANCA positivos. En nuestra experiencia, la deteccin de puntos intranucleares
en sueros P-ANCA positivos de pacientes con colitis ulcerosa no es siempre positiva por
microscopa de fluorescencia, por lo que su observacin no es til, en general. Adems
en sueros con P-ANCA debidos a anticuerpos a-MPO pueden hallarse tambin
anticuerpos dirigidos a estructuras de la membrana nuclear que den dichos puntos en IFI,
como ocurre con el suero 10 del taller.

5-En la mayora de casos, la IFI sobre portas fijados con formalina es ms sensible que la
IFI sobre portas fijados con etanol.

6-La presencia de anticuerpos dirigidos contra antgenos del citoplasma (antiprotenas
ribosomales, por ejemplo) puede dar lugar a patrones C-ANCA, difciles de distinguir de
los dirigidos a protenas especficas de los neutrfilos, como ha ocurrido con el suero 8 del
taller en el que se ha obtenido consenso cono C-ANCA positivo.

7-La absorcin con pellet de clulas HEp2 puede ayudar a distinguir los ANA de los
ANCA atpicos, como ocurre en el suero 6, sin embargo, slo un laboratorio ha informado
la utilizacin de esta tcnica.

8-La mayora de laboratorios no han utilizado portas fijados con metanol, por lo que no es
posible deducir conclusiones vlidas sobre los resultados obtenidos con los sueros del
taller. Los dos sueros a-MPO positivos presentan adems antinucleares por lo que
tambin han sido informados como positivos en metanol.
Los sueros A-ANCA positivos correspondientes a pacientes con colitis ulcerosa han sido
informados positivos en portas fijados con metanol, por los laboratorios que han utilizado
este substrato.

10.2-Tcnicas de determinacin de anti-MPO y anti-PR3

1-Un 12% de laboratorios utiliza tcnicas cualitativas para deteccin de a-PR3 y a-MPO.
En algunos laboratorios no se constata tcnica cuantitativa de confirmacin, necesaria
adems para un correcto seguimiento de los pacientes, segn recomendaciones
nacionales e internacionales (1,2,10).

2-Tal como refleja la literatura sobre el tema, existen diferencias en la sensibilidad de las
distintas tcnicas de deteccin de a-PR3:

Respecto a los sueros n 3 y 9, las distintas tcnicas se comportan de forma similar en
ambos: la mayora de ELISA directos dan negativos, mientras que las tcnicas anchor y
de captura son positivas. Las dos tcnicas de quimioluminiscencia utilizadas se
comportan de forma opuesta , aunque hay pocos laboratorios que las usan. Estos sueros
corresponden a un paciente no activo con granulomatosis con poliangiitis (anteriormente
denominada Granulomatosis de Wegener) (suero 3) y a un paciente con colitis ulcerosa
(suero 9). Este ltimo caso apoya recientes publicaciones sobre la asociacin entre a-PR3
y colitis ulcerosa y la utilidad en su diagnstico diferencial (8,9). En ambos casos hay
concordancia entre los patrones IFI y los ELISA, ya que la mayora de los laboratorios que
dan IFI positiva les atribuye un patrn C-ANCA, caracterstico de PR3.
El problema en el paciente 3 (positivo para PR3 en el 49% de determinaciones), es que
no est activo, aunque la persistencia de un valor positivo de a-PR3 podra indicar la
necesidad de un seguimiento estrecho, para evitar recada, segn postulan algunos
autores (15). Otros trabajos describen que el exceso de sensibilidad de algunas tcnicas
ELISA de ltima generacin puede crear confusin en este tipo de pacientes (16).

El suero 5, que corresponde a un paciente con colitis ulcerosa, slo en el 30% de las
determinaciones ha dado a-PR3 positivo y no corresponde a patrn C-ANCA por IFI, por
lo que la positividad a-PR3 es discutible. En este caso las tcnicas que dan resultado
positivo se encuentran dentro de los ELISA directos, dot-blot, quimioluminiscencia y
tcnica multiplex. Para la determinacin de a-MPO hay algunos laboratorios que utilizan
tcnica directa (mientras que usan una de ltima generacin para a-PR3), de acuerdo con
algunos autores que describen mejor capacidad diagnstica de esta tcnica para a-MPO
(13).

3- En cuanto a los dos pacientes a-MPO positivos, hay muy buena concordancia entre
laboratorios (100% los dan a-MPO positivos) y entre el patrn P-ANCA por IFI y la
positividad para a-MPO. Se trata de dos pacientes que presentan una conectivopata, en
el primer caso se trata de un lupus eritematoso sistmico y en el otro de una esclerosis
sistmica. El primero no presenta clnica de vasculitis y la biopsia renal tampoco es
compatible con vasculitis necrotizante, mientras que el segundo s que es una vasculitis
sistmica. Estos resultados apoyan el hecho de que la presencia de estos anticuerpos en
enfermedades autoinmunes sistmicas no va siempre asociada a vasculitis sistmica.

4-La introduccin del recientemente aparecido estndar internacional para a-PR3 y a-
MPO de los Centers for Disease Control and Prevention (CDC), de Atlanta, puede
contribuir a una mayor concordancia de las distintas tcnicas, a pesar de que se ha
descrito muy variable binding de distintos estndares a los diferentes kits (14,17). La

utilizacin del estndar internacional podra ser de gran utilidad en la optimizacin de las
tcnicas con menor sensibilidad.

5-La deteccin de antgenos distintos de MPO y PR3 se ha realizado por una minora de
laboratorios. En los sueros de pacientes con colitis ulcerosa (1, 4, 5) , no se ha detectado,
mayoritariamente, positividad para estos antgenos minor, de acuerdo con la literatura
(18).
