3.0 CONDITION OF VESSEL - Report the condition of the following as applicable,
giving details as shown:
HULL - Describe each section of the vessels hull port and starboard side including:
Stern area
Bow area
Port side
Starboard side
Ramps
Anchors
List any visible damage:
State of vessels paintwork or coating
State of wind/weather area
State of the anti-fouling paint where seen
Build up of marine life where seen
State of doors, ramps and anchors and chains
State of fenders and sponsons
STEEL DECKS - All steel decks are to be inspected including poop deck and
forecastle.
The different levels of deck are to be described. List:
Each deck as inspected
Any damage to the decks
Condition of deck covering or paintwork
Fittings on the deck and their condition
Machinery and mechanical equipment on the decks
Condition of the machinery
Whether machinery was run for inspection
Loose equipment on the decks
Consumables including ropes etc. on each deck

Condition of watertight doors
Location and condition of scuttles, portholes, windows
Condition of rails and bulwarks
Condition of stairs
Condition of cargo winch platforms
Condition of cargo winches and their wires
Condition of masts
Condition of container sockets on hatch covers and decks

Normal contents of tank
Whether full or empty or residue present
Condition of tank access openings and closing devices
Condition and type of loaders
Condition of each landing if fitted
Locations of landings
Condition of pipe work in tanks

Condition of suction, and suction filters
Condition of tank heating piping and supports
Condition of sounding tubes and striker plates
Condition of remote sounding fittings
COFFERDAMS - All cofferdams on vessel to be entered and inspected after ensuring it is
completely safe to do so. List:
Cofferdam entered
Any damage or wastage
Presence of water
State of manhole covers and seals
Coating type and condition
Position and tightness of transponders if fitted
HOLDS - All holds are to be entered and their condition reported on, List:
Each hold inspected
Type of hold and number of decks
Any damage and/or wastage visible
Coating of the hold sides, bottom, and deck head
Condition of the coating
Type of ladders
Condition of the loaders
Type of landing and location
State of bilge hatbox, or arrangement
Condition of pipe work within the holds
Condition of vents passing through the hold
Condition of lighting in hold
Condition of electrical fittings in hold
BALLAST TANKS - All ballast tanks are to be entered including peak tanks if they
are used for ballast after ensuring it is safe to enter. List:
Each ballast tank entered
Any damage and/or wastage present in tank
Coating of the tank
Condition of the coating
Condition of the manhole cover and seal
Condition of all pipe work in the tank
Condition of sounding tubes and striker plates
STORES - All stores are to be entered. Their location is to be clearly stated and
whether within watertight areas secured by watertight doors. List:

Any damage and/or wastage present in store o type of store
The storage facilities (racks, etc.)
The deck covering
Condition of deck covering
Complete and full list of contents
Complete and full list of all consumables o coating used in the store
Condition of coating
CARGO HANDLING AREAS All cargo handling areas and control rooms are to
be entered. List
Each cargo handling area or control room entered and it's purpose
Any damage and/or wastage present
Coating of the surrounds
Condition of coatings
Whether in use at time of inspection
ACCOMMODATIONS - All areas of the accommodations are to be entered. Stores
are to be inspected and their contents listed. List:
All areas entered
Any damage and/or wastage visible
List contents and their condition
Deck covering and it's condition
Condition of partitions and their surface condition
NAVIGATION AREAS AND BRIDGE - All areas of navigation on the vessel are to
be inspected including the bridge, radio room; chart room and upper navigation
bridge (Monkey Island). In general all equipment should be demonstrated to be
both available and in running order. Operate all lights and their alternates and
ensure all are operating. List:
All areas entered
Any damage and wasted visible
Coating of deck head, deck and sides
Condition of coating
Equipment installed
Whether working
Contents of book racks
Charts available
List of navigation assistance books, journals, and almanacs
Action of controls
Condition of all doors

Condition of aerials and radar antenna

ENGINE ROOM - All areas of the engine room are to be entered and detailed in
the report. A complete listing of all equipment would be desirable but this will not
always be possible. List:
All equipment under repair or routine maintenance
All equipment running at the time of inspection
All non-operating equipment if possible
All non-operating equipment is to be run, if possible. and their condition
reported
All pipe work is to be inspected together with all valves
Shipside valves are to be viewed for leakage and operating capability
Bilges are to be inspected for cleanliness and for the presence of water and
oil contamination
Report any leakages whether from pumps, tailshaft, pipe work, joints, seals or
other areas
Any damage and/or wastage visible
General state of all areas of engine room o cleanliness of all areas
Coating of all areas
Condition of coating of all areas
All spares in engine room and all engine room stores o all consumable stores
All packing material
Condition of walkways and platforms
Condition of rails, steps, and stairs o condition of floor plates and their
condition
ENGINE ROOM WORKSHOP - The engine room workshop is to be inspected and
it's status included in the report. List:
Equipment installed
Equipment that was operated
Defective equipment to be listed
Condition of work shop
Coating of all areas
Condition of coating
Heavy spare parts in this area and whether new or used
Class recommended spares and where located
Whether class spares are new or reconditioned

ELECTRICAL STORES/WORKSHOP - The electrical workshop is to be inspected
and a list of equipment installed made. The electrical spare parts store is to be
inspected. List:
Condition of workshop and store
Coating of both areas
Condition of coatings
Complete list of spare parts available
Complete list of parts under overhaul
Complete list of spares stored in this area
Complete list of consumables
GENERAL - Make a list of:
Firefighting equipment on board
Safety equipment on board
Lifeboats and liferafts on board
Condition of lifeboats and liferafts
Condition of lights
Condition of emergency batteries
Condition of radio batteries
TANK SOUNDINGS - all consumable liquids are to be measured and listed as
being on board. When sounding fuel oil tanks, water finding paste is to be used.
List:
Each tank dipped, its content and specific gravity
Each tank gauged, its content and specific gravity
All drums that are full, their contents, and their specific gravity
