PSYCHOLOGICAL TEST REPORT

II. GENERAL OBSERVATION
Subject, Michael Faypon, is a 46 year-old, right-handed, mesomorph,
married, male, admitted to an inpatient mental health center on Pavilion 1-Open
Ward. He is fair complexion and stands about 56. On February 16, 2014 he has
been admitted by his youngest daughter, Maricar Faypon, 21 years old, due to
complains of his assaultive and aggressive behavior. He was well-oriented on his
mental condition that he even narrated how and when it started. He reported that
on November 29, 2009 his father died and the first time he had auditory
hallucinations. The voice he heard was all self-inflicting and suicidal ideas due to
extreme depression. Since then he became irritable, ill-tempered, and chaotic.
He also reported that he had visual hallucinations mostly when hes alone such
as seeing female celebrities in their house and superstitious beliefs that he will
save the world from famine.
Michael was once an illegal drug user. But on the loss of his father, the
intake of illegal drugs and alcohols accrued amply. He added that he used drugs
because of his former job as construction worker which requires an active and
vigorous body and mind coordination. He also became addicted on cigarettes
and caffeine.
Michael is the only child in the family. When his father died, he became
full-time caregiver on his mother. Michaels wife is an illegal immigrant in Bahrain
for more than 8 years.
Michael was fully oriented to his surroundings at all times. He was dressed
with the patients gown uniform and engaged in conversation readily. He
appeared to exert full effort on all tasks presented to him and maintained
attention and concentration in this one-on-one testing situation. Based on all
circumstances, the environmental setting, Michaels motivation, and the testing
conditions, the results of this evaluation appear to be valid and interpretively
useful.

Classification - Borderline
The subject obtained a numerical IQ of 76 which falls currently along the
Borderline. However, his premorbid IQ of 88 lies along the Low Average level.
The result of his psychogram reveals that he has fair short term memory. He has
poor capacity to draw from experiences and of in general knowledge. The learning
ability, numerical reasoning, and common sense reasoning are all poor. Also, he has
poor capacity to verbal-concept information and to comprehend and evaluate total
situation. Impairments are found in his visual memory, deductive reasoning, sense of
spatial relations and visual motor speed.

BENDER VISUAL MOTOR GESTALT TEST
The BVMGT result indicates that Michael has alcoholic psychosis and
schizophrenic condition. He has low frustration tolerance and easily stimulated by
environment. He has the tendency to be ambivalent in decision making regarding with
his interpersonal relationship and depressive.

DRAW A PERSON TEST
The DAPT result discloses that the subject, Michael Faypon, has poor self-
concept and has character disturbances. He has possible auditory hallucinations, bad-
tempered tendencies and even assaultive tendencies. There are repressed or
suppressed aggressive tendencies where the aggression may be express in more
evasive ways perhaps in furtive outbursts.

SACHS SENTENCE COMPLETION TEST
The SSCT result indicates Michaels attitude towards his family, sexual
relationship, interpersonal relationship, and relationship to himself. He express positive
feelings towards his mother. He admires his father but wishes that their relationship
were closer. He may be aware that his family does not recognize him as a mature
person but has no difficulty in relating them. His attitude towards women are only minor
or superficial criticisms as he was satisfied in his relationship. In his interpersonal
relationship, there is no found disturbances. His attitude toward authorities are
commonly positive and he express good mutual feelings to his colleagues. He has fear
of self-assertion which is fairly common and not pervasive. His regrets over past seems
mildly disturbed by his failure to control with trouble as he stated that: Ang
pinakamalaking kamalian ko ay pagda-drugs; Ang pinakamasamang bagay na aking
nagawa ay hindi ko na babalikan pa gaya ng pagdodroga at pag-iinom. He appears to
be confident on his ability to overcome obstacles. He is well adjusted in his childhood
and seems confident in achieving his goals.
GENERAL SUMMARY
The subject, Michael Faypon, 46 years old, obtained a numerical IQ of 76 which
falls currently along the Borderline. Nonetheless, his premorbid IQ of 88 lies along the
Low Average level. He has possible auditory hallucinations, bad-tempered tendencies
and even assaultive tendencies. He is depressive and mildly disturbed by his failure to
control with trouble.
Results of psychological evaluation reveal an extended history of alcohol abuse
and a psychotic disorder characterized primarily by disturbance of thought content, with
relative integrity of thought process and no clear indication of perceptual disturbance.
The subject appears to be attempting to cope with his illness using extreme
guardedness and withdrawal.
