Administrator of the Estate of Christina Pratt, deceased, by and
through her attorney, JOSEPH R. ROMINSKI of SWANSON & ROM1NSKI,
and as Count I of her Complaint at Law against the Defendant,
CITY OF PARK CITY, states as follows:

1. That CAROL SPIZZIRRI is an idult female under no legal

disabilities who has been appointed by this Court as Special
Administrator of the Estate of Christina Pratt, her deceased
daughter, for purposes of instituting this suit.

2. That the Defendant, CITY OF PARK CITY, is a corporation

organized and existing under the laws of the State of Illinois,
which, at all times material herein, provided police and other
services at the area at Routes 41 and 43 in Park City, Illinois.
3. That on September 7, 1992, a 1987 Pontiac Grand Am motor
vehicle owned and operated by Christina Pratt was involved in a

Administrator of the Estate of Christina Pratt, deceased, by a-id
through her attorney, JOSEPH R. ROMINSKI of SWANSON & ROMINSKI,
and as Count II of her Complaint at Law against the Defendant,
WARREN-WAUKEGAN FIRE PROTECTION DISTRICT, states as follows:

1. That CAROL SPIZZIRRI is an adult female under no legal a

disabilities who has been appointed by this Court as Special
Administrator of the Estate of Christina Pratt, her deceased
daughter, for purposes of instituting this suit.
2. That the Defendant, WARREN-WAUKEGAN FIRE PROTECTION 1 YE

DISTRICT, is a corporation organized and existing under the laws 3

of the State of Illinois, which, at ail times material herein,

provided fire and rescue services, for Warren Township, Illinois.
3. That on September 7, 1992, a 1987 Pontiac Grand Am motor
vehicle owned and operated by Christina Pratt was involved in a

Township, Lake County, Illinois.
