Guidelines for Minimizing the Number of
OVIQ Inspection Observations

OVIQ is annual base and each inspection will be
different depending on the type of marine vessel
being inspected. When Chevron or any other Oil
Company initiates an OVIQ inspection the questions
will be generated by the OVID system are based on
the type and size of vessel being inspected.

All the questions are essentially safety based, OVIQ
is a safety inspection specifically designed to look at
safety of vessel operations. In todays world, safety
of Operations is our most important requirement, we
can conduct Operations and achieve 100% success
but if we injure one person then we failed to deliver.

These guidelines are look at to the basic requirements
for vessels to undertake an OVIQ inspection and
suggestions for vessel Owner/ Operator may like to
do before an inspection commence to reduce the
amount of non conformance observations. OVIQ is
much about vessel-shore based Safety Management
System (SMS) onboard and implementation. Below
are some suggestions of what you may like to
consider before doing the OVID inspection:

1. OVID Self Assessment
Perform its Self Assessment. OVIQ Master
Template (example) could be utilized as
reference. This template is available in OCIMF
OVID website.


2. ISMS Internal Audit
o Perform an internal ISMS Audit on vessel.
Find the gaps of non conformance and start
to correct them before the OVID inspection.
Ensure that all Internal ISMS Audit items
are addressed and ISMS is implemented.
o Review your last external ISMS audit and
clear all previous non conformances.
If a vessel does not have ISMS
implemented on board because it is under
the ISM Code Application then ensure
that a Safety Management System is in
place on board and implemented to cover
all safety risks.

Pengamatan Inspeksi OVIQ

OVIQ dilakukan setiap tahun dan setiap pemeriksaan
akan berbeda tergantung pada jenis kapal yang
diperiksa. Ketika Chevron atau lainnya memulai
inspeksi OVIQ, pertanyaan akan dihasilkan oleh
sistem OVID didasarkan pada jenis dan ukuran kapal
yang diperiksa.

Semua pertanyaan pada dasarnya mengenai
keselamatan, OVIQ adalah inspeksi keselamatan
khusus dirancang untuk melihat keselamatan
pengoperasian kapal. Dalam dunia sekarang ini,
keamanan Operasi adalah kebutuhan kita yang paling
penting, kita bisa melakukan Operasi dan mencapai
keberhasilan 100% tetapi jika kita melukai satu orang
maka kita gagal untuk mendapatkan keberhasilan.

Pedoman ini untuk melihat persyaratan dasar bagi
kapal-kapal untuk melakukan pemeriksaan OVIQ dan
saran untuk Pemilik/ Operator kapal yang mungkin
ingin dilakukan sebelum pemeriksaan dimulai untuk
mengurangi jumlah pengamatan tidak kesesuaian.
OVIQ adalah tentang kapal-Sistem Manajemen
Keselamatan di darat & di kapal dan
implementasinya. Berikut adalah beberapa saran
tentang apa yang Pemilik/ Operator kapal mungkin
ingin mempertimbangkan sebelum melakukan
pemeriksaan OVID:

1. Pemeriksaan sendiri untuk OVID
Lakukan pemeriksaan sendiri untuk OVID.
OVIQ Template (contoh) dapat dimanfaatkan
sebagai referensi. Template ini tersedia di
website OVID OCIMF.

2. Internal Audit ISMS
o Lakukan Audit Internal ISMS di kapal. Cari
ketidaksesuaian dan mulai memperbaiki
mereka sebelum pemeriksaan OVID.
Pastikan bahwa semua item Internal Audit
ISMS ditangani dan ISMS dilakukan.
o Meninjau Eksternal Audit ISMS terakhir
dan memperbaiki semua ketidaksesuaian
sebelumnya.
Jika kapal tidak memiliki ISMS ter-
implementasi karena di bawah
persyaratan aplikasi Kode ISM kemudian
pastikan bahwa ada Sistem Manajemen
Keselamatan di kapal dan dilaksanakan
untuk mencakup semua resiko
keselamatan.

3. Equipments check
o Make sure that all equipments on board are
fully operational and working as per design
approved by Flag State and Classification.
o If a piece of equipment is not operational
then ensure that in line with your ISMS
system the defect has been reported to shore
Management by vessel staff. Shore vessel
Management should have replied and a
mitigation plan for repair needs to be in
place e.g. when the spare parts ordered?
When will the spare parts arrive onboard
vessel? When will the support team join to
affect a permanent repair or has it been
included in the dry dock repair list?

4. PPE
o Ensure adequate valid in good order of PPE
is available on board and worn by all staff as
its required to get the job safe executed.
Seeing personnel out on deck with no work
suit, gloves, hard hat, safety shoes, or eye
protection is a real bad start for any
Inspector when he first boards vessel. Flip
flops, shorts and smoking will give a picture
to an Inspector that the crews have a very
low level of safety awareness and
Management possible doesnt care.

o Is the shore gangway properly rigged can
the Inspector get on board safely?
When the Inspector first boards ensure they
sign the Security Register, get a Visitors
Pass and receive a Safety Briefing -- we are
creating a safety culture that the Inspector
can see and feel.





5. Flag State and Class certification
Ensure that all Flag State and Class certification
is carried on board the vessel. While the original
certificates must be held by the Port Authority
once a vessel is based in a Port, vessel must keep
a photocopy of the certificates or take the
Inspector to view the originals as part of the
inspection.






3. Pemeriksaan peralatan
o Pastikan bahwa semua peralatan di kapal
beroperasi penuh dan bekerja sesuai desain
yang disetujui oleh Negara Bendera dan
Klasifikasi.
o Jika sebuah peralatan tidak dapat
dioperasikan kemudian pastikan bahwa
sejalan dengan sistem ISMS, kondisi
kerusakan telah dilaporkan ke manajemen
darat/ kantor. Manajemen darat harus
menjawab dan melakukan rencana mitigasi
untuk perbaikan, misalnya kapan suku
cadang diminta? Kapan suku cadang akan
tiba di kapal? Kapan tim dukungan dari
darat ke kapal untuk melakukan perbaikan
permanen ataukah telah dimasukkan dalam
daftar perbaikan dok?

4. APD
o Pastikan APD yang memadai, sah, dalam
kondisi baik tersedia di kapal dan dipakai
oleh semua staf sesuai keperluan untuk
mengeksekusi pekerjaan dengan aman.
Melihat personel keluar di dek tanpa baju
kerja, sarung tangan, topi pengaman, sepatu
pengaman, atau pelindung mata adalah awal
yang buruk dan nyata untuk setiap Inspektur
ketika ia pertama kali naik ke kapal. Sandal
jepit, celana pendek dan merokok akan
memberikan gambaran kepada Inspektur
bahwa kru memiliki tingkat yang sangat
rendah akan kesadaran keselamatan dan
Manajemen kemungkinan tidak peduli.
o Apakah shore gangway terpasang aman -
apakah Inspektur bisa naik kapal dengan
aman?
Ketika Inspektur pertama naik kapal,
pastikan mereka menandatangani buku
tamu, mendapatkan kartu pengunjung dan
menerima briefing keselamatan - kita
menciptakan budaya keselamatan yang
Inspektur dapat melihat dan merasakan.

5. Bendera Negara dan sertifikasi Klas
Pastikan bahwa semua Negara Bendera dan
sertifikasi Klas ada di kapal. Apabila
sertifikat asli harus dipegang oleh Otoritas
Pelabuhan saat kapal di wilayah pelabuhan,
kapal harus menyimpan fotokopi sertifikat
atau member petunjuk Inspektur untuk
melihat yang asli sebagai bagian dari
pemeriksaan.

6. Machinery guards.
Please ensure that these are all in place.

7. Navigation
o Navigation procedures must be available on
board and followed.
o Make sure that there is a valid statement
posted on the Bridge about safety entry into
the 500m zone of any offshore installation.
o In your procedures ensure that there is a
statement about minimum under keel
clearance for any offshore support vessel.

8. BBS or its similar system
o All crews must be using BBS or its similar
system in line with company Policy. Vessel
chartered by Chevron expected to has in
place a BBS or similar system. This is
minimum Chevron OE HES expectation.
o A proactive vessel Manager will ensure that
this system is actually being used by those
on board. This creates a positive safety
impact for the Inspector and generates a
better picture of the company managements
approach to safety.


9. Company SWA Policy onboard.
Every person on the vessel must be authorized
and responsible to use SWA at any time for any
un-safe condition or un-safe action without any
negative repercussion. If work has been stopped
using SWA then keep a record on board to show
the Inspector.

10. Company PTW and LOTO system
o Ensure the PTW and LOTO system in use
on the vessel. Vessel Shore Management
will ensure the PTW and LOTO are being
used. Answers to an Inspector advising our
system is carried on board does not say the
system is in use.
o Hot work permits, electrical permits,
confined space entry permits, etc. as
required must be used.










6. Pelindung mesin.
Pastikan bahwa Pelindung Mesin terpasang.

7. Navigasi
o Prosedur navigasi harus tersedia di kapal
dan diikuti.
o Pastikan bahwa ada pernyataan yang valid
diposting di Anjungan tentang masuk ke
zona aman 500m dari instalasi lepas pantai.
o Dalam prosedur, pastikan bahwa ada
pernyataan tentang berapa minimum jarak
dibawah lunas untuk keselamatan setiap
kapal pendukung lepas pantai.

8. BBS atau sistem yang mirip
o Semua kru harus menggunakan BBS atau
sistem serupa sejalan dengan Kebijakan
perusahaan. Kapal yang disewa oleh
Chevron diharapkan memiliki sistem BBS
atau sistem serupa. Ini adalah harapan
minimum Chevron OE HES.
o Manajer kapal proaktif memastikan bahwa
sistem ini digunakan oleh orang-orang di
kapal. Hal ini untuk menciptakan dampak
keselamatan positif bagi Inspektur dan
menghasilkan gambaran yang lebih baik dari
manajemen perusahaan untuk keselamatan.

9. Kebijakan SWA Perusahaan Dikapal.
Setiap orang di kapal berwenang dan
bertanggung jawab untuk menggunakan SWA
setiap saat untuk setiap kondisi tidak aman atau
tindakan tidak aman tanpa feedback negatif. Jika
pekerjaan telah berhenti menggunakan SWA
kemudian simpan catatan di kapal untuk
ditunjukkan ke Inspektur.

10. Sistem PTW dan LOTO Perusahaan
o Pastikan sistem PTW dan LOTO digunakan
di kapal. Manajemen darat akan memastikan
PTW dan LOTO digunakan. Jawaban ke
Inspektur menasihati sistem tersedia di kapal
tidak mengatakan sistem sedang digunakan.
o Izin kerja panas, izin listrik, izin masuk
ruang terbatas, dll yang diperlukan harus
digunakan.

11. Official Log books
Log books for Deck, Engine Room, Waste
Management, Oil Record Book, GMDSS, Master
and Chief Engineer night order book etc., must
be completed. Ensure all Officers have signed
vessel log books and maintained full and
complete records of all activities. Do not falsify
records.
This is easy for the Inspector to check and gives
a good impression of a well run vessel with on
board Leaders who take responsibility for their
actions.

12. DP Vessels
o Ensure DP system is in class.
o Complete DP foot prints regular.
o Complete DP check lists and keep historical
records on board.
o Conduct annual DP FMEA Verification with
3rd party Inspector and clear all non
conformances. All Officers should read and
sign that they have read the FME(C)A
(including Engineers).
o Complete DP Maintenance records and
document every failure. Make sure
equipment failures have been reported to
shore management.
o Close all A and B level items from DP
Audits.

13. Confined Space Entry
Vessel having a procedure and equipment
necessary for a safe entry to take place. Ensure
that the procedure is implemented and followed.
All spaces must be marked with their identity
and content.

14. Company Drug and Alcohol Policy
Ensure Drug and Alcohol policy is implemented
and that all personnel are tested in line with
companys policy.

15. Minimum Safe Manning certificate
If required by the Flag State requirement and
endorsement, ensure Minimum Safe Manning
certificate is available and manning levels meet
or exceed the minimum requirements.








11. Log Book resmi
Log book untuk Deck, Engine Room,
Pengelolaan Limbah, Buku Catatan Minyak,
GMDSS, Buku perintah malam Kapten dan
Kepala Kamar Mesin dll, harus dikerjakan.
Pastikan semua Perwira telah menandatangani
Log Book kapal dan mencatat penuh & lengkap
semua kegiatan. Jangan memalsukan catatan.
Ini mudah bagi Inspektur untuk memeriksa dan
memberikan kesan yang baik dari sebuah kapal
yang dikelola oleh para pemimpin yang
bertanggung jawab atas tindakan mereka.

12. Kapal DP
o Pastikan sistem DP sesuai Klas.
o Lengkapi foot print DP secara teratur.
o Lengkapi checklists DP dan simpan di kapal.
o Melakukan verifikasi tahunan DP FMEA
dengan Inspektur pihak ke-3 dan melakukan
perbaikan atas semua ketidaksesuaian.
Semua Perwira harus membaca dan
menandatangani bahwa mereka telah
membaca FME (C)A (termasuk Engineers).
o Melengkapi catatan pemeliharaan DP dan
mendokumentasikan setiap kegagalan
peralatan. Pastikan kegagalan peralatan telah
dilaporkan ke Manajemen darat.
o Tutup semua item tingkat A dan B dari
Audit DP.

13. Confined Space Entry
Kapal memiliki prosedur dan peralatan yang
diperlukan untuk masuk yang aman. Pastikan
bahwa prosedur dilaksanakan dan diikuti. Semua
ruangan harus ditandai jelas dengan identitas dan
isi.

14. Kebijakan Obat dan Alkohol Perusahaan
Pastikan kebijakan Obat dan Alkohol
diimplementasikan dan bahwa semua personel
diuji sesuai dengan kebijakan perusahaan.

15. Sertifikat Minimum Safe Manning
Jika dipersyaratkan oleh Negara Bendera dan
persetujuan, pastikan sertifikat Minimum Safe
Manning tersedia dan tingkat berjaga memenuhi
atau melebihi persyaratan minimal.

16. Security Plan
Ensure Security Plan is available on board and it
is being implemented. Approved Flag State SSP
for any vessel require for ISPS Code compliance.

17. Management of Change
Ensure MOC process is part of the Safety
Management System and it is implemented.

18. Safety Drills on ships
Implement a regular schedule for drills and
ensure vessels crews complete these drills, such
as for Emergency Drills, Lifeboat Drills,
Environmental loss of containment drills,
Security drills, Fire drills, Man over boards
drills, Emergency steering.

19. LSA and FiFi equipment
Ensure it is well maintained & working, defects
are reported and documented to shore
management for mitigation.

20. Stability Calculations
Complete stability calculations on a regular basis
and keep records.

21. Mooring equipment
Are those in good condition?

22. Lifting appliances
o Do you have certificates required on board
for all loose lifting equipment?
o Is your register of chains up to date?

23. Chemical storage
Store chemicals safely in a chemical locker with
complete MSDS.

24. General house-keeping
o Shore Based Managers must have regular
schedule to visit and walk round the vessel
and inspect for cleanliness and record these
walk round in the log books.
o Clean up any minor spills and leaks. Keep
the bilges dry.
o Make sure that safety equipment is available
for use with work shop machinery.
o Dont store spare parts and equipment so it
obstructs emergency escape routes.




16. Rencana Keamanan
Pastikan Rencana Keamanan tersedia di kapal
dan dilaksanakan. SSP yang disetujui Negara
Bendera untuk kapal diperlukan untuk
memenuhi kepatuhan akan ISPS Code.

17. Manajemen Perubahan
Pastikan proses MOC merupakan bagian dari
Sistem Manajemen Keselamatan dan
diimplementasikan.

18. Drills Keselamatan di kapal
Menerapkan jadwal rutin untuk latihan dan
memastikan kru kapal menyelesaikan latihan,
seperti untuk Latihan Darurat, Latihan Lifeboat,
Environmental loss of containment drills,
Latihan Keamanan, Latihan Pemadaman Api,
Latihan Penyelamatan Orang Jatuh ke Laut,
Latihan Kemudi Darurat.

19. LSA dan peralatan FiFi
Pastikan terpelihara dengan baik & bekerja,
kerusakan dilaporkan dan didokumentasikan ke
Manajemen darat untuk perbaikan.

20. Perhitungan Stabilitas
Lengkapi perhitungan stabilitas secara teratur
dan simpan sebagai catatan.

21. Peralatan Mooring
Apakah peralatan dalam kondisi baik?

22. Peralatan Angkat
o Apakah sertifikat yang diperlukan tersedia
di kapal untuk semua peralatan angkat?
o Apakah sertifikat up to date?

23. Penyimpanan bahan kimia
Simpan bahan kimia dengan aman di tempat
penyimpanan dengan MSDS lengkap.

24. General house-keeping
o Manajer di darat harus memiliki jadwal rutin
untuk mengunjungi, menginspeksi dan
memeriksa kebersihan kapal dan tertulis dalam
log book.
o Bersihkan tumpahan dan kebocoran kecil.
Pastikan bilges kering.
o Pastikan bahwa peralatan keselamatan tersedia
untuk digunakan di workshop mesin.
o Jangan menyimpan suku cadang dan peralatan
sehingga menghalangi rute darurat melarikan
diri.
