    undertake that I have qualified the UPSEE-2014 examination and want to appear in the
    counseling of UPSEE-2014.

    (Please tick ( ) the point(s) below, whichever is applicable and put a cross ()
    whichever is not applicable)
    1. I have passed qualifying examination and result of the examination has been declared but
    I have not received the Original Mark Sheet so far. I am submitting the attested Internet
    Copy of the Result. As per result, I fulfill the eligibility requirement of UPSEE-2014.
    2. I have appeared in qualifying examination conducted by the concerned Board/ University
    but result of the examination has NOT been declared so far.
    3. Final examination of my qualifying examination by the concerned Board / University has
    not been conducted so far but is about to be conducted soon.
    4. I belong to the Category --------------- (UPBC/UPSC/UPST/GDBC/GDSC/GDST/GDDA)
    and required certificate submitted by me as a proof of category is not as per format
    mentioned in the Information Brochure.
    5. I belong to the subcategory ---------------- (UPAF/UPFF/UPHC) but required certificate
    submitted by me as a proof of subcategory is not as per format mentioned in Information
    Brochure.
    6. I do not have my Character Certificate from the Institution last attended.

    7. (Only for PG Program candidates) I have passed required qualifying examination
    (Graduation) but the percentage of marks obtained by me in the qualifying exam
    (graduation) is below 50 percent (45 percent in case of SC/ST candidates) and I
    understand that I am eligible only for those State Universities of UP other than U.P.
    Technical University, Lucknow, where admissions are allowed as mentioned in the
    Information Brochure. I shall fill up the choices during counseling only for those
    Universities where I am eligible for admission.
