Construction Phase Health & Safety Plan

The Construction Phase Health and Safety Plan is a working document which will
be continuously reviewed and updated to reflect the current project status by the
Concorde Build + Developments Ltd Project Management team. Any information
received or actions suggested by any of the project team, including designers/CDM
Coordinator/Subcontractors, will be reviewed and implemented into the plan if
appropriate. If any residual risks are highlighted throughout the project they will
also be included within the document. All duty holders will be notified of any
changes to the plan

1. Description of the Project & Programme Details

1 [a] Location of the Works:


HMGCC, Hanslope Park, Milton Keynes, MK19 7BH

1 [b] Detail of the Work:


Refurbishment of existing office space to create welfare and café facilities within a 2
storey building.

Works comprise alterations to the existing first floor areas and external works
including installation of new light fittings, notice boards and temporary screens,
alterations to existing internal walls, removal of doors, stud walls and ceiling finishes,
timber frames and window surrounds; installation of new suspended ceilings, sanitary
ware, heating and fire alarm systems, ventilation and emergency lighting; the
provision of loose furniture, new finishes to walls and ceilings, minor alterations to
the existing corridor to staircase and toilets, installation of new external awning,
internal and external decorations and all associated mechanical and electrical works.

A scaffold loading area is to be constructed on the balcony area for the transferring of
materials using pallets and mobile plant or alternatively materials will be transferred
to site using a hoist and scaffold system. (Yet to be confirmed)

Towards the end of the project it is possible permission may be granted for the use of
the internal lift to transfer heavy or large appliances. (Yet to be confirmed)

2. Communication & Management of the Work


2 [a] Management Structure & Responsibilities:

Responsibilities:
To manage all aspects of the project including the production of all necessary health
& safety documentation, reviewing of subcontractor health & safety documents the
appointment of site management and personnel, allocation of resources and
equipment, the appointment of subcontractors, together with the general overseeing of
the project.

Ensure all health and safety defects that have been highlighted during site inspections
have been addressed. Ensure all RIDDOR accidents are reported to the HSE.

The day to day management and administration of health, safety and environmental
issues associated with the project. Supervising of all site personnel and subcontractors
and client appointed direct contractors, taking charge during cases of emergency. The
recording and reporting of all accidents and near miss incidents to the Project
Manager, and arrange for accident / near miss investigations to be carried out as
required.

Responsibilities:

To take charge of the site during the absence of the Site Manager, supervise site
operatives at the work face and monitor subcontractor work activities. During the
absence of the Site Manager take charge in cases of an emergency.

Responsibilities:
To assist with the production of project health & safety documentation, carryout
regular site health, safety and environmental inspections, providing a written health &
safety report for the Project Manager and Site Manager. Carryout site accident
investigations, report findings to the Project Manager for appropriate actions.

2 [b] Health & Safety Goals & Monitoring of Performance:


It is the intention and ultimate goal of the Principal Contractor to complete the project
with zero reportable accidents. This goal will be achieved by promoting a positive
safety culture during the project together with the implementation of strict health,
safety and environmental standards that will be monitored by the project management
team and our independent health and safety advisors. Every effort will be made to
ensure the clients health and safety goals are met. MECsafe Limited will visit the site
on a regular basis providing a written report on the safety performance of the site
workforce including subcontractor activities, site conditions and general working
practices. Copies of the report will be left with the Project Manager who will review
any issues with the project team.

2 [c] Liaison with Parties on Site:


An initial pre start meeting will take place prior to the start of construction and shall
include a site specific induction for all operatives and an explanation of the Site
Rules, Fire Plan and Traffic Management Plan. Further pre-start health & safety
meeting will take place with any subcontractor appointed after the initial meeting;
they will be required to submit all the relevant health and safety documentation
applicable to their individual works. Regular progress meetings will take place on site
once per week, health and safety issues will form an integral part of the agenda. When
deemed necessary subcontractors will be required to attend the progress meetings
and / or will be issued with minutes of any such meetings.

2 [d] Consultation with the Workforce:


All site employees will be notified of the procedure to adopt should they wish to
discuss any matter of site health & safety. The procedure will be explained to all site
workers during their pre-start health, safety and environmental induction. In the first
instance consultation will be via direct discussion with the Site Manager or if a
subcontract employee, with their immediate supervisor, all issues raised by the site
workforce will be documented in the Site Managers daily diary and raised for
discussion during the regular progress meetings. Information on health & safety will
be directed to the site workforce via the site notice board and / or disseminated via a
formal tool box talk session. When requested, the appointment of a workforce health
& safety representative will be encouraged.

2 [e] Coordination of Design Information:


Design issues and the possibility of health and safety interfaces will be discussed at
the regular progress meetings, relevant parties will be requested to attend this meeting
including designers and subcontractors when information will be passed on to the
appropriate parties, when necessary the project CDM Coordinator will be invited to
the progress meeting and / or will be forwarded the relevant information as required.
Design changes during the project will be subject to approval by the Lead Designer /
Architect, design changes will be discussed at the regular progress meetings, proposed
amendments to the original design will be forwarded to the CDM Coordinator for
comment.

2 [f] Selection and Control of Subcontractors:


All subcontractors appointed by the Principal Contractor are required to meet a basic
level of competency in Health & Safety. Subcontractors are required to provide
evidence of competency and training records to the company prior to the placing of
orders. The minimum criteria which each subcontractor must satisfy are. A health &
safety policy detailing the organisation and arrangements for managing health &
safety, a named company health & safety advisor, health & safety training and
competency details of supervisory staff and operatives. All subcontractors will be
required to provide a safety method statement and risk assessment for their individual
works packages. The documentation provided will be vetted and approved by the
Project Manager. Work methods will be monitored by the Site Manager in accordance
with the safety method statements which have been provided by the respective
subcontractors. Further monitoring of subcontractor activities will be carried out by
MECsafe Limited during the site health, safety and environmental inspections.
Subcontractors will be monitored continuously throughout the works to ensure
relevant health and safety information including Risk Assessments and Method
Statements are provided to the operatives.

2 [g] Exchange of Health & Safety Information with Subcontractors:


Where necessary subcontractors will be required to attend bespoke meetings with the
Project Manager / Site Manager to resolve significant health & safety issues
connected with the project. Information may also be provided by memo on the site
notice board and / or individual letters to respective subcontractors. Design issues will
normally be addressed at the regular progress meetings. Relevant Health and Safety
Information such as the F10, Construction Phase Health and Safety Plan, Traffic
Management Plan, Fire Plan and Site Rules will be displayed on the site notice board
which will be located in the site office.

2 [h] Site Security:


Contractor storage of small plant, equipment and materials is to be established within
the site compound which will be securely locked when left unattended. Safety signs
and notices are to be displayed at strategic access and egress points. A sign in / out
book will be kept at the entrance to the site and all personnel and visitors entering the
site will be required to sign in and sign out on leaving.

2 [i] Induction & “On Site” Training:


Prior to entering the work site all personnel and visitors will receive a health, safety
and environmental induction. On site, task specific health & safety training will be
provided to operatives as required, when necessary, tool box talks will be used as a
means of communication to continuously promote a positive safety culture and will
be periodically given to all site operatives and subcontractors by the Site Manager and
/ or MECsafe Limited during their site inspection visits.

[j] Welfare & First Aid Arrangements:


A secure site compound will be set up directly outside the work area ensuring access
and egress routes for staff and visitors to the building are kept clear at all times.
Within the compound portable cabins are to be utilised as office, mess & changing
facilities, with separate contractor’s toilet / hand washing provision. There will be a
nominated person amongst the workforce as site first aider (Section 2k).
Subcontractors and client direct contractors requiring to work out of normal site hours
will be required to provide a trained first aid person. A first aid box will be located in
the site office, all site personnel will be briefed on the site first aid arrangements
during the pre-start induction session.

2[k] Accident Reporting / Investigations:


All accidents will be reported to the Site Manager and recorded in the site accident
book together with the completion of the company accident reporting/ investigation
form. Minor accidents and injuries will be investigated by the Site Manager and
recorded on the company accident / investigation form. Major accidents and injuries
will be reported in the first instance to the Site Manager, to the HSE by telephone, and
then to MECsafe Limited who when required, will carry out a thorough investigation.
All major accidents, over three day injuries and dangerous occurrences will be
reported in compliance with RIDDOR 1995. All major injuries and dangerous
occurrences will also be reported to the Project CDM Coordinator.

……………….. will be the nominated person amongst the workforce acting as Site
First Aider. ………………… has the 3 Day HSE approved First Aid qualification. In
his absence from site an equally qualified and competent member of staff will operate
as site First Aider. Subcontractors and client direct contractors requiring to work out
of normal site hours will be required to provide a first aider with a valid 1 day
emergency first aid certificate for 5 or less operatives and a 3/4 day HSE approved
qualification if 6 or more operatives are present on site.

2 [l] Risk Assessments & Safety Method Statements:


All site activities where significant risks have been highlighted will require a formal
written risk assessment to be prepared. Work tasks that may involve a high risk
activity will have a formal written safety method statement that will require approval
from the Project Manager prior to the commencement of the work. All project safety
method statements and risk assessments will be filed in the relevant sections and
subcontractor sections of the “site safety administration folder” the information will
be made available for anyone to consult these documents.

2 [m] Site Rules:


Refer to the appendices for a copy of the site rules, the site rules will be explained to
the operatives during the pre-start induction session, a copy of the rules will also be
displayed on the site notice board. Subcontractors will be issued with a copy during
the pre-start safety meeting with the individual subcontractor companies.

2 [n] Fire and Emergency Evacuation Procedure:


The client’s fire and emergency procedure in compliance with regulations 38, 39, 40
and 41 of CDM 2007, will be explained to operatives during the pre-start induction
session, a copy of the procedure will also be displayed on the site notice board and
around the work area as appropriate. As work progresses, there may be additional
hazards with potential impact to access and egress routes requiring the procedure to
be reviewed and updated. All hot work operations will be subject to specific
management controls and permit to work system.

2 [o] Permits to Work System:


A permit to works system will be in operation for all hot works, Compressed Air and
Gas Systems, Live Electrical Apparatus Work, Lone Working and Confined Space
Entry

3. Arrangements for Controlling Significant Site Risks


3 [a] Safety risks:

Site Traffic Movements / Delivery Vehicles:


Access to the site is through the main entrance and via the client’s security checks.
The site work area will be accessed via an external staircase from the site compound
which is to be erected for the purpose of access and egress to site. All delivery
vehicles will be co-ordinated through the Site Manager or his deputy, with times
arranged to prevent queuing on the public highway and conflict with adjacent
properties and working practices within HMGCC. Upon arrival, delivery / materials
removals vehicles will be escorted onto site for offloading with a banksman used
whenever appropriate.

Services including temporary electrical installations:


All site electrics will be maximum 110 volts. Where existing underground live
services are present the cables and pipes will be identified and where possible marked
above ground. Suitable signage and or barriers will be positioned to exclude work
equipment that is not needed, including suspended protection where vehicles are
required to pass beneath cables.

Prevention of falls:
Any excavations within the work site which is a potential location where a person
may fall will have a barrier erected around the open edge and / or the area will be
suitably cordoned off to prevent access. A permit to work system will be in operation
for the use of ladders and step ladders, where possible podium steps and / or mobile
towers are to be used in preference to the use of step ladders when accessing work
areas at heights.

Water, Electricity and Gas Services:


All services are to be identified with any that are required to remain “live” being
assessed and marked accordingly. Temporary electrical connections for the site /
cabins are to be housed in a lockable unit and marked with the relevant warning
signs.

Maintenance of Plant & Equipment:


All plant, equipment and tools brought onto site must be properly selected and
maintained in compliance with the Provision and Use of Work Equipment
Regulations. All mechanical lifting devices must have the relevant test & inspection
certification. All electrical plant, tools and equipment must be well maintained and
tested. Access equipment such as ladders and mobile aluminium scaffolding towers
must be in good condition and have been inspected for defects prior to its arrival on
site. Tools, plant and equipment will be subject to spot checks for condition,
maintenance certification by the Site Manager and / or during site safety inspections
by MECsafe Limited.

Ground Conditions:
A Ground Investigation Report will not be required due to the nature of the
construction project. The immediate area surrounding the building is tarmac.

Storage of Hazardous Substances:


All hazardous substances will be kept in marked containers, when not in use they
must be kept in the store cabin. Petrol containers will not be allowed to be stored
inside any building, all petrol containers must be stored in a flammable vault / or
similar, on no account must petrol containers be left in the office / mess cabin areas.
Flammable gases must be stored upright in the open air in a caged store. If diesel oil is
required on site it must be stored in a bunded area or a bunded container.

Traffic Routes / Pedestrian Access / Materials Deliveries/Waste:


There is one contractor parking space available although contractors will be advised
prior to commencement of works on site to park off site and access the HMGCC
through the designated pedestrian access. Access to site is to strictly controlled by
HMGCC. All operatives will be made aware of all security arrangements in the pre
start induction. All materials deliveries will be co-ordinated through the Site Manager
who will obtain permission beforehand from the HMGCC representative. The
necessary directional and safety signs will be displayed at the entrance to the site.
Thoroughfares are not to be obstructed by deliveries and waste removals or by
parking at any time during the works.

Site Specific Risks:


Risk control measures are to be adopted for the activities listed below and must be
incorporated within safety method statements and risk assessments for the individual
subcontractor work packages:-

Traffic Management / Delivery Vehicles

Joinery Works: Noise, Dust, Work at Height, Manual Handling of Materials
Plumbing Works: Manual Handling, Live Services, Capping off Services
M & E Installations: Manual Handling, Commissioning & Live Services
New Floor & Ceiling Coverings: Manual Handling, Materials Delivery, Work at
Height
Removal of Stud/Partition Walls: Work at Height, Dust, Noise & Vibration
Installation of Awning: Work at Height, Materials Storage
Painting & Decoration: Work at Height, Handling & Storage

The above will all be referred to within site specific risk assessments and safety
method statements which can be accessed from the Site Manager.

3 [b] Health Risks:

Asbestos:
A full type 3 Asbestos survey is to be commissioned prior to the commencement of
works. When available a copy of the results will be inserted into appendix 3. If any
further asbestos is identified during the refurbishment, work must be stopped
immediately and appropriate action under the Control of Asbestos Regulations 2006,
shall be taken.

Hazardous Materials & Substances:


Materials and substances to be used on the project are those normally used in
traditional construction methods, all materials / substances used will require an
accompanying COSHH assessment, these assessments will be available for inspection
in the “site safety administration folder” located in the Site Managers office.

Manual handling:
Where necessary manual handling assessments will be produced, in all cases
mechanical means will be the first option for use when transferring materials around
site. Refer to risk assessments and method statements for further information.

Noise & Vibration:


If required a noise survey will be carried out on site the results of which would be
made known to site operatives. For noise above 80 dbl but below 85dbl, ear protection
will be available for operatives use. Operatives and general site personnel who are
engaged in or working adjacent to activities that are generating high noise levels
above 85 dbl will be required to wear ear protection. Where possible, noise will be
kept to a minimum level by use of noise suppressing equipment on tools and
machinery and also by careful selection of machine type and work method used.
Where plant and tools that generate high levels of vibration have to be used, they shall
be appropriately assessed and identified control measures implemented. The site
manager will obtain from the contractors noise ratings of all plant and machinery and
records will be kept in the health and safety file.

Dust Control:
Regular cleaning will take place by Concorde operatives. Temporary protection over
fixtures, fittings and equipment will be used when necessary.

Personal Protective Equipment


All personnel working on the construction site will wear high visibility jackets/vest,
Safety Hat and Safety Footwear. Other specialist safety clothing and equipment will
be issued as site work dictates through risk assessment and safety method statement.

Lighting:
Where the level of lighting is not adequate for a specific task then external lighting
will be provided.

3[c] Environmental Risks


No other significant environmental issues associated with the project other than those
already mentioned in the plan and those risks normally associated with general
construction activities.

4. The Health & Safety File:


All required information is to be developed, collated and issued by the Principal
Contractor as it becomes available and forwarded to the CDM Coordinator as the
project progresses. The Health and Safety File and provision of information will be
monitored by L. Dickinson. Information from subcontractors will be requested by the
aforementioned when there works commence on site.
