General Impression (appearance upon first contact):
The client is alert, awake with eyes open and looks at the student nurses. He also responds appropriately and oriented to time, place, and
person. He appears to be stated chronological in his age with cooperative attitude and behavior. Furthermore, he is dressed appropriately for
the place and weather. The client has an erect posture with smooth, coordinated, and steady gait. Hygiene is good with clean skin and nails
trimmed. He establishes recurrent eye contact to the student nurses and smiles appropriately. His speech is clear with moderate pace and
comprehensible and expresses full and free-flowing thoughts and answers to questions are based on his rationales.

The patient interview provides the gateway to the health of the patient. Because it is both a science and an art, the skill of
interviewing will improve as you continue your training. Although psychiatric patients differ in some ways from medical and surgical
patients, seating arrangements and use of space by patients can provide valuable information to the nurse, clarifying not only the
nurses appraisal of the patient but also the way in which the nursing interventions should be implemented. That is why it is very
crucial to let the patient choose what seating arrangement and where he is comfortable at conversing, given that it is accessible to
both the nurse and the patient. Offering a choice of seating arrangements allows greater flexibility in the interaction.
As shown in the figure above, the patient decided to sit on a wooden bench under the shade of a tree. Moreover, the student
nurses brought their seats about 3 to 4 feet away from the patient which facilitates interaction to be audible to both without invading
personal space and promotes social interactions. The patient may perceive the nurse-client interaction as a threat if his personal
space is violated therefore it is very decisive for the student nurses on-duty to maintain the social space. One student nurse is seated
lateral to the patient while the other is seated in front of the patient so to provide perimeter to which the conversation is contained.

Being of service in helping our patient cope up with the different stressors in life to help him be gainfully productive to the community where
he belongs is one of our prime motivations. For us to do this, it is our sincere desire to render our service and share the best of our abilities
and knowledge in guiding our patient gained back sound way of thinking and desired behaviour he used to have. Also, it will be an
experienced were hoping to have and develop the skills needed to become a better health care provider in this area.

We first desired to established rapport and accomplish contract setting for us to formally start our journey of mutual partnership wherein the
patient developed coping skills and we, student nurses, gained the lived experienced. This is not an easy task ahead of us, but, we are
hopeful and confident enough with the help our mentor and above all, the almighty God.

We desire to accomplish contract setting and foster an atmosphere where there is acceptance, understanding, genuine care, open
communication and trust for each other. Important points to consider where emphasized during the first meeting. Expectations from us were
laid out to our patient and we also asked our patient what he is expecting from us during the entire psych rotation that we will be with him.

Synthesis:


Our conversation with our patient revolved around establishing an atmosphere of acceptance, understanding, open communication and level
of trust and contract setting wherein we laid out our objectives and purpose of coming to take care of him during our Psyche rotation. The
contract setting is mutually beneficial. It is our objective that our patient will gain back his coping skills and be gainfully productive and
functional while, we the student nurses, gained the lived experience of taking care of patient with maladaptive behaviour and be of help in
guiding the patient lead a normal way of life.

We informed him that were coming every Friday morning, unless there will be changes, we will let him know ahead of schedule. Therapeutic
communication would last for 45 minutes and we expect him to be prepared for the conversation by doing his self-morning care, eating
breakfast, changed into clean clothes so we could start right away when we come and make the most of our limited time. We also informed
him that there would be times that apart from the regular 45 minutes conversation, we will have group activity for everyone with assigned
student nurses to interact among them. We give much emphasis of the importance of his participation and cooperation by sharing with us
what we will be asking for from him. Patient was informed that all information gathered would be dealt with utmost confidentiality. Only
people taking care of him will have access to the information like the doctor, nurses, our clinical instructor, and our classmates when we
present his case but rest assured your personal identity will be kept confidential. We will use pseudo name. Other health team members will
get to know if they directly involve in his care.

We also asked our patient his expectations from us and said none. Termination of our contract was also discussed with our patient. We had
our initial interview with our patient right after we had contract setting with him. We are glad our patient openly and elaborately shared with
us how it started. For a limited time, we already had an overview of how it started. We thank our patient for the time and we bid goodbye.
