Petitioner, Atty. MARIA FELICITAS V. ELE, in support of her petition for
renewal of appointment as Notary Public for and in the City of Makati City, respectfuliy
alleges that:
1' She is a citizen of the Philippines, of legal age, aresid.ent of Unit 1116 A. Venue
Suites, Makati Avenue, Makati City, and with office address at Legal Services Group,
L4ft Floor, BDO North Tower, BDO Corporate Center, 7899 Makati Avenue, Makati
City. The location sketch of the petitioner's office is attached and marked as Annex "A"
hereof;
2. She passed the Bar Examination and has been admitted to practice law in
Philippines on 04 May 2009, as shown by the Certificate of Membership in
Philippine Bar, attached and marked as Annex "B" hereof;
3. She is a member of good standing of the Philippine Bar as shown by the
Certification dated 2L December 2011,, issued by the Office of the Bar Confidant of the
Supreme Court, a copy of which is attached and marked as Annex "C" hereof.;

attached as Annexes "D" and"E", respectively, and forming integral parts hereof;
5. Likewise attached and marked herewith as Annex "F" LS the Certification of
petitioner's good moral character issued by Maria Teresita C. Sison Go and Dennis A. B.
Funa, National Treasurer and Director for Bar Discipline, respectively, of. the IBP
National Office;
6. Her Tax Identification No. is 210-557-186.
7. Her notarial commission with Appointment No, M-427 expired on 3L
December 2011., Hence, she is filing this Petition for Renewal with this Honorable
Court;
8. She possesses all the qualifications and none of the disqualifications for the
office of the Notary Public;
9. She has never been charged nor convicted of any crime involving moral
turpitude;
L0. She has no other commission as Notary Public in any jurisdiction;
LL. She is not a government employee;
12. She has attached hereto in a separate sheet of paper specimen of her official
signatures and marked as Annex "G" and made an integral part hereof;
13. She has likewise attached hereto her three (3) passport size color photographs
each bearing her signatures on the bottom part thereof, marked as Annex "H"; and,
14. Further, petitioner has complied with the required 36 units of Mandatory
Continuing Legal Education (MCLE) under Bar Matter No. 850, as shown by the
Certificate of Compliance with MCLE Compliance No. III
