This Affidavit/Declaration of Truth is lawful notification to you, and is hereby made and

Affidavit/Declaration, is your lawful, legal and binding agreement with and admission to
the fact that everything in this Affidavit/Declaration is true, correct, legal, lawful, and

In /une of 0,(,, I received a frivolous filing letter from the Internal %evenue 1ervice for
my 0,,2 ta3 returns The letter claimed that I owe the I%1 over 4005,,,, I )uickly
res!onded via certified letter or the like to A6ENT NAME, Internal %evenue 1ervice
Agent, in 7ontiac, Michigan, that this was not true and that I disagreed I asked to be
!rovided with facts, law, and evidence for which they were basing this claim The letter I
received in res!onse from A6ENT NAME, stated that my statements and )uestions
regarding the claim of the Internal %evenue 1ervice were frivolous and would not
warrant a res!onse In August of 0,(,, I sent another letter to this same A6ENT NAME
further re)uesting by fact and law the foundation for the claim and included my lawful
!osition su!!orting my not having any obligation to the Internal %evenue 1ervice
including my being denied due !rocess of law I received a follow8u! letter from
A6ENT NAME that stated that I owed the money, no lawful foundation was !resented,
and a date which I needed to !ay by or a levy and garnishments would start
6arnishments and levy soon started a!!ro3imately August of 0,(, and every attem!t to
sto! this from continuing des!ite the I%1 being !resented with the unlawfulness of the
levy, through my !hone calls and letters regarding the ongoing theft, and in com!lete
violation of my unalienable and guaranteed %ights, has continued and been unanswered
All my written corres!ondence, including my corres!ondence demanding a res!onse to
my )uestions have never received any direct res!onse They have made this levy and
have taken earning and savings from me without due !rocess of law
The Internal %evenue service !roceeded to take money from my on8line bank account
and two of my branch banks They have also been garnishing my wages from my

received my letter in 7ontiac, Michigan on March (,, 0,((, and the !resum!tive letters
addressed and sent under notary !resentment and certified mail s!ecifically to A6ENT
NAME and his 1u!ervisor, A6ENT197E%&I1:% NAME on or about 5/0;/0,(( To
date, I have received no res!onse Therefore, they both ac)uiesce to my claims that this
action against me is unlawful and fraudulent among other charges against them The
levy, garnishments and stealing of funds from my bank accounts have made my life a
com!lete mess and this has become a very stressful time
To date, the I%1 has taken through garnishments a!!ro3imately 4(5,+,, since August of
0,(, This is on average about 4(,5+, !er month of issuance of the non8statutory
com!any for my efforts to assist them Additionally, they have taken 4<=,,<002 from
my IN6 Direct account, another 40,>(;02 from 7N# $ank and another 4;,, from
various other accounts and from small withdrawals from my accounts All together they
have stolen from my bank accounts over 4==,<,, and another 4(5,+,, from my earned
wages Total theft in the amount of a!!ro3imately 4;,,;,, to date
Des!ite my re)uests, A6ENT NAME and his su!ervisor, A6ENT197E%&I1:% NAME
have failed to !rovide certified co!ies of their current :ath of :ffice !er 91# Title <
#ha!ter ++ 11 +++( which are re)uired to be on file for !ublic scrutiny and as well as any
bonds to guarantee their faithful !erformance of their duties, !ursuant to their oath, as the
law re)uires
This entire levy has been unlawful and my guaranteed #onstitutional %ights have been
