E-Folio Essential VI
The purpose of this paper is to delineate Essential VI-Inter-professional
Communication and Collaboration for Improving Patient Health Outcomes. The overview of
Essential VI is to emphasis the importance of effective communication and collaboration
amongst health professionals to deliver patient-center care. Effective and timely communication
has been and continues to be a prime form of safe and successful patient care delivering in health
care. Team work is also another huge contributing factor for health professionals conveying high
quality and safe patient care. Fundamental to effective inter-professional and intra-professional
collaboration is a definition of shared goals clear role expectations of members a flexible
decision-making process and the establishment of open communication patterns and
leadership (AACN, 2008).
Exemplar
During the course of Elements of the Professional Nursing Role, we were asked to
choose an issue and analysis it. The topic that I chose for this assignment was health promotion
and public health. The reason for my choice is because I believe health promotion plays a very
integral role in nursing practice. Historically, nurse educators have taught patients how to
manage illness; in recent years the focus has shifted to teaching individuals how to manage
illness and remain healthy at the same time. After analyzing the issues about health promotion, I
came to the conclusion that its not only an individual effort to promote health, but the entire
community as well. Nurses must advocate for patients that are in need of care and or at risk for
diseases or worsening chronic conditions.

Collaborating with other healthcare workers to ensure patients receive the optimum
possible care and distributing information to the entire community about available assistance for
lifestyle modification and disease prevention are also part of nurses responsibilities (AACN,
2008). Essential VI is for the professional nurse to Incorporate effective communication
techniques, including negotiation and conflict resolution to produce positive professional
working relationships; use inter- and intra-professional communication and collaborative skills
to deliver evidence-based, patient -centered care; and advocate for high quality and safe patient
care as a member of the inter-professional team (AACN, 2008). Nurses play an important role
in promoting public health. As nurses we must always be advocates, even outside the hospital
setting. During my analysis of health promotion and public health, I learned that it takes effective
communication, planning and team work for patients to receive optimum care in the community
and to promote health.
