AFFIDAVIT OF REGISTERED OWNERS INSTRUCTION

I, JOAN V. TAPAN-CALUGAY, of legal age, Filipino, and currently a resident of Blk 34
Lot 39 Krypton Street, Golden City Dasma 4 Phase 10, Salawag, Dasmarias City, Cavite,
after having been sworn in accordance with law hereby depose and state, that

1. I, JOAN V. TAPAN-CALUGAY and my minor children namely: JHON CHRISTOPHER V.
TAPAN (12y.o.); FRANZ ADRIAN V. TAPAN (9

/2y.o.); PRINCESS YSHA T. CALUGAY
(8y.o.); and MICHAELORD T. CALUGAY, JR., (5y.o.) are registered owners of the
following real property hereunder specifically described as follows:

2. The owners copy of the titles to said properties are currently in possession of its
former owner, Mario Demata, Sr., the great grandfather of my children. Said person
donated the above-mentioned real properties to me and my children. It is only a
matter of time that the possession of said owners c will be turned over to me.

3. I understand that Mario Demata Sr., (the donor) is already very old or of advanced
age. I do not want to rush him regarding the turn-over of the owners copy of the
titles; however, pending such turn-over I wish to ensure that no unscrupulous persons
will be able to make any attempt to enter or register any transaction concerning the
above-described titles.

4. I therefore respectfully inform and instruct the Honorable Register of Deeds of
Cagayan De Oro City that I do not authorize and will not authorize any transaction by
other persons concerning the above-described real properties. If there will be any
transcation, I will be the one to directly act on the same.

5. As registered owners, I on my own right as well as on behalf of my minor children
who are under my sole custody, hereby instruct in rem against the whole world that
no transaction by any person, except myself, should be entertained by the Honorable
Register of Deeds without my personal consent.
