COMPANY/VESSEL AUDIT CHECKLIST


1. Has an effective walkabout been carried out to assess the general condition of the
vessel's deck, accommodation and machinery spaces? (For guidance, refer to
Attachment D- SWZ-018-00-P06)
2. Have the special requirements of the relevant Flag Administration been compiled
with as needed?
3. Have the vessel's relevant Class and Statutory certificates been sampled for
validity?
4. Have the crew's qualification documents, including proficiency certification for Ship
Security Officers and medical fitness certificates been sampled for validity and
compliance with the vessel's safe manning document? (STCW Ch. VI-Reg VI/5)
5. Have the mandatory minimum requirement for security related training and
instruction been sampled? (STCW Ch. VI-Reg. VI/6)
6. Has the report and certificate(s) been distributed as per ABS reporting obligations
noted in Flag Administration Authorization list (Statutory Reference Documents in
QMX) and Flag Administration's Special Instructions?
7. Has VIM been updated as needed? (For changes in vessel particulars, i.e. vessel
name, Gross Tonnage, Call Sign, IMO number, vessel type, etc.)
8. Have the certificates been issued or endorsed, copies attached and O2K updated?
9. If this audit is overdue, has the certificate been withdrawn and a short term
certificate been issued?
10. Have all previous CAR(s) been closed after verification of corrective action?
11. Have the due dates for the corrective action plan(s) been set correctly?
12. Have the due dates for the CAR closure(s) been set correctly?
13. Has the MSC Divisional office been notified to update the Service Agreement?
(e.g. changes in fees, WCN information, fleet composition, service specification, etc.)
14. Is the report package complete, as required? (i.e. all forms, checklists,
certificates, auditor notes, etc.)
15. Should audit frequency and/or duration remain unchanged? (If no, contact
Division Head for Guidance.)
16. Proper use of logos and certification marks (if any) were verified?
