RELEASE AND AUTHORIZATION TO PHOTOGRAPH AND RECORD

Legal Guardian(s) for the child named herein], herein referred to as releasor, in favor of the the Fox
Valley Childrens Chorus, a corporation organized and existing under the laws of the Sate of Illinois,
herein referred to as releasee.

In consideration of releasee allowing ___________________________________________________[Name of Child] to
participate in a choir of the Fox Valley Childrens Chorus, releasor, on behalf of said child, hereby
expressly releases releasee, its officers, directors, licensees, employees, agents, successors and
assigns from all liability for claims and demands arising out of the agreement as set forth below.

Releasor hereby consents to the above-referenced child being the subject of photographs or releasee,
and hereby authorizes releasee to cause the same to be exhibited, with or without advertising
sponsorship, as still photographs, transparencies, motion pictures, television, video or other similar
media, including the use of any such photograph on its website.

Releasor also hereby authorizes releasee to record the above-referenced childs picture and voice on
photographs, films and audio- and videotapes, to edit these recordings at its discretion, and to
incorporate these recordings into movie and sound films or audio-and videotapes, broadcasts (radio
and television, including cable and satellite transmissions), or otherwise, and to use and license
others to use such recordings, movies and sound films and audio- and videotapes and broadcast
programs in any manner of media whatsoever, including unrestricted use for purposes of publicity,
advertising and sales promotions and to use the above-referenced childs likeness and voice in
connection therewith including the use of such recordings on releasees website.

Releasor further agrees to release releasee, its officers, directors, licensees, employees, agents,
successors and assigns, from any and all claims and liability for damages, losses or expenses of any
sort arising from the making of such photographs and recordings and their use. Releasor further
acknowledges that there were no promises of any compensation for such use by releasee or by
anyone associated with releasee and, that releasee exclusively owns all right to these photographs
and recordings regardless of the form in which they are produced or used. No compensation nor
royalties of any kind shall be paid to releasor or to the above-referenced child for the use by releasee
of any such photographs or recordings.

Releasor warrants and agrees that he/she has read and understood the contents hereof.
