AFFIDAVIT OF SELF-ADJUDICATION

I, MARIA S. CRUZ, of legal age, Filipino, single, and a resident of 89
San Diego St., Brgy. Canumay, Valenzuela City, Philippines, after having
been duly sworn in accordance with law hereby, do hereby depose and state
that:

1. I am the sole heir of the late MARIO F. CRUZ, being his only
biological daughter;

2. The late MARIO F. CRUZ passed away on June 30, 2013 and died
without any last will and testament;


3. At the time of his death, my father had no debts, liabilities or
obligations to any persons, agency or institutions;

4. The only property left by my late father is a parcel of land situated in
56 Corinthians St., St. Peters Subdivision, Brgy. Canumay,
Valenzuela City, more particularly described as follows, to wit:

TCT No. 289678. A parcel of land (lot 56, S4, Area II, of the subdivision
plan, Bsd-04-000987, being a portion of theland described on plan Swo-
04-000878, parcel 1, LRC Rec. No. 787), situated in Brgy. Canumay, City
of Valenzuela. Bounded on the NE., along lines 2 to5 by lots 57, block 40,
of the Sub. Plan on the S., along lines 1 to 3 by Road Lot 54, Bsd. 04-
000767, on the W., along line 5 to 6 by lot 52 to, block 40, and on the N.,
alongline by alley 1 both of the subd. Plancontaining an area of ONE
HUNDRED SQUARE METERS (100)

5. As the sole heir of the herein decedent, I hereby adjudicate the
abovementioned property solely for and in my name as well as any
and all liabilities arising from the said property;

6. I hereby execute this affidavit for the purpose of processing the
transfer of the said property in my name, the settlement of the estate of
the late MARIO F. CRUZ, as well as for the release of any other claim
or benefit in relation to his death before any government/private office
and banking institution and for any other legal purpose this may best
serve.
