I, Bon Juvan A. Loquellano, an alumna of this University graduated with the
degree of Bachelor of Science in Electronics Engineering last April 2, 2014 hereby
authorize Juliet A. Loquellano to deal any legal transactions under my name in this
University. I have given her authority to process my Transcript of Records (TOR)
request because I will not be available for five (5) months due to exam review at
Cebu City in preparation for the upcoming Electronics Engineering Licensure Exam
this September 27-28, 2014.
