1. Review and streamline the registration process.
2. Evaluate state mandated trainings and implement online professional
development.
3. Create a PSAT course at Lodi High School to increase college and career
readiness and exposure to the PSAT test.
4. Begin planning for the implementation of NJ Partnership for School Based HIV,
STD, and pregnancy prevention.

4. Sick Leave of Absence

I am in receipt of a letter from J oan Facella, LDT-C, requesting a paid sick leave of
absence utilizing her accumulated sick days from August 19, 2014 to September 19,
2014. She will resume her duties September 22, 2014.

5. Child Study Team Service

I request confirmation of approval to enter into an agreement with LJ F Education
Services in order to provide Child Study Team services at a rate of $325.00 per diem for
the 2014-2015 school year. Said services are needed due to a sick leave of absence.

6. 2014-2015 Merit Bonus Goals and Action Plans

I request approval of the following 2014-2015 Merit Bonus Goals and Action Plans as
listed below. I further request approval to submit said 2014-2015 Merit Bonus Goals and

Description of Goal:

Increase professional development opportunities for all staff (certificated and non-
certificated) by creating a local Professional Development Academy that will provide
instructional and non-instructional training programs to Lodi staff at no expense to the
Board of Education.

Evidence of Completion:
Two reports will be generated to provide evidence of the goals effectiveness. The first shall
be a report that will show the number of staff who attended training sessions throughout
the year. The second shall be a report that will show the staff rating of the effectiveness of
each training program provided.

Restructure the Guidance Department at Lodi High School to increase the overall
effectiveness and efficiency of the department, by lowering the overall student to counselor
ratio, and implementing additional guidance programs in all grade levels throughout the
year.

Evidence of Completion:

A dually certified SAC/School Counselor will be hired to replace a full-time SAC
Counselor, a part time secretary and a part-time Student Services Consultant will be hired
to replace a full-time secretary.
Reports will be generated to provide evidence that show a reduction in the student to
counselor caseload for 2014-15 in comparison to the two previous years. Another report
will show the total number of additional guidance programs conducted throughout the
year by topic, grade level, and month.

services for non-public school students through I.D.E.A. funding. The service that
Bergen County Special Services will provide is Supplemental Instruction and Speech.
The Lodi School District will pay Bergen County Special Services up to the maximum of
$29,360.00 following the start of services.

8. Consulting Services Agreement Northern Valley Regional High School District

I request confirmation of approval of the contract between the Northern Valley Regional
High School District and the Lodi Board of Education for the provision of a home
consultant to provide home based therapy for the student listed below:

14. Sick Leave of Absence/Retirement

I am in receipt of a letter from Geri Stolz, Thomas J efferson Middle School Science
Teacher, requesting a paid sick leave of absence utilizing her accumulated sick days from
September 2, 2014 to October 20, 2014. Mrs. Stolz further requests an unpaid leave of
absence from October 21, 2014 to October 31, 2014 upon which time she shall retire.

15. Staff Resignation

I am in receipt of a letter from Alisa Miller, Lodi High School Guidance Secretary,
resigning from her position effective August 8, 2014.

16. Appointment of LHS 12 Month Part Time Guidance Clerk Typist

I request confirmation of approval to appoint Vivian Cangialosi to the 12 month part time
clerk typist position at Lodi High School effective August 4, 2014. Hours of work will
be from 8:00 a.m. to 12:30 p.m. on a daily basis. Salary shall be $34,356.00 with no
medical benefits.

17. Bergen County Department of Health Services-Bloodborne Pathogen Compliance
Program 2014-2015 School Year

I request approval to authorize the Bergen County Department of Health Services to
provide Bloodborne Pathogen Training and Hepatitis B vaccinations for Lodi Public
Schools at an estimated cost of $3,000.00 for the 2014-2015 school year.

18. HIB Trainings and Programs

In accordance with the Anti-Bullying Bill of Rights Act, districts must report to the New
J ersey Department of Education on the trainings conducted and programs implemented to
reduce harassment, intimidation, and bullying (HIB) twice each school year between J uly

1 and December 31 (Report Period 1) and between J anuary 1 and J une 30 (Report
Period 2). Similarly, all HIB trainings and programs must be reported to the Board of
Education. Attached please find HIB trainings and programs that were submitted to the
New J ersey Department of Education for Report Period 2 for the 2013-2014 school year.

19. Public Hearing on School Violence, Vandalism and Substance Abuse

As per law and code, the Superintendent of Schools at a public hearing must report to the
Board of Education all acts of violence, vandalism and substance abuse including
harassment, intimidation and bullying two times each school year. The reporting periods
shall be between September 1 and December 31 (Period 1) and J anuary 1 to J une 30
(Period 2). The presentation and information provided by the Superintendent shall
include data submitted under the Electronic Violence and Vandalism Reporting System
(EVVRS) to the New J ersey Department of Education for the previous school year
(2013-2014). A copy of all data that the Superintendent will present to the Board is listed

20. Appointment of Vice Principal

I request confirmation of approval to appoint Kevin Dowson to the position of Thomas
J efferson Middle School Vice Principal effective August 4, 2014. Salary shall be step 1
MA +30 in accordance with the labor agreement between the Elementary, Middle and
Secondary School Administrators Association (EMSSAA) and the Lodi Board of
Education.

21. Appointment of LHS Interim Supervisor of Mathematics

I request confirmation of approval to appoint J ohn Way to the position of Lodi High
School Interim Supervisor of Mathematics effective September 1, 2014. Salary shall be
step 16 MA in accordance with the Lodi Association of School Supervisors and the Lodi
Board of Education.

22. Appointment of LHS Supervisor of Social Studies/Spanish

I request confirmation of approval to appoint Robert Cannizzaro to the position of Lodi
High School Supervisor of Social Studies/Spanish effective September 1, 2014. Salary
shall be step 10 MA in accordance with the Lodi Association of School Supervisors and
the Lodi Board of Education.

23. Appointment of LHS Assistant Football Coach

I request confirmation of approval to appoint Corey Paladino, Elementary School Health
and Physical Education Teacher, to the position of Lodi High School Assistant Football
Coach effective for the 2014-2015 school year. Stipend shall be $7,619.00 in accordance
with the labor agreement between the Lodi Education Association and the Lodi Board of
Education.

24. Appointment of LHS Guidance Counselor

I request confirmation of approval to appoint Geraldine Passano, Lodi High School
English Teacher, to the position of Lodi High School Guidance Counselor effective
September 1, 2014. Salary shall be step 12 MA in accordance with the Professional
Specialists Personnel Organization and the Lodi Board of Education.

25. Supplemental Student Services

I request confirmation of approval to enter into an agreement with Student Support
Services Solutions, LLC in order to provide supplemental student services at a rate of
$150.00 per diem. Said services shall not exceed $22,500.00 for the 2014-2015 school
year.

26. Essex Regional Educational Services Commission, 2014-2015 School Year

I request authorization to enter into an agreement with the Essex Regional Educational
Services Commission effective September 1, 2014 to J une 30, 2015. This contract allows
for the purchase of the Alternative Education slots/seats in the Essex High School for at-
risk students at a fee of $18,040.00 per slot/seat.

27. Donation of Services

I request confirmation of approval for the donation of services from the J ennifer Scimeca
Memorial Fund. The Washington School faculty and staff established this fund in order
to construct a memorial garden and granite bench in memory of Ms. Scimeca. The
garden will be maintained by the memorial fund.

28. Increased Work Day of Teacher Aide

Due to additional responsibilities, I am requesting approval to increase the workday of
the following teacher aide as listed below:

1. Aynur Dere, Lodi High School Teacher Aide, by fifteen (15) minutes per day at her
currently hourly rate of $17.22. Said increase will be effective from September 1, 2014
to J une 30, 2015.

29. Resignation of Teacher Aide

I am in receipt of a letter from J oyce Rodenbaugh, Hilltop School Teacher Aide,
resigning from her position effective J une 30, 2014.

30. Account Clerk Services

I request confirmation of approval to enter into an agreement between the Lodi Board of
Education and Accountemps in order to provide temporary account clerk services to
cover a medical leave of absence in the Business Office at an hourly rate of $35.00. Said
agreement shall remain in effect until August 29, 2014.

31. Donation of Services

I request approval for the donation of services from The Home Depot in order to erect a
Heros Garden that will serves as a 9-11 memorial at Lodi High School. The memorial
will include a paver walkway leading to a paver planting box with flowers and shrubs.
Two memorial stones for the Lodi High School Alumni will also be displayed.

This office has been notified by the New J ersey Department of Agriculture, Bureau of
Child Nutrition Programs, that the food service management company contract
submitted for fiscal year 2015 has been approved.


4. COLUMBUS SCHOOL CLASSROOM FLOORS REPLACEMENT
PROJECT
CHANGE ORDER #01

In accordance with the recommendation of SSP Architectural Group., the Boards
Architectural Firm, I request confirmation of approval of the change order in the
amount of $684.00 be approved. Said change order is to repair a 12 x 12 section of
damaged sub flooring in Room 6. However due to the built in $2,975.00 repair
allowance, the original contract sum will remain unchanged.


5. THOMAS JEFFERSON MIDDLE SCHOOL AUDITORIUM
RENOVATION PROJECT
CHANGE ORDER #01

In accordance with the recommendation of SSP Architectural Group., the Boards
Architectural Firm, I request confirmation of approval of the change order in the
amount of $5,000.00 be approved. Said change order is to provide additional material
to cover larger openings around existing lights and to repair plaster at the upper rear
portion of the stage. However due to the built in $10,000.00 repair allowance, the
original contract sum will remain unchanged.

6. LODI HIGH SCHOOL BOILER REPLACEMENT PROJECT
CHANGE ORDER #1

In accordance with the recommendation of SSP Architectural Group., the Boards
Architectural Firm, I request confirmation of approval of the change order in the
amount of $3,137.00 be approved. Said change order is to replace a pair of doors
between the boiler room and the corridor. However due to the built in $10,000.00
repair allowance, the original contract sum will remain unchanged.


7. DONATION OF EQUIPMENT TO THE BOYS AND GIRLS CLUB

I request confirmation of approval of the donation of a movie screen, TAG# 2711, to
the Boys and Girls Club as requested by Frank DAmico, Principal of Lodi High School in
accordance with a memo dated J uly 10, 2014.

In accordance with the recommendation of SSP Architectural Group., the Boards
Architectural Firm, I request confirmation of approval of the change order in the
amount of $550.00 be approved. Said change order is to reduce the height of existing
subfloor saddle between the classroom and closet in Room 6, also to replace an area of
bowed subfloor in room 11. However due to the built in $2,975.00 repair allowance, the
original contract sum will remain unchanged.

amount of $560.00 be approved. Said change order is to trim the underside of cabinets
in the Main Office. However due to the built in $2,975.00 repair allowance, the original
contract sum will remain unchanged.



11. HILLTOP PAVEMENT RECONSTRUCTION PROJECT
CHANGE ORDER #01

In accordance with the recommendation of SSP Architectural Group., the Boards
Architectural Firm, I request confirmation of approval of the change order in the
amount of $3,500.00 be approved. Said change order is to install 125 linear feet of PVC
piping under the pavement in connection with the field irrigation system for a total cost
of $8,500.00. However due to the built in $5,000.00 general repair allowance, the
contract sum will increase by $3,500.00.


12. COLUMBUS SCHOOL CLASSROOM FLOORS REPLACEMENT
PROJECT
CHANGE ORDER #05

In accordance with the recommendation of SSP Architectural Group., the Boards
Architectural Firm, I request confirmation of approval of the change order in the
amount of $(55.00) be approved. Said change order is to replace and repair concrete
and wood framing in classroom 7 for an amount of $822.00. The remaining $55.00 of
the total repair allowance is being credited to the original contract sum.



13. WILSON SCHOOL CLASSROOM FLOORS REPLACEMENT PROJECT
CHANGE ORDER #01

In accordance with the recommendation of SSP Architectural Group., the Boards
Architectural Firm, I request confirmation of approval of the change order in the
amount of $(3,100.00) be approved. Said change order is a credit of the unused
portion of the general repair and wood subfloor allowance.

and patch various unforeseen gaps in the floor under door frames and under ventilators
and around piping.

In accordance with the recommendation of SSP Architectural Group., the Boards
Architectural Firm, I request confirmation of approval of the change order in the
amount of $4,079.00 be approved. Said change order is to relocate cold water piping at
the ceiling to accommodate the new expansion tank and repair flue opening at boiler
room wall. However due to the built in $10,000.00 repair allowance, the original
contract sum will remain unchanged.
