Emergency Care for Academies of ICICI Bank

This note details the procedure for Academies to provide officers and managers
of various Academies to avail medical attention in case of an emergency. ICICI
Lombard has a tie up with 93 hospitals across the country where during
emergency the hospital will not demand deposit from the officer and will
commence treatment immediately.

A. In case of an emergency at the hostel/campus, following steps have to be
followed: -
1. Warden to be contacted immediately, who in turn will arrange the
vehicle available 24*7 at the hostel/campus.
2. The patient to be taken to the nearest hospital, taking into account the
severity of the case
OR
In case of a life threatening situation, ambulance to be arranged
immediately, with all life saving instruments.
3. Warden/hostel staff to inform Admin Head and ICICI Bank
representative.
4. Subsequently, a call to be made on the emergency contact no. The call
centre personnel will take the concerned representative from the
academy on conference call with the employee/attendant.
5. The patient to be admitted to the Emergency Ward or OPD depending
upon the severity of the case.
6. Representative from the academy can assist the employee on further
proceedings.
B. In case of an emergency outside hostel/campus, following steps have to be
followed: -
1. The employee/attendant to call up the Emergency care number, printed at
the reverse of the Identity card.
2. The call centre personnel will take the concerned representative from
the academy on conference call with the employee/attendant.
The patient will be taken to the nearest hospital immediately, taking
into account the severity of the case
OR
In case of a life threatening situation, ambulance to be arranged
immediately, with all life saving instruments
3. Representative from the academy to inform the Admin Head and ICICI
Bank representative and assist the employee on further proceedings.

Definition of Medical emergency

Only sudden, unforeseen, unplanned, life threatening or serious medical issues
faced by the officer that may require immediate medical attention will be
within the coverage of medical emergency. Examples of such instances are
road accidents, cardiac arrest, paralytic stroke, seizures etc.


Eligibility

All officers of Academies of ICICI Bank will be covered under this.


What will be the room eligibility criterion for hospitalization?
The following eligibility criteria will be applicable for rooms at the time of
hospitalization:
* For Primary / Secondary Hospital*: Twin Sharing
**For Tertiary Hospital: Economy Class

(*Primary / Secondary Hospital : Basic amenities with 5-10 beds / Specialty for one
issue only
** Tertiary Hospital : All Multispeciality facility, including organ transplant)

What is the process when the officer reaches the hospital during a medical emergency?
When the officer reaches the hospital, he will be required to show his academy
identity card to the hospital officials. If the same is not available, he can give his
identification number and submit a copy of his academy identity card to the hospital
within 48 hours from the time of admission. ICICI Bank has a tie up with 93 hospitals
across the country where during emergency the hospital will not demand deposit from
the officer and commence treatment immediately. Please refer to Annexure 2 for the
list of these hospitals. In all other hospitals, officer may be required to provide an
initial deposit before treatment is commenced.

What is the subsequent process to be followed by the hospital?
The hospital will get in touch with our emergency contact centre, whose number has
already been provided to them. Our emergency centre will confirm to the hospital, if
the officer is part of Academy of ICICI Bank, and details about his/her room eligibility
and coverage in the policy will be provided. A confirmation email will be sent from the
Emergency Contact Centre to the hospital as well, stating that the treatment can
begin without the officer having to make a security deposit.

How will the issue be handled if the person who needs medical help is covered under
the company insurance policy?
If the person admitted is covered under our policy, the following two options can be
exercised :-
The hospital will fax the cashless request to ICICI Lombard General Insurance who will
issue the Credit Letter upon receiving the request.
OR
The hospital will get in touch with the representative of the academy, whose details
are given to them, and they will have to fax a Credit Letter, asking the hospital to
send the bill to the Bank for the payment of the same.

What is the billing process (and timelines for the same) for those who are covered under
the policy?
Wherever the cashless facility is issued by Lombard, the employee will only have to
pay for the items which are not covered in the policy, like food, telephone, etc. For
covering such expenses, the hospital may take a deposit of Rs. 5000/- from the
employee. Please note, this amount may vary depending on the hospital.
If the Credit Letter is issued by the Bank, the hospital is required to send the original
bills and receipts to Sachin Pednekar (153205) or Geofrey Fernandes (101728), and the
Bank will make the payment to the hospital within 30 days from the date of receipt of
the bills.

What if the person is not hospitalized and is discharged on the same day?
If the person is not hospitalized and is discharged on the same day, the treatment
expenses will have to be paid by the officer, as per the hospitalization policy.

What is our stand on other expenses of the hospital which are not covered under the
insurance policy?
In cases where ICICI Lombard has issued the Credit Letter, expenses not covered by
ICICI Lombard will not be payable by the insurance company and the difference will be
recovered from the officer before discharge from the hospital.
In cases where the Bank has issued the Credit Letter to the hospital, the officer will
have to pay the differential amount to Bank, once the insurance company settles the
claim.

How will the transport be arranged in case of an emergency at any of the Institutes?

All institutes will have an emergency vehicle (four wheeler) available 24*7 in the
premises of the institute.

Sometimes the hospital sends an ambulance to pick up the patient. What will be our
stand with regard to such expenses?
In such cases the officer is required to bear the cost for the ambulance and any
attending doctor, if any.

How will emergency care number be shared with officers?

Emergency care number will be printed at the reverse of the Identity cards of the
officers. Institutes will share the Insurance Policy number with all the officers. This
insurance policy number will also be displayed at a prominent place in the institute.
