Helpful Information for the Order Form

Complete one form per child.
Taxes are included in the prices.
White and chocolate milk are the same price, 75. Milk can be ordered on its own. You do not have
to order food as well.
Subs are simply ham or turkey or veggie. They are not a combination.
The veggie plate will feature seasonal fresh vegetables. Likely items could be any combination of
carrots, celery, cucumber, cherry tomatoes, broccoli, cauliflower, or peppers. The dip will be a
creamy ranch style dressing.
Lasagna plates will be served hot at the school.
Pizza can be ordered and paid for ahead of time using this form. Orders on this form must have
payments included now or the pizza will not be ordered. Example: 2 slice(s) x $2 = $4
Please include your choice of cheese or pepperoni.
Pizza can also be ordered on Friday mornings, using the pay as you go format. Please do not order
using this form but pay on Friday morning. This creates confusion. Please use one system or the
other.
Pizza is the ONLY item with a pay as you go format. All other items must be ordered and paid for
ahead of time, using this form.
Food must be paid for using cash only. Exact amounts must be included. Payments must
accompany this form.
Late orders will not be accepted.
This months orders and payment must be received by Friday, September 19
th
at 9:00 am.

We need your help to deliver our lunch program to your children. If you are available between
the hours of 12:00 and 1:00 pm, please contact the school to sign up for a day of the week.
