DEED OF UNDERTAKING FOR BANK DEPOSIT PAYMENT

(For Corporation, Partnership, Sole Proprietor)

KNOW ALL MEN BY THESE PRESENTS:
I, ____Monte Alver M. Daran____, of legal age, Filipino, with office address
at__Brgy.Sta. maria lallo cagayan________________________________________________,
and on behalf ______Monte Alver Roofline______, do hereby declare and undertake that a.
Monte Alver Roofline is a customer of United Steel Technology International
Corporation (Steeltech). The Management of Monte Alver Roofline is fully aware and informed
that Steeltech does not allow direct cash payment to its Account Specialists, sales personnel,
employees or other unauthorized representatives; the preferred mode of payment being through
check payments to protect the integrity of payments.
b.
To facilitate the payment of orders in cash (without the issuance of checks),
__Monte Alver roofline____ undertakes to pay its orders from Steeltech through bank deposit, the
details of which have been properly discussed with Steeltech, and are as follow:

The Steeltech Business Office shall directly issue and send the official receipt/s for our order/s.
c.
____Monte Alver Roofline_______ shall make bank deposit payment only
through its authorized representative. The Steeltech Business Office shall directly issue and send
the official receipt/s for our order/s. No Account Specialists, sales personnel or other employees of
Steeltech are authorized, at any given instance, to receive any direct cash payment from the
customer, and neither are they authorized to issue any form of receipts (or acknowledgement
receipts) as a result of such direct cash payment.
d.
At all instances, ___Monte Alver Roofline_ or any of its representative shall not
give, entrust, hand-over or delegate any cash payment (including cash payment to be deposited to
the bank account of Steeltech) to any Steeltechs Account Specialist, sales personnel, employees or
other unauthorized representatives. Any damage or loss resulting from this act shall be borne by
us, the customer, and Steeltech shall have no responsibility to honor/recognize as valid payment
any cash disbursement given to Steeltechs Account Specialist, sales personnel, employees or other
unauthorized representatives.

This Deed of Undertaking shall refer and continue to be effective for all
transactions that Monte Alver Roofline has with Steeltech and shall not only refer to the present
transaction with Steeltech.
f.
Monte Alver Roofline shall faithfully abide with the terms and conditions of
this Deed of Undertaking.
g.
I am fully authorized by __Monte Alver Roofline___ to execute this Deed of
Undertaking on its behalf. Moreover, I have read and understood this document and have executed
this document willingly and voluntarily and with full knowledge of my rights under the law.
