Alaskan Malamute Sleddog Day 23 25 January 2015 for:
Responsibility for risks of injuries, loss of personal effects due to thieving or any other reason, as well as civil
responsibility towards third party for death, personal injuries, damage of equipment of third party during the days of
the meeting. I declare that I will follow the instructions briefed by the organizer committee as far as safety measures
and will wear the protections required. I declare that I am a subscriber of a general liability insurance that covers for
damages towards third party or property damage caused by me.
I also, with the below subscription, authorize the organizer committee to use at no charge the pictures, videos, tapes,
etc. taken during the meeting. I agree to the use of my personal data limited to the SERAM/CIRM Club association
purposes.
I accept and recognize in full the rules applied during the meeting
