I. General Statement of Safety Policy:


ItisthepolicyofHeardConstructiontoprovideprotectiontothelifeandhealthofemployeesandother
persons;forpreventionofdamagetoproperty,materials,suppliesandequipmentinaccordancewith
the requirements of OSHA standards and safety provisions. It is the responsibility of the following
managementandsupervisorypersonneltoimplementthispolicy.

II. General Work Rules


ABRASIVEGRINDING
(SeeFederalOSHAStandard1926.303)
Abrasive wheel bend or stand grinders must have safety guards strong enough to withstand bursting
wheels. Adjust work rests on grinders to a clearance not to exceed 1/8 inch between rest of wheel
surface.Inspectandringtestabrasivewheelsbeforemounting.Alwaysleavewheelinworkingcondition
fornextuser.Properlydresswheelbeforeusingand/orwhenfinished.TheRPMratingoftheabrasive
wheelmustbeinaccordancewiththeRPMratingofthemotor.

ACCESS
Onlysafemeansofaccesstoandfromworkareaswillbeused.Jumpingfromortoworkareasisnot
allowednorisslidingdowncableropesandguys.

AIRTOOLS
(SeeFederalOSHAStandard1926.302)
Secure pneumatic tools to hose in a positive manner to prevent accidental disconnection. Install and
maintain safety clips or retainers on pneumatic impact tools to prevent attachments from being
accidentally expelled. All hoses exceeding inch inside diameter require safety devices at source of
supplytoreducepressureincaseofhosefailure.

ATTITUDE
(SeeFederalOSHAStandard1926.20)
Allcompanyemployeesarerequiredtotreatsafetyasanumberonepriority.Assuchtheyareexpected
toreporttoworkingoodmentalandphysicalconditiontosafelyperformtheirassignedduties.Before
starting any task, employees must consider the possible effects of their actions on themselves and
othersandtakeappropriateprotectivemeasures.

BELTSANDINGMACHINE
(SeeFederalOSHAStandard1910.243)
Beltsanderswillnotbeusedwithoutguardsinplace.Thedirectionaltravelindicatedonthebeltmust
matchthetravelofthesander.

COMPRESSEDAIR,Useof
(SeeFederalOSHAStandard1926.302)
Compressed air used for cleaning purposes may not exceed 30 psi and then only in conjunction with
effectivechipguardingandpersonnelprotectiveequipment.Exceptionsto30psiareallowedonlyfor
concrete form, mill scale, and similar cleaning operations. The use of compressed air to clean off
yourselforotherworkersisNOTallowed.

Putvalveprotectioncapsinplacebeforecompressedgascylindersaretransported,movedorstored.
Cylindervalveswillbeclosedwhenworkisfinishedandwhencylindersareemptyorbeingmoved.

Compressedgascylinderswillbesecuredinanuprightposition,withchain,steelangle,etc.,atalltimes.
Keep cylinders a safe distance or shield from welding or cutting operations and placed where they
cannotbecomepartofanelectricalcircuit.Oxygenandacetylenemustnotbestoredtogether.

Oxygenandfuelgasregulatorsmustbeinproperworkingorderwhileinuse.

CONCRETE,CONCRETEFORMSANDSHORING&LIFTSLABOPERATIONS

(SeeFederalOSHAStandard1926.700706&1926.305)
All protruding reinforcing steel onto which employees could fall must be guarded to eliminate the
hazardofimpalement.Wiremeshneedstobesecurefromrecoiling.

Formwork and shoring will be designed and construction to safely support all loads imposed during
concreteplacement.Allcomponentswillbeinspectedpriortoerection.Drawingsorplansofjacklayout,
formwork,shoring,workingdecksandscaffoldingsystemswillbeavailableatjobsite.

Forms and shores may not be removed until it has been determined that the concrete has gained
sufficientstrengthtosupportitsweightandsuperimposedloads.

CONFINEDSPACES

(SeeFederalOSHAStandard1910.146,1926.650,MaritimeStandard1915.4554,1917.4654)
This section contains requirements for practices and procedures to protect employees in general
industryfromthehazardsofentryintopermitrequiredconfinedspaces.Thissectiondoesnotapplyto
agriculture,toconstructionortoshipyardemployment.

Confinedorenclosedspacesinclude,storagetanks,processvessels,bins,boilers,ventilatorsorexhaust
ducts,sewers,undergroundutilityvaults,tunnels,pipelines,andopentopspacesmorethan4ft.in
depthsuchaspits,tubs,vaults,andvessels.

Confinedspacemeansaspacethatislargeenoughandsoconfiguredthatanemployeecanbodilyenter
andperformassignedwork;andhaslimitedorrestrictedmeansforentryorexit(forexample,tanks,
vessels,silos,storagebins,hoppers,vaults,andpitsarespacesthatmayhavelimitedmeansofentry);
andisnotdesignedforcontinuousemployeeoccupancy.

Acceptableentryconditionsmeanstheconditionsthatmustexistinapermitspacetoallowentryandto
ensurethatemployeesinvolvedwithapermitrequiredconfinedspaceentrycansafelyenterintoand
workwithinthespace.

Attendant means an individual stationed outside one or more permit spaces who monitors the
authorized entrants and who performs all attendants duties assigned in the employers permit space
program.

Authorizedentrantmeansanemployeewhoisauthorizedbytheemployertoenterapermitspace.

Blankingorblindingmeanstheabsoluteclosureofapipe,line,orductbythefasteningofasolidplate
(such as a spectacle blind or a skillet blind) that completely covers the bore and that is capable of
withstandingthemaximumpressureofthepipe,lineorductwithnoleakagebeyondtheplate.

Permitrequiredconfinedspace(permitspace)meansaconfinedspacethathasoneormoreofthe
followingcharacteristics:

Containsorhasapotentialtocontainahazardousatmosphere;

Containsamaterialthathasthepotentialforengulfinganentrant;

Hasaninternalconfigurationsuchthatanentrantcouldbetrappedorasphyxiatedbyinwardly
convergingwallsorbyafloor,whichslopesdownwardandtaperstoasmallercrosssection;

Containsanyotherrecognizedserioussafetyorhealthhazard.

Permitrequiredconfinedspaceprogram(permitspaceprogram)meanstheemployersoverallprogram
for controlling, and, where appropriate, for protecting employees from permit space hazards and for
regulatingemployeeentryintopermitspaces.

Permitsystemmeanstheemployerswrittenprocedureforpreparingandissuingpermitsforentryand
forreturningthepermitspacetoservicefollowingterminationofentry.

Testing means the process by which the hazards that may confront entrants of a permit space are
identified and evaluated. Testing includes specifying the tests that are to be performed in the permit
space. Testing enables employers both to devise and implement adequate control measures for the
protection of authorized entrants and to determine if acceptable entry conditions are present
immediatelypriorto,andduringentry.

The employer shall evaluate the workplace to determine if any spaces are permitrequired confined
spaces. (Note: proper application of the decision flow chart in Appendix A to section 1910.146 would
facilitatecompliancewiththisrequirement*seeatendofsection)

If the workplace contains permit spaces, the employer shall inform exposed employees by posting
dangersignsorbyanyotherequallyeffectivemeans,oftheexistenceandlocationofandthedanger
posedbythepermitspaces.(Note:AsignreadingDANGERPERMITREQUIREDCONFINEDSPACE,DO
NOTENTER)orusingothersimilarlanguagewouldsatisfytherequirementforasign.

PresenceoftoxicgasesEqualtoormorethan10ppmhydrogensulfidemeasuredasan8hour
timeweighted average. If the presence of other toxic contaminants is suspected, specific
monitoringprogramswillbedeveloped.
Presence of explosive/flammable gases: Equal to or greater than 10% of the lower flammable
limit(LFL).
OxygenDeficiency:Aconcentrationofoxygenintheatmosphereequaltoorlessthan19.5%by
volume.

EntrywithoutPermit/Attendant
Certification:confinedspacesmaybe enteredwithoutthe needforawrittenpermitorattendant
providedthatthespace canbemaintainedinasafeconditionforentrybymechanicalventilation
aloneasprovidedinSection1910.146(c)(5).Allspacesshallbeconsideredpermitrequiredconfined
spacesuntilthepreentryproceduresdemonstrateotherwise.Anyemployeerequiredorpermitted
toprecheckorenteranenclosed/confinedspaceshallhavesuccessfullycompleted,asaminimum,
thetrainingasrequiredbythefollowingsectionsoftheseprocedures.Awrittencopyofoperating
andrescueproceduresasrequiredbytheseproceduresshallbeattheworksiteforthedurationof
thejob.TheConfinedSpacePreEntryCheckListmustbecompletedbytheLEADWORKERbefore
entryintoaconfinedspace.Thislistverifiescompletionofitemslistedbelow.Thischecklistshallbe
keptatthejobsitefordurationofthejob.Ifcircumstancesdictateaninterruptioninthework,the
permitspacemustbereevaluatedandanewchecklistmustbecompleted.
Controlofatmosphericandengulfmenthazards

Pumps and Lines: All pumps and lines which may reasonably cause contaminants to flow into
thespaceshallbedisconnected,blindedandlockedout,oreffectivelyisolatedbyothermeans
to prevent development of dangerous air contamination or engulfment. Not all laterals to
sewersorstormdrainsrequireblocking.However,whereexperienceorknowledgeofindustrial
use indicates there is a reasonable potential for contamination of air or engulfment into an
occupiedsewer,thenallaffectedlateralsshallbeblocked.Ifblockingand/orisolationrequires
entry into the space, the provisions for entry into a permitrequired confined space must be
implemented.
Surveillance: The surrounding area shall be surveyed to avoid hazards such as drifting vapors
fromthetanks,piping,orsewers.
Testing: The atmosphere within the space will be tested to determine whether dangerous air
contamination and/or oxygen deficiency exists. Detector tubes, alarm only gas monitors and
explosionmetersareexamplesofmonitoringequipmentthatmaybeusedtotestpermitspace
atmospheres.TestingshallbeperformedbytheLEADWORKER,whohassuccessfullycompleted
theGasDetectortrainingforthemonitorhewilluse.Theminimumparameterstobemonitored
are oxygen deficiency, LFL, and hydrogen sulfide concentration. A written record of the pre
entry test results shall be made and kept at the work site for the duration of the job. The
supervisorwillcertifyinwriting,basedupontheresultsofthepreentrytesting,thatallhazards
havebeeneliminated.Affectedemployeesshallbeabletoreviewthetestingresults.Themost
hazardousconditionsshallgovernwhenworkisbeingperformedintwoadjoining,connecting
spaces.
Entry Procedures: If there are no nonatmospheric hazards present and if the preentry tests
show there is no dangerous air contamination and/or oxygen deficiency within the space and

there is no reason to believe that any is likely to develop, entry into and work within may
proceed.Continuoustestingoftheatmosphereintheimmediatevicinityoftheworkerswithin
thespaceshallbeaccomplished.Theworkerswillimmediatelyleavethepermitspacewhenany
ofthegasmonitoralarmsetpointsarereachedasdefined.Workerswillnotreturntothearea
untilaSUPERVISORwhohascompletedthegasdetectortraininghasusedadirectreadinggas
detectortoevaluatethesituationandhasdeterminedthatitissafetoenter.
Rescue:Arrangementsforrescueservicesarenotrequiredwherethereisnoattendant.Seethe
rescue portion of Section B below for instructions regarding rescue planning where an entry
permitisrequired.

ProceduresforAtmosphericTesting
Atmospherictestingisrequiredfortwodistinctpurposes:evaluationofthehazardsofthepermitspace
andverificationthatacceptableentryconditionsforentryintothatspaceexist.

EvaluationTesting:Theatmosphereofaconfinedspaceshouldbeanalyzedusingequipmentof
sufficientsensitivityandspecificitytoidentifyandevaluateanyhazardousatmospherethatmay
exist or arise so that appropriate permit entry procedures can be developed and acceptable
entry conditions stipulated for that space. Evaluation and interpretation of these data, and
developmentoftheentryprocedure,shouldbedoneby,orreviewedby,atechnicallyqualified
professional(e.g.,OSHA consultationservice,orcertifiedindustrialhygienist, registeredsafety

Protectionofemployeesinexcavationsagainstcaveinsexceptwhentheexcavationisinstable

that a cavein should be expected; and against falling rock, soil or material by use of an
adequate system. The latter operation includes scaling to remove loose rock or soil,

DutyAsEmployer
In emergencies, or when feasible engineering or administrative controls may not be effective in
controlling toxic substances, approved respiratory protection equipment will be used. Respiratory
protective devices will be approved for the hazardous material involved and extend to the nature of
work requirements and conditions. Employees required to use respiratory protective devices shall be
thoroughly trained in their use. Respiratory protective equipment will be inspected regularly and
maintainedingoodcondition.

Selection
In order to determine the proper respiratory devices to be used when performing a specific job task,
samplings shall be taken. Also, contractor will provide additional requirements for proper respiratory
deviceuse.

MedicalEvaluationProcedures
The employer shall identify a physician or other licensed health care professional (PLHCP) to perform
medical evaluations using a medical questionnaire or an initial medical examination that obtains the
sameinformationasthemedicalquestionnaire.

FollowUpMedicalExamination
Theemployershallensurethatafollowupmedicalexaminationisprovidedforanemployeewhogives

MedicalDetermination
In determining the employees ability to use a respirator, the employer shall obtain a written
recommendation regarding the employees ability to use the respirator from the Heard Construction
physician. The recommendation shall provide only the following information: Any limitations on
respiratoruserelatedtothemedicalconditionoftheemployee,orrelatingtotheworkplaceconditions
inwhichtherespiratorwillbeused,includingwhetherornottheemployeeismedicallyabletousethe
respirator.

ProperUsage
Respiratorsareaneffectivemethodofprotectionagainstdesignatedrespirablehazardswhenproperly
selectedandworn.Respiratoruseisencouraged,evenwhenexposuresarebelowtheexposurelimit,to
provide an additional level of comfort and protection for workers. However, if a respirator is used
improperly or not kept clean, the respirator itself can become a hazard to the worker. Sometimes,
workersmaywearrespiratorstoavoidexposurestohazards,eveniftheamountofhazardoussubstance
does not exceed the limits set by OSHA standards. If your employer provides respirators for your
voluntary use, of it you provide your own respirator, you need to take certain precautions to be sure
thattherespiratoritselfdoesnotpresentahazard.

Maintenance,cleaningandcare,andwarningsregardingtherespiratorslimitations:

Chooserespiratorscertifiedforusetoprotectagainstthecontaminantofconcern.NIOSH,the
national Institute for Occupational Safety and Health of the U.S. Department of Health and
HumanServices,certifiesrespirators.Alabelorstatementofcertificationshouldappearonthe
respirator or respirator packaging. It will tell you what the respirator is designed for and how
muchitwillprotectyou.

Do not wear your respirator into atmospheres containing contaminants for which your
respirator is not designed to protect against. For example, a respirator designed to filter dust
particles will not protect you against gases, vapors, or very small solid particles of fumes or
smoke.

Keeptrackofyourrespiratorsothatyoudonotmistakenlyusesomeoneelsesrespirator.

InanyworkplacewhererespiratorsarenecessarytoprotectthehealthofHeardConst.employeesor
whenever respirators are required, Heard Const. will determine proper usage procedures for its
employees.

Heard Const. will provide the necessary training to its employees on the proper usage of the specific
respiratorsusedbyHeardConst.foranyjobtasksrequiringtheirusage.

Training of Heard Const. employees for the proper usage of respirators shall include putting on and
removingthem,anylimitationsontheiruse,aswellastheirmaintenance.

All employees will be fit tested. Fit testing is required before use of any respirator with a negative or
positivepressuretightfittingfacepiece.Theemployeemustbefittestedwiththesamemake,model,
styleandsizeofrespiratorthatwillbeusedinordertocompleteaspecialjobtask.

The program administrator will perform the specific fit testing on Heard Const. employees. Proper
trainingwillbeusedtoqualifytheadministratortoperformthesetasks.

Cleaning&Inspection
Cleaning and Disinfecting: The employer shall provide each respirator user with a respirator that is,
sanitary and in good working order. The employer shall ensure that respirators are cleaned and
disinfected using the procedures in this section, or procedures recommended by the respirator
manufacturer, provided that such procedures are of equivalent effectiveness. The respirators shall be
cleanedanddisinfectedatthefollowingintervals:

Respiratorsissuedfortheexclusiveuseofanemployeeshallbecleanedanddisinfectedasoften
asnecessarytobemaintainedinasanitarycondition.

Respirators issued to more than one employee shall be cleaned and disinfected before being
wornbydifferentindividuals.

Storage
Theemployershallensurethatrespiratorsarestoredasfollows:

All respirators shall be stored to protect them from damage, contamination, dust, sunlight,

orstoredtopreventdeformationofthefacepieceandexhalationvalve.

IV. Hazardous Communication Program


This program has been prepared to comply with the requirements of the Federal OSHA Standards
1926.56(Construction)&1910.1200(GeneralIndustry)andtoensurethatinformationnecessaryforthe
safe use, handling and storage of hazardous chemicals is provided to and made available to all
employees.

A.CHEMICALINVENTORY

Heard Construction, Inc. will incorporate waste minimization and pollution prevention (P2)
considerations and actions into all USA projects. Methods of eliminating waste sources or reducing
wasteproductionshallbeevaluated.Productorprocesschangesthatcouldeliminatetheproductionof
hazardous wastes shall be explored. Work crews shall be actively involved with USA personnel to
identifytheuseofmaterials,processes,orpracticesthatreduceoreliminatethecreationofpollutants
orwasteatthesource.

Design for the environment (DFE) is a systematic method of integrating environmental, safety and
health objectives into the design and planning processes, while considering potential impacts
throughoutthelifecycleoftheproductstobeused.Thepotentialforreuseandrecyclingofmaterialsis
maximized with the entire product lifecycle in mind, while maintaining price, quality, safety and
performancestandards.
HeardConstruction,Inc.willevaluateandinclude(DFE)considerations,wheneverpossible,duringUSA
projects,andseekassistancefromUSAenvironmentalmanagementpersonnelandwebsources.

AffirmativeProcurement(AP)
On 9/14/1998, Executive Order 13101, was signed into law. Known as Greening the Government
ThroughWastePrevention,RecyclingandFederalAcquisition,thisorderisdesignedtofurtherexpand
and strengthen the Federal governments commitment to recycle and buying recycled content and
environmentallypreferableproducts,(includingbiobasedproducts).
The Comprehensive Procurement Guideline (CPG) requires federal agencies and government
contractorstobuyitemsmadefromrecoveredmaterials,andispartoftheEPAscontinuingeffortto
promote the use of materials recovered from solid waste. (Reference FAR Subpart 23.4) The CPG
requiresfederalagenciesandgovernmentcontractorstobuyitemsmadefromrecoveredmaterials,as
designedbytheEPA.Approximately40itemsin8productcategories(includingconstructionproducts)
havebeendesignatedforuse,unlesstheitemsarenotavailablewithinareasonabletimeperiod,failto
meet performance standards of specifications, are not available at a reasonable price, or are not
availablefromasufficientnumberofsourcestomaintainasatisfactorylevelofcompetition.
Examples of construction products that Heard Construction, Inc. will evaluate for use in USA projects
are:Buildinginsulation,carpetcushions,floortiles/patioblocks,laminatedpaperboard,railroadgrade
crossing surfaces, cement/concrete containing fly ash, cement containing Ground Granulated Blast
Furnace Slag (GGBF), latex paint, polyester fiber carpet, shower/restroom dividers and structural
fiberboard.
Heard Construction, Inc. will evaluate and include AP considerations, whenever possible, during USA
projects,andseekassistancefromUSAenvironmentalmanagementpersonnelandwebresources.

PollutionControlandHazardousMaterialsSpills
HeardConstruction,Inc.allemployeesandsubcontractorsshallhaveappropriatespillkits/absorbents
availableatallKSCworksites,andshallreportallspillstotheUSAprojectCTMimmediately.Allfederal,
state,interstateandlocalpollutioncontrolregulationsapplyattheKennedySpaceCenter.Heard
Construction, Inc. shall comply with all applicable pollution control regulations regarding construction
operations and with KHB 1040.1E, Annex K "Hazardous Material Spill Response". Heard Construction,
Inc. shall immediately report (by phone or in person) any pollution incident or waste spill to USA site
Safety, the CONTRACT TECHNICAL MANAGER (CTM), and Environmental Management. A confirming

writtenreport,whichusesinformationprovidedbyHeardConstruction,Inc.willbesubmittedbyUSA
usingKSCForm29864"EnvironmentalReleaseDocumentation"within24hoursoftheincident.
Environmental Management will determine if a KSC Form 21555 "KSC Pollution Incident Report" is
necessary, and will complete and submit any further paperwork required. If needed, Heard
Construction, Inc. will supply additional information about the incident. Employees involved in the
incident shall be counseled, as necessary, to ensure that they are aware of and follow procedures to
preventareoccurrenceoftheevent.HeardConstruction,Inc.maysolicitandreceiveadditionaltraining
and/orinstructionfromUSApersonnel.

TurbidityandErosionControl
HeardConstruction,Inc.shallpreventthedischargeofsedimentintodrainageditches,canals,streams,
rivers or lakes due to construction operations. Approved erosion control devices shall be installed to
prevent the discharge of sediment into any dry or wet watercourse. Erosion control shall consist of
anchoredhaybales,mulchandnetting,filterclothbarriersorothererosioncontrolmethodsapproved
by Environmental Management. Stockpiled fill material shall not be stored in a manner which allows
runoffintoanywatercourse.BestmanagementpracticesforerosioncontrolareavailablefromtheCTM
orEnvironmentalManagement.
HeardConstruction,Inc.shallnotbeginworkinanywatercoursewithoutanapprovedregulatorypermit
obtained from Environmental Management. Heard Construction, Inc. shall comply with the turbidity
controlrequirementsoftheregulatorypermit,whichataminimum,willincludeturbiditycurtains.
HeardConstruction,Inc.shallpreventincreasesinturbidityinexcessofwaterqualitystandardsforthe
receiving water class, unless increases in turbidity are specifically allowed by the regulatory permit.
Should the activities of Heard Construction, Inc., cause an increase of turbidity above the allowed
standard,HeardConstruction,Inc.shallceaseallcontributoryconstructionactivityandinstalladditional
turbiditycontrolmeasures.Allnecessarysoilerosionand/orturbiditybarriersshallbeinstalledpriorto
workefforts,andactivelymaintainedduringKSCprojects.

ConstructionDewatering
Projectspecificguidelines,viathescopeandconditions,and/orprovidedbyUSAenvironmentalaspart
of the environmental handouts during prebid and other project meetings, will outline all required
dewateringinstructions.
Depending on the project and location, this may include dewatering of effluents to grade for natural
percolation back into the surrounding soils, or may involve the use of equipment and/or tanks to be
supplied by others, for the containerization and/or processing of contaminated groundwater at some
locations.Otherprojectsmayrequiretheuseofaspecialconsumptiveuse(dewatering)permit,which
willalsobeprovided(and/oroutline)byUSAenvironmentalduringthebiddingprocess.
Heard Construction, Inc. shall review, use and follow USA environmental guidance and NASA Best
ManagementPracticesforalldewateringactions,andshalluseandsubmitasinstructed,theUSADaily
DewateringLogsforalldewateringactions.

ManagementofKSCBorrowAreas
HeardConstruction,Inc.,withwrittenauthorization,mayusetheKennedySpaceCenterborrowareato
obtainfillforUSAcontracts.Theborrowareashallbemanagedinaccordancewiththeclausesentitled
"TurbidityandErosionControl","ConstructionDewatering","WasteManagementandDisposal",andas
follows:
Any KSC borrow pit areas to be used will be authorized by NASA prior to use. Provisions
governingtheuseandmaintenanceoftheareawillbeoutlinedinadvance,toensureproblem
freeoperations.

ExcavationandSoilDisturbance
Excavation and/or soil disturbance (trenching, raking, hand digging, directional boring, the driving of
anchorsorgroundrods,earthmovementwithheavyequipment,theremovalofgrassortrees,etc.)of
any kind at KSC requires an excavation permit. Heard Construction, Inc. will ensure that all required
authorizationisreceived(fromUSACTM)priortoanysoildisturbance.
Project specific safety and environmental guidance regarding actions, cautions, potential soil
contamination, PPE use, etc. shall be observed by all employees of Heard Construction, Inc. during
excavationwork.
Heard Construction, Inc. will inform employees (including subtier contractors) about the proper
excavation practices to be observed during KSC projects. Any unauthorized excavation or vegetation
removalwillsubjectthesubcontractortopossibledisciplinaryactions.

AbrasiveBlastingandSprayPainting
Abrasiveblastingandspraypaintingwillbe,whereverpossible,performedpriortodeliveryofmaterials
toKennedySpaceCenter.WherefieldoperationsonKSCarenecessaryorrequiredbythespecifications,
HeardConstruction,Inc.shallperformtheoperationswiththefollowingenvironmentalcontrols:
For sandblasting and painting operations, a Material Safety Data Sheet (MSDS) for each paint (and
associated solvents, chemicals, and abrasive blast media) to be used shall be submitted to the CTM
and/or provided to Environmental Management in accordance with KHB 8800.7 "Hazardous Waste
Management". If, after review, USA determines it necessary, they will submit a "Process Waste
Questionnaire"(KSCForm25551)foranevaluationandhazarddetermination.
For abrasive blasting operations on existing KSC structures, the CTM shall have on file the results of
previouslycompletedlaboratoryanalysisoftheexistingpaintsorcoatingstoberemoved.Acopyofthis
analysis, along with a written determination from Environmental Management detailing the nature of
the waste, whether or not it is a hazardous waste, along with copies of the necessary Technical
ResponsePackages(TRP's)andinstructionsforhandlingandprocessingthewasteareavailablefromthe
CTM and/or Environmental Management. Blasting and painting efforts shall not begin until this
determinationandanalysisresultsareonfile.

ForabrasiveblastingoperationsonpaintedstructuressuppliedbyHeardConstruction,Inc.,therequired
analysisresultsand/orsamplesofeachpaintonthestructure(asappliedandcured)shallbesubmitted,
alongwithproductMSDS's,sothatUSAcanprepareaKSCForm25551"ProcessWasteQuestionnaire"
ifnecessarytoobtaintherequiredhazarddeterminations.Forplanningpurposes,aminimumof3work
weeksMAYbenecessaryforthisanalysisanddetermination.
When the item or items being sandblasted are not located over an impervious surface, a temporary
impervioussurface,suchasawaterproofdropcloth,shallbeinstalledbeneaththestructureorobjectto
collect the abrasive blasting debris and the paint overspray. These materials shall not reach the
environment, and any/all measures necessary to prevent these materials from contaminating the
ground,air,andwaterofsurroundingareasshallbetaken.
Weatherconditionsshallbecloselymonitoredtopreventabrasiveblastingdebrisandoversprayfrom
releasetotheenvironment.Duringstrongwinds,operationsshallbeceased.Thedebrisandoverspray
shallbecollecteddaily.Thedebriswillbesampled,storedanddisposedofinaccordancewiththeclause
entitled"WasteManagementandDisposal"undertheHazardousandControlledWastesubheading.If
notpreviouslycompleted,alaboratoryanalysisoftheblastdebriswillberequiredtodetermineifthe
levelofcontaminationwarrantsdisposalasahazardousorcontrolledwaste.
Heard Construction, Inc. shall request and have on file, copies of the hazard determination and
laboratoryanalysisfromEnvironmentalManagementortheCTM,priortodisposalofthesematerialsin
anymanner.
Inearly1997,NASAupgradedtheKSClandfillrequirements,andissuedinstructionsforthedisposalof
spent abrasive blast media at the landfill. This disposal guidance requires that certification forms be
completed and accompany each delivery of this waste material taken to the KSC landfill. Heard
Construction,Inc.willfollowandcomplywithapplicableguidelines,asdirectedbyUSA.

WaterBlastingOperations
NASABestManagementPractices(KBMPIW0001)guidelinesforhighpressurewaterandwetblasting
and cleaning shall be followed, as directed by the CTM and Environmental Management personnel.
Specific instructions, with different levels of guidance (for the type of blasting being performed), as
appropriate,shallbeobserved.
Ingeneral,forfacilities/structureswheretheexistingpaintfilmfailsTCLP:
WaterblastingWILLNOTbeusedasamethodtoremovedirt,rustoroldpaint.
Ingeneral,forfacilities/structureswherethepaintfilmdoesnotfailTCLP:
1. Nowaterblastingoperationdischargewillbereleasedtosurfacewaters.
2. Whenwaterblastingoperationsareperformedoverapervioussurface,a40micronfiltermesh
willbeplacedtoscreenthewastewaterdischarge.Thesolidwasteresiduewillbecontainedand
disposedofatanapprovedlandfill.
3. When water blasting operations are performed over impervious surfaces, operations will be
performedonlyafterpointsofdischargehavebeenidentified.Thedischargewillbechanneled

toperviousareaswithacombinationofsandbagsanda40micronfiltermesh.Residuecollected
fromthesandbagsandfiltermeshwillbedisposedofatanapprovedlandfill.
4. Water blasting will typically be performed using only potable water,. however, the use of
biodegradable, phosphatefree detergents and/or low concentrates (5% or less) of sodium
hypochlorite (bleach), calcium hypochlorite or hydrogen peroxide may be permitted in some
applications, with USA preapproval. No chemicals, cleaners, solvents or detergents will be
added to the water stream or applied to the facility/structure unless previously determined
necessaryandapprovedinwritingbyUSAenvironmentalmanagement.
RecordsofthewaterblastingoperationswillbekeptandsubmittedtotheCTM,perBestManagement
PracticeKBMPIW0001guidelines.
RefertoKBMPIW0001forspecificguidanceinvolvingsurfacecleaning,waterblasting,andwetblasting
operations.

WasteManagementandDisposal
HeardConstruction,Inc.'sworkareasshallbekeptinacleanandsafeconditionatalltimes.Cleanupof
theworkareashallbeconductedatleastdaily.Wasteanddebrisshallnotbeallowedtoaccumulatein
suchquantitiesastocreateanunsightlyappearanceorasafetyorworkhazard.Wastematerialsshall
besegregatedandproperlystoredpriortodisposal.

ConstructionDebrisandSalvageableMaterials
Combustible debris such as cleared trees, shrubs, and roots and scrap lumber shall be removed from
KSCanddisposedofaccordingtoKSCpolicies,andcurrentstateandlocalregulationsunlessotherwise
directedbyaTechnicalResponsePackage.HeardConstruction,Inc.willfollowestablishedKSCpolicies
forsolidnonhazardouswastedisposal,includingspentabrasiveblastmediaandpaintedconstruction
debris.Theprecoordinationoflandfilland/ordumpsterwastematerialsdisposalbyworkcrewswith
theprojectCTMshalloccurasnecessary.
NonCombustible,nonmetallicdebrissuchasunsuitablesoilsandscrapconcreteshallberemovedfrom
KSC and disposed of in accordance with current state and local regulations. Metals, and any material
designated in the contract to be salvaged, will be segregated, protected, itemized, delivered and
offloadedatasalvageareaonKSCdesignatedbytheCTM.Aninventoryofmaterialsshallbeprovided
tothereceivingorganization.

OfficeTrash
When Heard Construction, Inc. maintains an office trailer on KSC, Heard Construction, Inc. shall
participate in the KSC office paper recycling program. Nonconfidential white office paper shall be
segregated and delivered to a consolidated site on KSC. Heard Construction, Inc. shall remove other
nonhazardousofficewastesfromKSCanddisposeoftheminaccordancewithallcurrentstateandlocal
regulations. Office materials such as copy machine or blueprint machine chemicals shall be evaluated
anddisposedofundertheHazardousandControlledWastesrequirementsbelow.

Hazardous,ControlledandUniversalWastes
Hazardous, controlled and universal waste shall be identified, accumulated, and disposed of in
accordancewithproceduresinKHB8800.7"HazardousWasteManagement"andasfollows:
a. Heard Construction, Inc. shall identify all known and expected waste producing processes
including all chemicals, paints, products and solvents and their containers. Submit a Material
SafetyDataSheet(MSDS)foreachwasteproducingsubstancetoUSApriortoproceedingwith
thework.
b. USA will obtain a hazard determination (to identify whether the material is a hazardous,
controlledoruniversalwaste)forthewasteandnotifyHeardConstruction,Inc.Insomecases,
processknowledgemaybeutilizedinlieuofanalysisforfrequentlyand/orpreviouslyanalyzed
materials,atthediscretionofUSAEnvironmentalManagement.
c. If the waste is determined to be nonhazardous and noncontrolled, Heard Construction, Inc.
shalldisposeofitundertheappropriatecombustibleornoncombustiblewasterequirementsas
notedabove.
d. Ifthewasteproducingmaterialsorprocessesaredeterminedtogeneratepotentiallyhazardous
or controlled wastes, Heard Construction, Inc. shall properly contain and store the wastes at
approvedonsiteaccumulationandstagingareasinaccordancewithKHB8800.7andasdirected
byUSA.AnyrequiredlaboratoryanalysisofthewastewillbecoordinatedthroughtheCTMprior
todisposal.USAwillgenerallydirectwastemanagementactionsinwriting.
e. HeardConstruction,Inc.shallrequestpickupanddisposalthroughtheCTMifthewasteistobe
pickedupdirectlyfromthejobsite.Insomecases,suchaswithindustrialwastewaterproduced
bysafetyshowertraileroperations,regularlyscheduledpumpoutactivitiesmayoccuratthe
worksite.ThesewillbeprecoordinatedbyHeardConstruction,Inc.asdirectedbyUSAforeach

HeardConstruction,Inc.willprecoordinatewiththeCTMforplacingwasteina90daystaging
area located at K61896 (or as instructed) if a USA approved collection area has not been
established at the jobsite. It is Heard Construction, Inc. responsibility to ensure that waste is
properlysegregated,identified,labeledandpackagedinaccordancewiththecorrectTechnical
ResponsePackage(TRP).TRP'swillbeprovidedbyUSA(Generallyatthepreawardconference
or as needed) using information contained in the engineering and/or work package, from
supplied MSDS sheets, as well as information provided by the CTM and contractor personnel.

The method of recovery and refillable containers shall conform to ASHRAE Guidelines 31990. The
containersshallbelabeledandturnedovertotheCTM,orasdirected.

on site employees will have access to a copy of Heard Constructions Hazardous Communication

Personnel, tools and equipment have specific tethering requirements. Know the rules before entering

Within the processing areas and construction sites, much work requires handling of hazardous

Onfacilitystructuresandareaswithgrateflooringorexposedcables,shoesmustcovertheentirefoot,

Housekeeping
Housekeepingisdefinedasthearrangementofoperations,tools,equipment,materialsandsuppliesin
anorderlymannerconducivetothesafetyandwelfareoftheworkteam,andforthegoodappearance
oftheworkareas.
Goodhousekeepingisessentialtosafeoperations.Keepallfloorswithintheworkareasdry,cleanand
freefromobstructions.Promptlycleanuporremoveanyspilledoil,materialsorliquid.Accumulationof

ManualLiftingTechnique
Lifting safely is an individual responsibility. Safe lifting means keeping the back aligned while lifting,
maintainingyourcenterofbalance,andusingyourlegstoperformthelift.
PersonalProtectiveEquipment(PPE)includesdevicesandclothingdesignedtoprotectindividualsfrom
jobrelatedhazards,suchasharmfulobjects,substances,conditionsorradiation.
Each job is assessed to determine if hazards are present and to establish requirements. Know what
personal protective equipment is needed for each work area prior to entering. When using PPE, it is
important it fits properly each time it is worn and is comfortable as to provide free movement when

Training

LightningWarningPolicy
The Lightning Warning Policy will be implemented upon notification issued by the Cape Canaveral
Forecast Facility (CCFF) for specific KSC areas/facilities. The following restrictions will apply, unless
specificallyidentifiedotherwise.

Phase One Lightning Advisory: (Lightning within 25 miles) adverse weather conditions are
developing. Prepare to cease operations on short notice. Announced when an observable
weather system is approaching, or producing lightning anticipating to arrive at specified KSC
area within 30 minutes. Advisories are intended to provide personnel conducting hazardous
operations sufficient lead time to secure the operation before the forecasted weather system
arrives.
Phase Two: (Lightning within 5 nautical miles) or the threat of lightning is immediate, i.e.,
increasing growth or rapid movement of a thunder cell. Personnel shall cease all work in
unprotected outside areas and seek shelter in buildings, metal structures, or metal enclosed
vehicles. All KSC buildings and metal structures that are daily work areas, are considered
lightningprotectedsheltersforpersonnel,unlessspecificallydesignatedtobeevacuated.

Iflightningwarningisannounced,personnelobservinglightningconditionshavetherightandobligation
toadvisetheirsupervisionandrequestalightningwarning.
Personnelaccesstoroofsortoplevelsofstructuresisprohibited.

HurricaneConditions(HURCON)
All preparations are based on weather severity (winds 50 knots or greater) and projected hurricane
distance/directionfromKSC.Conditionsareasfollows:

FallRestraintSystem

A fall protection system that prevents them from reaching an

components typically include a lanyard and may include a lifeline and

FloorOpening

An opening measuring 12 inches or more in its least dimension in any

FreeFall

The act of falling before a personal fall arrest system begins to apply
forcetoarrestthefall.

FreeFallDistance

The vertical displacement of the fall arrest attachment point on the

AuthorizedPerson(User)
Eachauthorizedpersonshallprotectthemselvesbyapplyingfallprotectionpracticesduringthecourse
of the work. When the nature of the work, the workplace, or the methods of control change to an
extent that prior training is not adequate, the authorized person shall be retrained. Retraining is also
required when it becomes apparent to supervision, a competent person, and/or qualified person that
theauthorizedpersondoesnothavetherequiredlevelofskillsandknowledgeorisnotfollowingthe
requiredmeansandmethods.
a. Each authorized person shall inspect, install, use, and dismantle fall protection equipment
accordingtomanufacturerinstructions,OSHArequirements,andlocalpolicy.
b. Theauthorizedpersonshallnotconductanyworkwhererequiredfallprotectionisnotinplace
or the performance of the fall protection system is unpredictable. In these situations the
competentpersonforthatworkorareashallbenotifiedfordeterminationoftheappropriate
actiontobetaken.
c. Every authorized person shall receive formal classroom training in fall protection from a
competentpersonorqualifiedpersonbeforetheyusefallprotectionsystemsorareexposedto
afallhazard.Trainingforauthorizedpersonsshallinclude:
1) Howtoselectanonengineeredanchoragethatwillsupport5,000pounds
2) Howtoinspect,anchor,assemble,andusethefallprotectionequipmentcommonlyusedin
locationswheretheywork
3) Fallhazardrecognition
4) Fallhazardeliminationandcontrolmethods
5) Applicablefallprotectionregulations

d. Theauthorizedpersonshallremaincurrentwithchangingrequirementsandshallberetraineda
minimumofonceeverythree(3)years.

FallHazardControls
Thefollowingcontrols,inorderofpreference,shallbeused:
a. Hazard Elimination: The specific work that created the fall hazard should be evaluated to
determine if a change in process, area, technology, or equipment would eliminate the fall
hazard.
b. Guarding: Physical barriers between the worker and the fall hazard can be established so the
barrierpreventstheworkerfromfalling(i.e.Guardrails,verticalnetting,covers,etc.).
c. FallRestraint:Fallprotectionpersonalprotectiveequipmentcanbeusedtoassembleasystem
(permanentortemporary)thatwillpreventaworkerfromreachingthefallhazard.
d. Fall Arrest: Fall protection personal protective equipment can be used to assemble a system
(permanent or temporary) that allows a worker to fall, but arrests the fall safely before the
workerstrikesthegroundorsurroundingstructure.
e. AdministrativeControls:Administrativecontrolsarereservedforsituationswhereallotherfall
protectionmethodsaredeemedinfeasible.Thisoptionisavailableonlytoemployeesengaged
in leading edge work or precast concrete erection work. The methods used for this plan shall
conformto29CFR1926.502(k)(1)through(10).

FallProtectionSystemPerformance
FallRestraintSystems

FallArrestSystems
Fall Arrest Systems shall be designed to meet the requirements set forth in the applicable OSHA
subpart(s). ANSI Fall Protection Standards will be followed except where deemed inappropriate by a
qualifiedperson.Fallarrestsystemsshallbeusedandinstalledtoreducethefreefalldistance.
a. Whenusingfallarrestsystems,thefreefalldistanceofthesystemshallnotexceed6feetand
900 pounds maximum arresting force except when designed by a qualified person. In no case
shall the manufacturer instructions be violated without the approval and documentation of a
qualifiedperson,norshallanarrestingforceof1,800poundsbeexceededinafallgreaterthan
6feet.
b. The installer of the fall protection system (authorized / competent person) shall be
knowledgeableofthearrestingforceandthetotalrequiredclearanceofthesystem.
c. Ifthesystemisbeinginstalledforworkersotherthantheinstalleraprocedureshallbeprovided
toensurethattheauthorizedusercandeterminethatthesystemisapprovedforuse.
d. Acceptable anchorage structure may be Ibeams, columns, tower legs, stairwell support
structure,orotherstructurethatiscapableofholding5,000poundsperpersonordesignedbya
qualifiedpersonwithasafetyfactorof2.Iftheanchorageisquestionable,thesystemshallnot
beusedandacompetentorqualifiedpersonshallbenotified.
FallArrestandRestraintSystemComponents
FallarrestandrestraintsystemcomponentsshallmeettheOSHAstandards,andANSIZ359.1Standard
when applicable. Only a qualified person is permitted to approve not complying with ANSI and those
casesmustbedocumentedasindicatedinthispolicy.
PersonalFallArrestSystem
A personal fall arrest system shall be used in conjunction with a work positioning system. It shall be
riggedsotheemployeeshallnotfreefallmorethan2feet.

FallProtectionEquipment
a. Fall protection equipment shall be selected from equipment meeting or exceeding the OSHA
andANSIFallProtectionStandards.All newfall protectionequipmentmust meetANSIZ359.1
currentatthetimeofpurchase.
b. Fallprotectionequipmentshallbeusedaccordingtomanufacturerinstructions.Onlyaqualified
person may change the instructions, and those changes are documented prior to use and
maintaineduntiltheequipmentisremoved.
c. Equipment not designed for fall protection use shall not be used without prior approval of a
qualifiedperson,documented,andbelabeledForFallProtectionUseOnly.
d. Theprogramadministratorisresponsibleforensuringthatfallprotectionequipmentprocuredis
includedinthetrainingofcompetent,authorizedandapplicablequalifiedpeople.

FallHazardsandAssessments
FallProtectionforWalking/WorkingSurfaceswithUnprotectedEdge/OpeningorotherFallPotentialof
FourFeetorGreatertoSurfaceBelow
a. Ifavehicleortrailerisusedasawalking/workingsurfaceotherthannormaloperationalmodes
suchasoperation,maintenance,transport,loading,offloading,theneedforfallprotectionshall
beevaluatedbyacompetentperson.
b. If work requires employees to be closer than 6 feet from any unprotected edge or opening,
positivefallprotection(Guardrailsystemorpersonalfallprotectiontoincludeharnessw/arrest
orrestraintsystem)ortheuseofothermeanssuchasvehiclemountedworkplatformsand/or
scaffoldingisrequired.
NOTE:Becautiouswhenusingguardrailsorchainsasasecondarymeansofpositivefallprotection.Such
mechanisms may be subject to failure due to wear and tear and/or faulty design, construction,
installation,andtesting.Personnelshouldnotleanonguardrails,gates,orchains.
HorizontalLifeLines
Commercially available ANSI approved temporary horizontal life lines are to be installed per
manufacturers written instructions. Only a qualified person may change the instructions, and those
changesaredocumentedpriortouse.Ifnotcommerciallydesigned,thenaqualifiedpersonmustdesign
andprovideinstallation.
Over/NearWaterOperations
IfemployeesarerequiredtoworkOver/NearWatertheyshallbeprotectedfromfallingasrequiredby
this policy. This section applies to construction activities, and is not intended to apply to marine
operations that are governed by 29 CFR 1917, Subpart B, Marine Terminal Operations. For
construction/maintenanceactivitiestherequirementsof29CFR1926.106shallalsobecompliedwith.
TowerClimbingOperations
Climbingtowerspresentuniquehazardsnotassociatedwithotherjobsrequiringfallprotection.Priorto
towerclimbingitisrequiredthatthefollowingbeaddressed:weatherconditions,coordinationwithFire
Rescue, development of a rescue plan, safe transport of equipment/tools, tower energy sources have
beensafed,andatleasttwoclimbersarepresentandtrainedtoclimbtowers.
RoofsFallProtectionRequirements
a. RoofWorkOnRoofs
1) Metal Roof: Work on metal roofs requires the use of positive fall protection at all times
regardlessofslope.

applies.

Extensible/ArticulatingLifts
Anenergyabsorbinglengthadjustablelanyardandfullbodyharnessshallbeused.Thelanyardshallbe
connected to an approved anchor point in the basket and adjusted in length in such a manner that it
reducesthepossibilityoftheworkerfallingovertheguardrailsyetpermitstheworktobeaccomplished.

Scaffold
a. Scaffoldplatformswherethepotentialforafallfourfeetorgreaterexistsshallbeprotectedby
the use of fall protection systems (guardrails, fall arrest, fall restraint). If these cant be
accomplished, a qualified person shall develop a fall protection plan for scaffolding up to ten
feetinheight.Ascaffoldgreaterthantenfeetshallhavepositivefallprotection.
b. A competent person shall evaluate the scaffold erection and dismantling to determine if fall
protection systems (guardrails, fall arrest, fall restraint) can be used while erecting and
dismantling scaffolds. If these cant be accomplished a qualified person shall develop a fall
protectionplan.

1) Thenatureoffallhazardsintheworkarea.
2) Thecorrectproceduresforinstalling,inspecting,anddisassemblingfallprotectionsystems.
3) Theuseandoperationoffallprotectionsystemstobeused.
4) Eachemployeesroleinthesafetymonitoringsystem,ifthissystemistobeused.
5) Thelimitationsontheuseofmechanicalequipmentonlowsloperoofingjobs.
6) The correct procedures for the handling and storage of equipment and materials and
installationofoverheadprotection.

4.FallProtectionEquipment
a. FallprotectionequipmentshallmeetallOSHA,applicableANSI,andmanufacturerrequirements
foruse,andbeproperlystoredwhennotinuse.
b. Allfallprotectionequipmentshallbeinspectedbytheuserbeforeeachuseandbyacompetent
person (fall protection) annually in accordance with applicable regulatory standards or per
manufacturersrecommendations,whicheverismorestringent.
c. Annual inspections of fall protection equipment shall be documented and meet the following
criteria:
1) Allfallprotectiveequipmenttoincludesafetyharnesses,droplines,lifelines,fallarrest,as
well as positioning lanyards, ladder safety climb, and rigid rail sleeves (e.g., Skates, Rope
Grabs)shallbeinspected.
2) Selfretracting lanyards shall be visually inspected and checked according to the
manufacturers recommendations to ensure there are no obvious signs of damage due to
wearandtear.
3) Allequipmentshallhaveamanufacturersserialnumberonit(e.g.,tag,webbing)oritshall
beserializedbyusingamethodnotdestructivetotheequipment.
4) Maintenance servicing must only be completed by the manufacturers approved service
technician,trainedtorepairandservicetheirequipment.
d. A documented equipment tracking system that uses the method identified from 4.c(3) above
shallbeused.

e. Any fall protection equipment that is missing an inspection tag or is past due for annual
inspectionshallbeimmediatelyremovedfromservice.
