Employee Contract

DURATION:
This contract is for a duration of one year with the option for renewal of additional years
thereafter.
RESPONSIBILITIES:
As a nurse practitioner the employee will be responsible for patient care that includes but is not
limited to preventative care, patient education, complete physical exams, diagnoses of acute and
chronic conditions, and treatment of said conditions.
PROVIDING CARE:
The role of the Nurse Practitioner includes completing physical exams, ordering diagnostic tests,
diagnosing diseases, and treating said diseases.
A. In performing the physical exam the nurse practitioner, through their training, will
identify all medical conditions which affect the patients current health.
B. The use of diagnostic testing is crutial for accurate diagnosis of disease processes.
The nurse practitioner will be required to order and interpret diagnostic

labs/procedures to include but not limited to: Complete Metabolic Panel, Urinalysis,
Complete Blood Count, Lipid Profile, Thyroid Function Test, X-ray, Ultrasounds, CT
scan, MRI
C. In treating the diagnosed health conditions the Nurse Practitioner will be required to
prescribe medications, perform simple procedures, and make referrals to specialists.

The Nurse Practitioner will prescribe medications to effectively treat the


diagnosed diseases in accordance with state and federal laws. The Nurse
Practitioner will be able to prescribe medications that include but are not
limited to diuretics, NSAIDs, antibiotics, statins, and antihypertensives.

The Nurse Practitioner will perform simple procedures as necessary skin


biopsies, skin tag removal, excision of cysts and other skin abnormalities,
and cleaning/suturing/dressing of wounds.

WORKING CONDITION/TERMS OF WORK:


A. The Nurse Practitioner will be required to work Monday through Thursday 9:00 am
to 5:00pm and Fridays 9:00 am to 12:00 pm. The nurse will be given 30 minutes for
lunch Monday through Thursday.
B. In the event of an acute illness the Nurse Practitioner will contact the responsible

physician immediately. The employee will be given one week of sick leave per year.
In the event of an illness requires hospitalization or the employee will need additional
days beyond the given week the employee will be required to provide documentation
from their primary care physician. The use of more than one week of paid sick leave
will result in forfeiture of vacation days.

C. The Nurse Practitioner will be given two weeks of vacation for the first year of

employment. After three years of employment the Nurse Practitioner will accrue a
third week of vacation time and after the fifth of employment the Nurse Practitioner
will accrue a fourth week. The Nurse Practitioner will give a request for use of
vacation time no later than 4 weeks prior to the proposed vacation. The request for
use of vacation time will be given to the employer in a written request which will be
accepted or denied within one week of the request.
D. Paid holidays will include all days the office is closed. The these holidays include
New Years eve, New Years day, Memorial Day, July 4th, Thanksgiving and the day
after, Christmas Eve and Christmas. These days will be paid but will not be deducted
from the employees vacation days.
E. Request for unpaid time off will be given to the employer in a written request and will
be confirmed or denied at the discretion of the employer depending on the patient
load and the circumstances of the request.
COMPENSATION:
A. The annual salary of ____________ will be paid to the Nurse Practitioner on the
same pay schedule of all other employees.
B. As this contract expires after the first year, the nurse practitioner will be considered
for an increase in pay at the renewal of the contract. The employer will conduct an
evaluation of the employee and provide the results to the nurse practitioner in written
form.
BENEFITS TO THE EMPLOYEE/PAID EXPENSES:

A. The employer will make available to the Nurse Practitioner medical insurance plans
that are available to all other employees. If the Nurse Practitioner does not find any
of the plans acceptable the Nurse Practitioner may find an insurance plan through a
private company. The employee will submit the plan request to the physician which
details the costs, insurance company name, and benefits to the Nurse Practitioner.
B. Malpractice insurance will be covered by the employer for the duration of the Nurse
Practitioners employment. The Nurse Practitioner may purchase additional
insurance if they deem it necessary but this will be at the employees expense.
C. The employer will pay for any and all license renewals, continuing education,
prescriptive fees, and professional memberships that directly relate to the Nurse
Practitioners employment.
NON COMPETE:
Due to the skills and knowledge gained through employment of this facility the Nurse
Practitioner may not gain employment in a like specialty within a 30 mile radius for the course of
one year. The Nurse Practitioner may be employed in any other type of specialty or family
practice that does not directly compete with this facility.
TERMINATION:
A. The employer may terminate this contract at any point during the 90 day introductory
period. The employer has no responsibility to the Nurse Practitioner at that point
other than to pay for services rendered and time given.
B. The Nurse Practitioner may terminate this contract at any time if the employer is
provided with a written notice within 60 days of termination of this contract. The

employer may end the contract at any time after the written notice is given but the
Nurse Practitioner will be provided any and all compensation of time up to that point.
