All praise be to Allah, Lord of the worlds, Eternal Guardian of the Heavens and the Earth,
Disposer of all created beings, Dispatcher of messengers (peace and blessings be upon
them all). I bear witness that there is no god but Allah the Creator and Sustainer, who
created death and life, and I bear witness that Muhammad is His servant and messenger.
I, (insert name), residing at (insert address), being of sound mind and body, do hereby
revoke any and all former wills and codicils thereof made by me, and in lieu of declare
this as my last will and testament. My advice to my relatives and friends, my Muslim
brothers and sisters, is to strive to be true Muslims, and submit to Allah Almighty and
worship Him alone as He alone is to be worshipped. Let them obey Him and hold fast to
His book (the Quran) and die not except in a state of Islam.
ARTICLE 1: FUNERAL AND BURIAL RIGHTS
I ordain that no autopsy be done on my body, that without justified delay, my body should
be handled in accordance with the Islamic funeral process. I ordain that absolutely no
non-Islamic religious service or observance shall be conducted upon my death, or on my
body. I ordain that my body shall not be transported over any unreasonable distance from
the locality of death (particularly when such transportation would necessitate embalming)
unless decided by my Muslim family members.
I ordain that my body should be washed and prepared first by (insert name and
relationship to you). If he/she is unable to perform this duty, then I ordain that righteous
individuals chosen by my spouse or my nearest Muslim relative shall wash my body.
Such individuals should be known for their upright character and knowledge of the
Janaza process.
If I am under any circumstances physically disabled to the point that I am unable to direct
my own medical care, or communicate my needs, and/or my medical condition deems the
use of artificial means to maintain the basic body functions for any extended period of
time, I direct that such medical intervention should be stopped in the following two cases:
1. My spouse or nearest Muslim family member deems that such treatment should
be stopped or

2. The doctors in charge of my care declare that such treatment is unlikely to


improve my condition.
This clause does not preclude the use of a feeding tube or other device used to administer
nutrition, it applies only to medical interventions that maintain life processes in place of
my own body.
ARTICLE II: DEBTS AND EXPENSES
I direct that all trust propertied (listed below) shall be returned to the rightful owners. I
further direct that my executor hereafter named first apply the assets of my estate to the
payment of all my legal debts, including such expenses incurred by my last illness and
burial as well as the expenses of the administration of my estate. I direct said executor to
pay any outstanding obligations to Allah (huquq Allah) which are binding on me
including unpaid Zakah, or kaffarat and vows. I further direct that all inheritance, estate,
and succession taxes payable by reason of my death shall be paid out of and charged
generally against the principal of my residuary estate.

ARTICLE V: DISTRIBUTION OF ESTATE
I direct and bequeth all the residue and remainder of my estate after providing for debts,
trusts, and burial expenses be allocated according to Islamic law. The manner and
divisions of allocating the estate shall be decided by a competent scholar who is
knowledgeable in the field of inheritance law in consultation with my executor.

competent jurisdiction, that other parts shall remain valid and enforceable.
