COMPLAINT AFFIDAVIT
I, Joseph O. Olaes, 31 years old, Filipino, single, a resident of 001 P.
Guevarra Extension, Brgy. Pagswitan, Sta. Cruz, Laguna, Philippines, after
having been duly sworn to in accordance with law, hereby depose and state
that:
1. That on November 6, 2007 at around 10 oclock in the evening,
while I was heading home from buying medicine from Mercury
Drug, I met James R. Dizon at dark area before our house;
2. That without warning, he suddenly attacked me with a 2x2 piece of
wood while saying, Mayabang ka masyado! Dapat sayo
tinuturuan ng leksyon!;
3. That due to the said incident, I suffered an 8-inch wound on my
head, a fractured left arm and 2 broken ribs due to severe beating;
and has been confined in the hospital for 11 days and advised to a
complete rest for 3 weeks or more until my arm brace has been
removed and my ribs have completely healed. Attached herewith
as Annex A is a copy of the medical certificate issued by my doctor
bearing the aforementioned facts.
4. I am executing this complaint-affidavit in order to file a case of
Serious Physical Injuries against James R. Dizon.
