ANNUAL HEALTH EXAMINATION OF TEACHING AND NON-TEACHING

PERSONNEL

Teaching and Non-Teaching Personnel

1.
Pursuant to Regional Memorandum No. 147 s. 2012 dated November 22, 2012 regarding
Annual Physical Examination of DepEd Region III Employees (Teaching and Non-Teaching
Personnel), all Teaching and Non-Teaching Personnel are required to undergo Annual Physical
Examination in compliance with the Civil Service Memorandum Circular No.17 s. 1989. This is
to ensure that all personnel are physically fit to perform their assigned roles and functions.
2. The Annual Health Examination of all DepEd Teaching and Non-Teaching employees shall
start on April 6, 2015 up to June 30. 2015 so as to allow enough preparation for the said activity.
3. The cost of all Laboratory and Ancillary Examinations shall be shouldered individually by all
Teaching/Non-Teaching Personnel. Each personnel are entitled to choose his/her Reliable
Laboratory Medical Service Provider. A copy of the schedule for the Annual Health Examination
shall be prepared and distributed in advance to each District/ Secondary School for compliance.
4. The DepEd District Nurses shall consolidate and make initial evaluation of all
Laboratory/Ancillary Results for each District (Elem/ Secondary Schools). All laboratory results
of Teaching/ Non-Teaching Personnel with significant findings shall be forwarded to the DepEd
Medical Officer for further evaluation and management.
5. All Laboratory Test Results should be available not later than March 30, 2015. Each School
Head should inform the concerned DepEd Nurses so that we can proceed with the contemplated
activity.
6. The Clinical Evaluation and Management shall be done by the DepEd Medical Officer for
those who have significant findings in their Laboratory Results. Signing of Clearances will be
done after the evaluation.

Failure to comply with the Annual Health Examination, No signing of Clearance.

7. The following Laboratory Examinations as prescribed by Regional Memorandum No. 147 s.

2012 shall be required to all Teaching and Non-Teaching Personnel:
a) Chest X-ray (10 x12)- Except for Pregnant Patients
- If the Female Personnel is not sure if she is Pregnant or Not, Please inform the
Radiologic Technologist/ Nurse or any Medical Personnel before doing any Xray
Procedure.
b.) 12-L ECG for 40y/o and above
c.) Fasting Blood Sugar
d.) Total Cholesterol
