NOTE
The intent of this document is to honour all HUET training tickets for their current two-year
validity.
After March 31st 2000 all Woodside employees or Woodside paid Survival Training will concur
with this document.
1 Definitions
1.1. Offshore Asset
The term Offshore Assets as used in this document, means any offshore Assets including
platforms, FPSOs, MODUs, vessels and the like, which have helicopter landing facilities.
1.2. Person(s)/Personnel
The term Person(s) or personnel means those who normally access an Offshore Asset via
helicopter and includes Woodside and/or its suppliers, contractors, subcontractors and consultants
and all other persons who are required to visit an offshore asset. Where required, persons not
Woodside personnel, are collectively referred to as "Contractor Personnel".
Appendix 1
Categories and Training Requirements for Offshore Facilities
DEFINITIONS
The following categories of offshore personnel are used in identifying training requirements. The
Woodside categories and training requirements reflect the Guideline for Preparation of Safety
Cases, July 1994.
CATEGORY 1
(Personnel permanently, regularly and occasionally assigned to offshore assets.)
All personnel who travel and work offshore must have a valid offshore survival certificate. The only
exception to this are those classed as visitors (see definition below).
It should be noted that permanently assigned or regularly assigned means permanently or
regularly assigned to work offshore in general and does not necessarily mean being assigned to a
specific asset. Similarly, those who work offshore occasionally is not asset-specific. All days or
nights spent offshore, or anticipated to be spent offshore, are to be counted.
CATEGORY 2
Visitors
Personnel who may visit offshore and will do no more than three trips in a 12-month period, and
spend no more than two nights offshore in the same year. When outside the accommodation and
while flying in a helicopter, visitor will be accompanied and escorted by a person in category 1, so
far as is reasonably practicable.
The approval of visitor status resides with the Asset Manager. For personnel to be granted visitor
status and travel offshore without survival training will only be for, compassionate reasons, safety
critical visits and one off visits. An application for offshore visitor travel must be filled out prior to the
visit. (See appendix 7.)
CATEGORY Z
Not permitted to travel offshore.
SURVIVAL TRAINING
CATEGORY 1
Attending an approved offshore survival and firefighting course, which includes practical skills
in Helicopter Underwater Escape Training (HUET); Sea Survival; Basic fire fighting; and smoke
evacuation techniques.
Note: Initial training should be conducted over a minimum of two days, subsequent refresher
training should be conducted over a one-day duration. Where prior learning is recognised either by
previous training or work experience the refresher training is sufficient.
Installation Inductions
Personnel should be made aware of:
Attending an induction program just after arriving on an assets for the first time. The training
should include being shown the location and operation of emergency equipment on the assets;
and
Training in the location and use of assets specific survival craft and other escape equipment.
Attending a helicopter safety briefing prior to each flight or within twenty-four hours of flying, if
the same type of aircraft is used.

Visitors should know:

the satisfaction of the Director in the safety case submission, be accompanied by a
category 1 person.

Appendix 2
Booking Procedure for IFAPs Offshore Survival Training courses;
1. Woodside Personnel
The Woodside representative of the course nominees department must confirm the training
needs of the individual and telephone IFAP on 9310 3760 to determine course availability
and make provisional bookings.
All final bookings must be authorised by the relevant manager or supervisor and confirmed
via fax to IFAP by close of business two working days prior to the course date - the
attached booking pro-forma must be used in all cases. The completed form must contain:
clear identification of the person responsible for the booking,
fax number or address for trainees' joining instructions;
correct cost codes.

Offshore Operations and associated contractors and by the training course provider for all
other attendees.
2. Contractor Personnel
The Contractors representative must confirm the training needs of the individual and
telephone IFAP on 9310 3760 to determine course availability and make provisional
bookings and inform IFAP whether or not the booking is to be made under the training
contract between Woodside and IFAP.
All final bookings must be confirmed via fax to IFAP by the close of business two working
days prior to the course date. The attached booking pro-forma must be used in all cases.

Contractors are eligible for the Woodside course rate, if their contract extends over
the validity of the training. A valid Woodside contract number and the name of the
Woodside representative are required to enable IFAP to invoice Contractors at the
prevailing IFAP / Woodside training course rates.
Failure to provide information will result in Contractors being invoiced by IFAP at the ruling third
party rate. Clear details, including the Contractors official Order, must accompany the form to
facilitate direct invoicing of the course fees by IFAP to the Contractor.
Joining instructions will be sent out to nominated personnel by IFAP for all attendees, except
Offshore Operations staff and Contractors who will receive their instructions directly from the WEL
focal point.
NOTE: Failure to send IFAP a properly completed form will result in the booking being cancelled.
The Contractors representative must ensure that if an alternate training provider to IFAP is
used then the individual contractor is responsible for bookings, confirmation and payment.
Appendix 3
Medical Requirements for Offshore Survival Training
All course nominees, both Woodside and contractor personnel are required to undergo a medical
examination within three months prior to each attendance at survival training. This examination
must be conducted by a Woodside nominated Medical Practitioner (see Health Assessment
Procedures, or the trainee must provide satisfactory alternative evidence of having been passed as
medically fit by UKOOA or Woodside standards within the three months immediately preceding the
course.

All Persons must utilise one of the Woodside approved Medical Practitioners. Alternatively,
Woodside Persons may utilise GWA / NRA based Medic or the Karratha based
Occupational Health Nurse. All the nominated Practitioners, the GWA / NRA based Medic
and the Occupational Health Nurse, have the necessary Medical Advice forms and
guidelines.
Persons must inform the Medical Practitioners reception that the appointment is for the
purpose of a medical examination required as a prerequisite to an Offshore Survival
Training course and subsequent working at an offshore asset.
All persons must provide the Medical Practitioners reception with clear details to facilitate
direct invoicing of the medical examination fee to either Woodside or to a Contractor.
Following the medical examination, the examining Practitioner (or Medic / Occupational
Health Nurse) will:
provide the Person with a Woodside Offshore Medical Advice Form, (WOMAF),
which gives summary advice regarding the employee's medical suitability for
survival training,
mail the full medical examination report directly to the Woodsides Employee Health
& Rehabilitation Co-ordinator (WEHRC).
only in the event that the examining Medical Practitioner required further clarification
regarding the suitability of the Person for Offshore Survival Training, will the
WEHRC specifically review an individuals case. In such an instance, the WEHRC
will advise the Woodside Person or the Persons supervisor, or in the case of a
Contractors employee, the persons employer.
The person making the course booking must be in possession of an approved WOMAF, before
proceeding with the booking formalities.
Overseas personnel required at short notice, or their employers, may be given (via fax) copies of;
1. Woodside Guidelines for Medical Practitioners,
2. Woodside Medical Examination Report - Offshore, for use in an examination by a suitable, local
registered medical practitioner. Alternatively, overseas personnel in this situation should be
advised to seek medical certification in accordance with UKOOA Guidelines for Medical Aspects
of Fitness for Offshore Work, 1992.
In either case, the completed medical examination report form or medical certification as
appropriate, must be faxed direct (to maintain confidentiality), to WEHRC, on fax number (08) 9348
4431. In these cases, the Woodside representative should advise the HSER Department that such
a process is under way.

Evidence Guide:
Competency will be demonstrated through simulator exercises in underwater escape.
Representative doors and window mechanisms must be used during the exercises.
Competency must be demonstrated by successfully jettisoning doors and windows and making an
unaided escape from an inverted simulator.
Sea survival competency can be demonstrated through, practical drills, role-plays, and simulations.
Basic Fire Training Control small uncomplicated emergencies.
Performance Criteria:
Appropriate equipment is selected to control small emergency situations.
Equipment is checked to ensure it is safe and ready for use.
Equipment is used in accordance with accepted safe practices.
Australian Standards colour coding and equipment is used during the training.
Range of Variables:
Equipment should include: Portable fire extinguishers, fire hose reels, fire blankets, spill control
equipment.
Other emergency actions may include shutdown procedures or evacuation.
Emergency situations may include: fires of the A,B,C, D, E and F classes, fuel and other spills,
process overheating and equipment failure.
Evidence Guide:
Competency can be demonstrated under simulated emergency conditions.
Underpinning knowledge should be assessed through oral or written questions on the following:
Identifying the types of emergencies.
Selecting appropriate fire fighting equipment.
Using fire fighting equipment
Understanding the theory of fire.
Understanding the implications of the incorrect use of equipment.
Describing the safe use of fire fighting equipment and co-ordination with other actions.
Emergency Evacuation from smoke filled accommodation:
Performance Criteria:
Emergency evacuation from a smoke filled multi-levelled accommodation block is conducted
according to emergency plans, procedures and training.
Arrangements are made for the care of the unconscious or injured during the evacuation.
Range of Variables:
Emergency evacuation maybe total or partial to an external or internal refuge.
Emergency evacuation will include the use of smoke hoods or other breathing apparatus.
Expertise limited to general awareness of likely emergency behaviour.
Mobility impaired people may require assistance during the emergency.
Evidence Guide:
Competency will be demonstrated through simulations, role-plays or case studies.
Underpinning knowledge should be assessed through oral or written questions on the following:
Understanding emergency evacuation signals, alarms and procedures.
Understanding emergency plans.
Understanding arrangements for evacuation of mobility-impaired people.
Recognising the hazards and the precautions necessary during evacuations.
