Admission Assessments are scheduled by calling 419-530-2673. No walk-ins permitted. Testing is in Collier Building on
Health Science Campus; room will be posted. Park in Lot 44. Map http://www.utoledo.edu/campus/virtualtour/index.html

Test Dates: Jan 16 (8-12), Feb 13 (2-6p), Mar 20 (8-12), Apr 17 (8-12), May 15 (8-12), June 19 (8-12)

Arrive early. Bring a photo ID. Have at least two sharpened pencils with erasers. Electronic devises (calculators, cell
phones, etc) are banned during testing. Students observed cheating receive “zero”. Answers are written in space
provided or bubbled in on form. Answers written elsewhere are not graded. Partial credit is not provided; either the
answer is correct or incorrect. The full assessment is taken only one time. One retake of the math test is permitted;
student is responsible for the testing cost. Applicants not passing the second math test are ineligible for admission.

Students needing assistance with math, reading or critical thinking are advised
to meet with the nursing advisor for referral to appropriate university resources.
MATH ASSESSMENT:
Mastery of basic mathematics is associated with success in the nursing major and is essential for performing medication calculations in
the clinical setting. Misplacement of a decimal point or a small calculation mistake may seem trivial, but an error makes a difference in
patient outcome. The assessment contains 50 problems; do not memorize metric to other measurement systems conversion. Ten
items require reading a statement to determine the answer. Change all fractions to decimals, making sure to carry your answer out to
the farthest point.

The doctor writes a prescription for Ampicillin 500 milligrams four times a day. The container is labeled Ampicillin 250 milligrams per
tablet. How many Ampicillin tablets does the nurse give the patient at 6:00pm?

READING ASSESSMENT
Ability to read complex content is important for nursing student success. There is no strategy for practice on this two-part assessment,
that involves matching words with meanings and answering questions about a reading passage.
