Inflamasi pada Penderita Tuberkulosis (TB) Aktif, Kontak TB-IGRA Positif
(TB Laten) dan Kontak TB-IGRA Negatif (Sehat): Kajian terhadap Kadar
Interleukin-8 (IL-8), IL-10 dan IL-12 (Dibimbing oleh Muh. Nasrum Massi,
Syakib Bakri, Budu Mannulusi)
Tuberkulosis masih menjadi masalah kesehatan utama secara global.
Seseorang yang terinfeksi Mtb dapat langsung mengeliminasi kuman secara
sempurna (sehat) atau berkembang menjadi TB aktif maupun TB laten.
Beberapa sitokin telah teridentifikasi berperan terhadap infeksi Mtb, diantaranya
interleukin-8 (IL-8), IL-12, dan IL-10.
Penelitian ini bertjuan untuk meneliti hubungan antara kadar IL-8 dan IL-12
serum sebagai sitokin proinflamasi dan kadar IL-10 serum sebagai sitokin anti
inflamasi pada penderita TB aktif, TB laten dan individu sehat.
Subyek penelitian adalah penderita TB aktif kasus baru, yang berkunjung
ke Infection Center Rumah Sakit Wahidin Sudirohusodo, Makassar, Sulawesi
Selatan dan orang-orang yang kontak serumah dengan penderita tersebut dan
tidak menunjukkan gejala TB aktif.
Penelitian ini diikuti 132 subyek, terdiri atas 44 subyek TB aktif, 42 subyek
kontak serumah dengan hasil uji Quantiferon-TB Gold-In Tube (QTF-GIT)
positif yang selanjutnya ditetapkan sebagai TB laten, dan 46 subyek dengan
hasil uji QTF-GIT negatif yang selanjutnya ditetapkan sebagai individu sehat.
Didapatkan, kadar IL-8 serum pada TB laten dan individu sehat lebih tinggi
secara bermakna dibanding TB aktif (p=0.001 dan p=0.000), sedang antara TB
laten dengan individu sehat tidak berbeda bermakna (p=0.170). Kadar IL-10
serum lebih tinggi secara bermakna pada TB laten dan TB aktif dibanding
individu sehat (p=0.000 dan p=0.001), sedang antara TB laten dengan individu
sehat tidak berbeda bermakna (p=0.501). Kadar IL-12 serum lebih tinggi pada TB
aktif dibanding TB laten dan individu sehat, namun secara statistik tidak
bermakna. Rasio rerata kadar IL-8/IL-10 serum lebih tinggi secara bermakna
pada individu sehat dibanding TB aktif dan TB laten (p=0.012 dan p=0.015),
sedang antara TB aktif dengan TB laten tidak berbeda bermakna (p=0.461).
Rasio rerata kadar IL-12/IL-10 serum lebih tinggi pada TB aktif dibanding TB
laten dan individu sehat, namun secara statistik tidak bermakna. Dari uji korelasi
ditemukan hubungan bermakna antara kadar IL-8 dengan IL-10 serum pada TB
aktif, yaitu makin tinggi kadar IL-8 serum, makin tinggi pula kadar IL-10 serum
(p=0.013).
Dari penelitian ini dapat disimpulkan bahwa kadar IL-8 serum secara
bermakna lebih tinggi pada TB laten dan individu sehat dibanding TB aktif. Rasio
rerata kadar IL-8/IL-10 serum yang tinggi pada individu sehat dapat dijadikan
pembeda terhadap TB laten. Terdapat korelasi positif antara kadar IL-8 dan IL-10
serum pada TB aktif.

Tuberculosis remains a major health problem globally. A person which infected
by Mtb may directly eliminated the bacilli or progress into active or latent TB. The
role of several cytokines have been identified against Mtb infections, which
includes interleukin-8 (IL-8), IL-12, and IL-10.
This study was designed to examine the relationship between serum level of IL-8
and IL-12 as a proinflammatory cytokines and serum level of IL-10 as an antiinflammatory cytokines in patients with active and latent TB, also healthy.
Subjects were new cases of active TB patients, who visited Infection Center
Sudirohusodo Wahidin Hospital, Makassar, South Sulawesi and household
contacts without symptoms.
This study was followed by 132 subjects consisting of 44 subjects of active TB,
42 subjects household contact with positive test results of Quantiferon-TB
Gold-In Tube (QTF-GIT), defined as latent TB, and 46 subjects with negative
results of QTF-GIT, defined as healthy. We found that serum level of IL-8 in
latent TB and healthy were significantly higher than active TB (p=0.001 and
p=0.000), whereas between latent TB and healthy did not differ significantly
(p=0.170). Serum level of IL-10 were significantly higher in latent TB and active
TB than healthy (p=0.000 and p=0.001), whereas between latent TB and healthy
did not differ significantly (p=0501). Serum level of IL-12 was higher in active TB
and latent TB as compared with healthy, but not statistically significant. The ratio
of mean serum level of IL-8/IL-10 was significantly higher in healthy as compared
with active TB and latent TB (p=0.012 and p=0.015), whereas among active TB
and latent TB was not significant (p=0461). The ratio of mean serum level of IL12/IL-10 was higher in active TB and latent TB as compared with healthy, but not
statistically significant. From correlation test, we found a significant correlation
between serum level of IL-8 and IL-10 in active TB, which is the higher the serum
level of IL-8, the higher the serum level of IL-10 (p=0.013).
From this study we may concluded that the serum level of IL-8 were significantly
higher in latent and active TB as compared with healthy. Higher ratio of mean
serum level of IL-8/IL-10 in healthy can be used as a differentiation for latent TB.
The higher the serum level of IL-8, the higher the serum level of IL-10 serum.
