PETITION TO DENY DUE COURSE
AND TO CANCEL CERTIFICATE OF CANDIDACY
Petitioner, through counsel, respectfully alleges:
1. Petitioner _____________, is a natural-born citizen of the Philippines, __________ years of
age, able to read and write English, Tagalog, Ilocano, a registered voter of __________, and is
and has been a resident __________, continously since ___________up to the present.
2. Petitioner duly filed his certificate of candidacy for the on of _________, on __________, for

4. Respondent's certificate of candidacy contains material representations to the effect that

WHEREFORE, petitioner prays that this Commission:
1. Declare respondent ineligible for the position for which he filed a certificate of candidacy and to
deny due course to such filing and to cancel to such filing and to cancel the certificate of
candidacy.
2. Direct the various boards of election inspectors of the Province of _____________ and the
Provincial Board of Canvassers of such province not to count any votes cast for respondent's
