INTRODUCTION
Scaffolding works requires all erectors to work at height during erection of scaffold. As
the number one killer for all industries is Fall from Heights, it is essential that adequate
measure must be taken to protect personnel working at height. AH SCAFFOLDING
management is committed to minimize the risks associated with fall from heights and will
ensure all measure are carried out while working at site.
1. PURPOSE
This Safe Work Procedure (SWP) is to ensure that erection, dismantling and
modification of scaffold are carried out safely as all this working process are associated
closely with WSH (Work at Heights) 2013 Regulations and WSH(Work at Heights)
(Amendments) Regulations 2014
2. SCOPE
This safe work procedure is applicable for all scaffold supervisors and erectors that are
exposed to the hazard of falling from height while working in site
3. RESPONSIBILITIES
3.1.

Top Management

3.1.1. It is the responsibility of the top management to ensure that sufficient


resources are available to fully support the fall protection plan (FPP) so that all
the objectives can be achieve. Sufficient resources such as adequate P.P.E
and equipment eg: Sala blocks and life lines must be provided by the
management to ensure all men are safe while working at height. The
management shall also ensure and appoint competent personnel to maintain
the fall protection plan.

3.2.
Scaffold Supervisors
3.2.1. To ensure that all PPE are adequate and are in working condition before start
of work.
3.2.2. To make sure all equipment use such as life lines and sala blocks are used by
the scaffold erectors when required too.
3.2.3. To ensure all scaffold erectors hook up their lanyards when working above 2
meters.
3.2.4. To conduct tool-box meeting and risk assessment before starting work.

3.3.
Scaffold Erectors
3.3.1. The scaffold erectors must always hook up both of their lanyards while
working at height.
3.3.2. The scaffold erectors shall ensure all practicable measures are taken to
ensure that the scaffold work is being carried out in a safe manner.
3.3.3. The scaffold erectors are to ensure the safety harness with double lanyards
and other appropriate Personnel Protective Equipment (PPE) are worn at all
times when performing scaffold work.
3.4.
W.A.H SUPERVISORS
3.4.1. It shall be the duty of the WAH Supervisor to ensure that the scaffold erectors
who carries out or is to carry out any work at height is there to supervise with
the job. WAH hazards must be brief to them and they are to ensure the
scaffold erectors are equipped with the necessary fall arrest equipment. They
too shall ensure all control measure are adhered and implemented effectively
on site before submitting the application to the assessor for evaluation.
W.A.H ASSESSORS
3.5.1. It shall be the duty of the WAH Assessor to properly access the job site before
endorsing the PTW. They should be able to evaluate the effectiveness of fall
prevention measures, advises, identify and foresee potential hazards. Conduct
routine inspection and identify any unsafe work conditions, safety lapses and
advise authorized manager to revoke any PTW when not in compliance.

4. HAZARD IDENTIFICATION AND RISK ASSESSMENT
4.1.

Hazard Identification
Hazards that are involve with the work process that may cause harm to the
person working at height without any proper fall protection must be made known
to all working personals. Examples of potential of fall from heights are given
below:

Slippery working platform


Scaffold platform with no guard rails
Unsecure working platform.
Struck by moving objects or equipment
Loss of balance due to uneven work surface

4.2.

Risk Assessment
A team of competent and knowledgeable personals from the organization should
conduct risk assessment. The team members should include all stakeholders as
this will ensure that a wider point of view with regards to the topic. They should
access potential area that are a likelihood of a fall occurring and extent of any
potential harm or injury and consequently, determine a level of risk. Hazards that
have higher risk should be given priority and suitable control measure should be
taken to bring down the risk level. Responsible person shall then be appointed to
carry out the given control measures. Therefore risk assessment should always
be conducted before the start of every work.

4.3.

Risk Control Measure/Methods


Appropriate control measure should be in place to minimize the risk level. The
control measure should also be applicable to the situation and shall be carefully
assessed and implemented to ensure its effectiveness. The new control measure
shall not introduce new hazards as it should eliminate the identified hazards. It
shall also be regularly monitor and reviewed to ensure that they are effective.

5. SAFE WORK PROCEDURES
Safe Work Procedure (SWP) is commonly used as a form of administrative controls in a
workplace. It is a set of instructions on how work can be carried out safely. A set of SWP
shall be form up in accordance to the risk assessment conducted by the R.A team. It is
therefore very important for the management and relevant stakeholder to communicate
to everyone involve in the job regarding the SWP so that they are aware of their roles
and responsibilities.
6. PERSONAL FALL PROECTION EQUIPMENT
Personal protective equipment (PPE) should be always use by all scaffold erectors and
supervisors while at work. It is also one of the control measures that is applicable to
every working situation, so everyone is to put on their PPE before the start of work and
ensure that it is in a good working condition. The right PPE and also equipment should
be chosen for the proper type of work to be carried out.
6.1.

Scaffold erectors and supervisor


All erectors and supervisors shall be equip with full body harness and lanyards
The full body harness should be equipped with 2 lanyards.
All PPE use should be in a good working condition

6.2.
Working at height without any anchoring points
Cable life lines should be set up and be used as anchoring points.
Sala blocks should be attached to life lines to break falls.

7. INSPECTION AND MAINTENANCE


7.1.

Inspection
Inspection should be carried out monthly. At the end of every month the
supervisors should inspect all the erectors PPE to make sure all of them are free
from defects. Any defects found on their PPE, it shall be replaced immediately
and shall not be allowed to be use at work again.

Maintenance should be done for all the PPE and equipment used for work daily.
A monthly maintenance will ensure that all the equipment used are in working
condition. Sala blocks and cable life line must be send to their individual
PAGE 6 OF 6 specialize contractors for maintenance. Only a trained and competent person
should be carrying out all the maintenance work and it will be his responsibility to
ensure that the equipment is in working condition.
8. TRAINING
All personals working should have had undergo proper training and they should be
provided with sufficient knowledge and information of the risk of falling from height which
is involved with their work so they can perform their work safely. Re-fresher training
program should also be conducted to ensure that they will always be up to date with the
recent implementation done. The training should cover the topic mention below: Nature of fall hazards
Inspecting fall protection system
Use of fall protection system
Roles and responsibilities of stakeholders on fall protection plan

9. INCIDENT INVESTIGATION
All incidents related to fall from height should be investigated and determine the cause of
it. A new implementation shall be done to eliminate the cause of the fall and also to
prevent future incidents from happening again. The risk assessment and the fall
protection plan should be reviewed again and be updated with the current
implementation.
10. EMERGENCY PREPAREDNESS
The organization should a come out with an emergency response plan for all emergency
plan for all emergency situation relating to work at height. To further prepare the
organization, drills and exercises should be conducted to always prepare the
organization personal make sure they are aware of their roles and responsibilities.
During an emergency for example an event where a fall from height had just happen, the
scaffold supervisor should immediately inform the main contractor safety department. In
an event whereby the scaffold supervisor is not present, the scaffold erectors should
quickly inform the main contractor safety department. They are to assist the safety ERT
members with adequate information with regards to the incident. Rescuing should be
done only by the trained ERT team members.
