RE: CONTRACT OF LEGAL SERVICES
(For: Quasi-Delict/Damages Case Pertinent to Mrs. Ma. Emma Sontillanosa Against a Group of
Doctors and WVSUMC)
Dear MR. SONTILLANOSA:
We agree to be your counsel in the civil case you are going to file:
The scope of our legal services shall be as follows:
1. We will prepare and notarize whatever pleadings, agreement, if any, affidavits and other
related documents as well as Motions;
2. We will appear in the pre-trial, trial and all conferences, meetings, before any courts; and
3. We will follow-up the issuance of orders, decision, certifications and other documents related
to the case.
COSTS AND CONSIDERATION OF THE LEGAL SERVICES
A. ATTORNEYS FEES: Our total Attorneys Fees for handling the case is ONE HUNDRED
THOUSAND (P100,000.00) Pesos, Philippine currency or TWO THOUSAND FIVE HUNDRED US
DOLLARS(US $ 2,500.00). However, by way of Acceptance Fee, we will require you a down payment
in the amount of P40,000.00 PESOS OR ONE THOUSAND DOLLARS (US$ 1,000.00) The fee shall
be payable upon execution of this contract of legal services. The balance of P30,000.00 PESOS OR
SEVEN HUNDRED FIFTY DOLLARS (US$ 750.000) will be payable before the first scheduled
hearing of the case and the remaining P30,000.00 PESOS OR SEVEN HUNDRED FIFTY DOLLARS
(US$750.000) will be payable once the case has been submitted for decision.
Our appearance fee will be THREE THOUSAND (P3,000.00) Pesos, Philippine Currency OR
SEVENTY FIVE DOLLARS (US$ 75.00) , per scheduled hearing of the case within the City of Iloilo and
FIVE THOUSAND (P5,000.00), Pesos, Philippine Currency OR ONE HUNDRED TWENTY FIVE
DOLLARS (US$125.00), per scheduled hearing of the case outside Iloilo City, exclusive of transportation
and accommodation expenses.

B.DOCKET FEES: THE TOTAL DOCKET FEES FOR THIS CASE IS THREE
THOUSAND DOLLARS (US$ 3,000.00). Exclusive of our Attorneys Fees, upon the signing of this
contract, ALL expenses as well as payments for FILING and DOCKET FEES before the court,
transportation, mailing, photocopy, service of summons, execution, sheriff fees and the like, shall be
charged to you. The same will be liquidated to you with receipts.
This contract may be rescinded by either of us upon proper notice and we shall be entitled to our
attorneys fees based on quantum meruit. If appeal will be resorted to, it will be subject to another
contract.

The lead counsel for this case is Atty. Salex E. Alibogha, while the collaborating cousel is Atty.
Noel Siosan, Jr.
