Provide supervision for the northeastern regional office’s 7 nurse care managers and 4 medical assistants. Work
directly with the personal visiting physicians to coordinate care and services for the members enrolled in
contracted health plans for high risk, medically fragile patients. The program interfaces with the contracted health
plans’ objectives to transition their members to ‘the medical home’ concept. I am responsible for coordination of
care with the health plan, pcp, and home visiting physician, by ensuring the physician’s plan of care is
implemented, and following up with the members to determine the effectiveness of the plan. Additional
responsibilities include coaching members to achieve self management by providing telephonic assessments,
follow up calls, education, as well as 24/7 clinical support.

DELAWARE COUNTY MEMORIAL HOSPITAL, Drexel Hill, PA 2004-

Providing end-of-life care in the home or nursing home settings. Design a plan of care for the hospice patient.
Oversees the interdisciplinary team services provided to the patient. Teaches families and health care professionals
the importance of comfort to the dying patient. On call from 4:00pm- 8:00am during the week and weekend
rotation of 4:00pm Friday to 8:00am Monday. Attend weekly “team” meetings. Evaluate patients in the home,
hospital or nursing home for the suitability of hospice. Program explanations to families and physicians for
hospice.

Provide end-of-life nursing care to 4-6 patients daily. At the direction of physicians, perform patient assessments,
order and administer medications, and lend emotional support to patients and their families.

Pulmonary Function Therapist
Conducted breathing studies to diagnose lung disease. Drew arterial blood gases and performed methacholine
challenge studies. Reported test results to physicians. Represented the hospital at off-site Respiratory Health Fairs.
