1.0

Introduction

The Radioactive Liquid Waste Treatment Facility (RLWTF) located at the Los Alamos National
Laboratory (LANL) has been in operation since 1963. It is currently well beyond its original design life
of 25 years. The RLWTF Upgrade Project (RLWTF-UP) will provide replacement facilities designed to
receive, treat, and discharge liquid waste feed from various LANL technical areas (TAs). The
RLWTF-UP Low-Level Waste (LLW) Subproject will be constructed for the treatment of LLW.
Currently, RLWTF has limited ability to meet regulatory requirements and respond to system failures as
they occur. The RLWTF is vulnerable to major system failures that could shut down its radioactive
wastewater treatment capability. Such a shutdown in operations represents a single point of failure for all
nuclear, environmental, and research and development operations that generate radioactive liquid waste at
LANL, and place mission-essential LANL capabilities at risk.
The LLW Subproject will be comprised of a building for the treatment of LLW, together with ancillary
facilities that include a LLW Utility Building, LLW Drum Storage Area, and LLW Effluent Storage Tank
Area. Specifically, this includes:

LLW storage and treatment;


Secondary waste storage, treatment, and packaging;
Treated effluent storage and discharge;
Chemical receipt and storage;
Solid waste storage and handling; and
Electrical, control, and data transmissions and receipts for equipment associated with the LLW
treatment processes, treated effluent storage, and treated effluent discharge.

The 2009 International Building Code (IBC 2009) has set forth a number of stipulations in which the
employment of Special Inspectors is mandatory for continuous or periodic inspections during
construction of new facilities (IBC, Chapter 17, Section 1704). These activities are in addition to the
routine inspections specified in the IBC (Chapter 1, Part 2, Section 110) and other construction standards.
The IBC applies to all Management Levels (i.e., quality levels of building work covered by its scope).
This stand-alone Test and Inspection Plan has been prepared as discussed below to address the IBC
special inspection requirements for the LLW Subproject.

1.1

Purpose

The purpose of this Plan is to:

Identify the special inspection, test, and structural observation requirements as established in
the IBC (Chapter 17), that are determined by the responsible Structural Design Engineer to be
applicable to construction of the LLW Subproject.

2.

Establish LLW Subproject plans and expectations for the performance and documentation of
special inspection, test, and structural observation requirements, during the projects execution
phase. This includes establishing the minimum qualification requirements for Special Inspectors.

3.

Define the specific attributes to be verified along with the appropriate acceptance criteria.

This Plan does not provide a detailed explanation of required inspection or test procedures, nor is it
intended to address the Construction inspection/test activities required for the LLW Subproject by the
approved construction documents (i.e., drawings and specifications).

Scope of Special Inspections, Test, and


Structural Observations

LANLs Engineering Standards Manual (ESM) (STD-342-100, Chapter 16, IBC Program) invokes
mandatory special inspection requirements for LANL construction projects. This is defined as the
process of inspection, testing, and reporting by approved Special Inspectors to assure the LANL Building
Official (LBO) that the construction of critical building elements, materials, and life safety systems is
being performed in accordance with the approved construction documents and IBC Chapter 17. LANLs
ESM (Chapter 5, Structural) further establishes that the special inspection, test, and structural
observation requirements given in Chapter 16 for new construction or modifications to existing structures,
systems, and/or components (SSCs) are intended to ensure that construction is implemented in the manner
intended by the responsible Structural Design Engineer. On this basis, the special inspection
requirements defined herein for the LLW Subproject are developed from the minimum requirements
contained in IBC Chapter 17 and as augmented by the LANL ESM IBC Program (Chapter 16). The IBC
special inspection requirements have been specifically tailored to the LLW Subproject using a graded
approach, with the level of rigor appropriate for construction of a Performance Category 1 (PC-1). The
IBC special inspection, test, and structural observation requirements established in this Plan are
independent of, and in addition to, the inspection requirements specified in other approved construction
documents for the project.
Special inspection as used in this Plan includes those activities required of materials, installation,
fabrication, erection or placement of components, and connections requiring special expertise to ensure
compliance with approved construction documents and referenced standards, including proper quality,
workmanship, and materials. Test activities use critical examinations, observations, or evaluations
conducted in accordance with a standardized process/procedure to produce a measurable basis for
determining the acceptability of an item. Structural observations include visual observation of a
structural system by a registered design professional for general conformance to the approved
construction documents.
The IBC establishes the requirement for the building owner, or the registered design professional acting
as the owners agent, to employ one or more Special Inspector(s) to provide inspections during
construction on the types of work listed under Chapter 17, Section 1704 of the IBC. The Special
Inspector shall be a qualified person who has specialized knowledge, training, experience, and
certification(s) for one or more types of the construction subject to special inspection. Special Inspectors
shall be employed by an established and recognized agency (i.e., Special Inspection Agency) regularly
engaged in conducting tests or furnishing inspection services, who shall demonstrate competence to the
satisfaction of the LBO, for performing the specified inspections, tests, or structural observations. The
Special Inspection Agency may be LANL or one of its approved subcontractors.
This Plan will be submitted with the Final Design Package to the LBO for review and approval. The
LBO, or his designees, will review and approve the design media and this Plan and return it to the project
for implementation, or return it with comments that need to be resolved. If there are comments to be
resolved, the project will not proceed with construction or structural fabrication activities without the
LBO concurrence. Once this Plan is fully approved by the LBO, or his designee, and returned to the
project, the project team can begin the appropriate phases of the work.
The Project Team will execute the Special Inspections Plan by performing and documenting the required
inspections. The LBO shall be notified of any inspection or inspection step that cannot be completed and
must approve, in writing, any deviation from the approved Special Inspections Plan.
On completion of the work, the Project Manager will notify the LBO, who will then perform a final
inspection review to his/her satisfaction before granting an occupancy authorization for the new facility.

Structures, Systems, and/or Components to


be Inspected

Construction of critical building elements, materials, and life-safety systems will be performed in
accordance with the approved construction documents. These construction activities will be subject to
special inspections, tests, and structural observations under the IBC purview as detailed in this Plan.
The following IBC sections (Chapter 17) address the requirements for special inspections, tests, and
structural observations applicable to this project:

Steel Construction (Section 1704.3),
Concrete Construction (Section 1704.4),
Masonry Construction (Section 1704.5),
Soils (Section 1704.7),
Special Cases (Section 1704.15),
Special Inspections for Seismic Resistance (Section 1707),
Structural Testing for Seismic Resistance (Section 1708), and
Structural Observations (Section 1710).

Other IBC special inspection, test, and structural observation requirements (Chapter 17, Section 1704
through 1715) have been considered and determined to be not applicable for the LLW Subproject (note,
this determination may be revised based on review of the completed final design for the project). A
detailed discussion of the invoked special inspection, test, and structural observation requirements is
provided in Section 5.0 of this Plan, and an inspection checklist is provided in Appendix A.

2.0

General Building Information

The LLW Subproject will be located west of the existing building (TA-50-1) and southeast of building
TA-50-37. The project consists of a new LLW Treatment Building that is designed as an industrial
processing facility with three support out-buildings: (1) the LLW Utility Building, (2) the LLW Drum
Storage Area, and (3) the LLW Effluent Storage Tank Area. Structural design basis and descriptive
information for these facilities are discussed in detail in the Design Basis and Approach for the TA-50
Radioactive Liquid Waste Treatment Facility Upgrade Project (RLWTF-UP) Low-Level Waste
Subproject (60239831-TRPT-001), and summarized in the following subsections.

2.1

Low-Level Waste Treatment Building

The LLW Treatment Building is a single-story structure with a high-bay portion that provides the
capability to process LLW waste streams generated at the LANL Site. This building has been identified
as a Management Level (ML)-4 building and has been designed to PC-1 criteria in accordance with DOE
Standard Natural Phenomena Hazards Design and Evaluation Criteria for DOE Facilities (DOE-STD1020-2002), for earthquake, high wind, and snow loads.
The LLW Treatment Building is approximately 72 ft-8 in. wide by 93 ft-6 in. long, and is all concrete
construction. Continuous spread footings support perimeter retaining wall system of varying heights
(greatest along the east side of the building). The building is divided into the following containment
areas:

High Bay with LLW Primary and Secondary Waste Treatment processing equipment,
Single-story with about half designated as a Wet Chemistry Laboratory, and
Single-story with about half designated as support areas (change rooms, electrical room,
telecommunications room, operations center).

These containment areas provide segregation of the areas which handle waste samples and process liquid
LLW received from the Influent Storage Facility.
The vertical loads at the roof are supported by pre-cast concrete, double-tee beams, spanning from
exterior walls to an interior bearing wall separating the two building areas. Exterior and interior walls are
cast-in-place concrete, and function as bearing walls, shear walls to resist lateral forces, and retaining
walls. Retaining conditions occur where the finished floor elevations is above the adjacent grading
elevations. All walls are supported by the foundation system.

2.2

Low-Level Waste Utility Building

The LLW Utility Building is a single-story, reinforced, concrete masonry unit structure located adjacent
to, north of, the LLW Treatment Building and which houses various support systems and equipment for
the LLW treatment processes, including an air compressor, natural gas-fired boilers, and the chilled water
pump skids. The LLW Utility Building is approximately 32 ft wide by 36 ft long. Process and facility
chillers are located adjacent to and north of the LLW Utility Building.
The vertical loads at the roof are supported by a 1-1/2-in. by 18-gauge metal roof deck system supported
on metal roof joists spanning 30 ft to the exterior 8-in. solid-grouted Concrete Masonry Unit Bearing
Wall System using special reinforced masonry shear walls. The vertical loads for the floor are supported
on a 6-in. concrete slab-on-grade. The 8-in. solid-grouted, reinforced, Concrete Masonry Unit shear walls
distribute the vertical and lateral loads to the foundation system. The Lateral-Force Resisting Systems
consist of a Bearing Wall System using special reinforced Masonry Shear Walls with shear walls and
flexible metal roof deck diaphragm that transmit horizontal forces from the superstructure to the
foundation system below grade. The foundations will be reinforced-concrete strip footings on 3 ft of
structural fill.

2.3

Low-Level Waste Drum Storage Area

The drum storage pad consists of an 8-in.-thick concrete slab-on-grade which is reinforced with #5 bars at
18 in. on-center, each way. The slab contains a 36-in.-deep by 12-in.-wide turn-down around the
perimeter, which is reinforced with approximately three #6 bars continuous, top and bottom. #5 diagonal
bars are included at 18 in. on-center to tie the slab into the turn-down. The concrete slab rests on a
compacted 2-in. bed of sand placed on top of a vapor barrier, which in turn lies on a 6-in. compacted
granular base. Beneath that is 3 ft of compacted structural fill. The structural fill is extended 3 ft beyond
the slab perimeter.

2.4

Low-Level Waste Effluent Storage Tank Area

The LLW Effluent Storage Tank Area is comprised of a secondary containment structure with an adjacent
LLW Effluent Sample Building. The mat foundation for the secondary containment consists of an
18-in.-thick slab-on-grade with #7 bars at 9 in. on-center each way, top and bottom, typical. A
16-in.-thick reinforced concrete wall retaining a maximum of 12 ft of soil surrounds the mat foundation to
provide a sump for secondary containment of effluent. The containment wall is reinforced with #7 bars at
9 in. on-center vertical, #5 bars at 12 in. on-center horizontal, and extends from the top of the mat
foundation to a minimum of 6 in. above the surrounding grade.
At the LLW Effluent Sample Building, the vertical loads at the roof are supported by a 1-1/2-in. by
18-gauge metal roof deck system supported on metal roof joists spanning 12 ft to the exterior 8-in. solidgrouted Concrete Masonry Unit Bearing Wall System using special reinforced masonry shear walls. The
vertical loads for the floor are supported on a 6-in. concrete slab-on-grade. The 8-in. solid-grouted,
reinforced, Concrete Masonry Unit shear walls distribute the vertical and lateral loads to the foundation
system. The Lateral-Force Resisting Systems consist of a Bearing Wall System using special reinforced

Masonry Shear Walls with shear walls and flexible metal roof deck diaphragm that transmit horizontal
forces from the superstructure to the foundation system below grade. The foundations will be reinforcedconcrete strip footings on 3 ft of structural fill

3.0

Special Inspector Qualifications

In accordance with Chapter 17, Section 1704 of the IBC, Special Inspectors shall be employed to provide
inspections during construction on the types of work listed under Section 1704 and implemented by this
Plan for the LLW Subproject. The IBC inspection process is specifically for IBC (2009) and 2009
International Existing Building Code (IEBC 2009) work.
Special Inspectors are those qualified by the LBO, either directly, or through the LBOs approval of an
Inspection Agency. The Special Inspector shall be a qualified person who shall demonstrate competence
for inspection of the particular type of construction or operation requiring special inspection.
Welding inspection subject to the IBC (Chapter 17) shall only be performed by a Certified Welding
Inspector that has been approved by the LANL Welding Program Administrator/Chief Inspector for IBC
Welding on behalf of the LBO. LANL personnel pre-approved for such inspection are listed on the
LANL Welding Program website in the Approved Inspector listing. Non-LANL Certified-Welding
Inspectors shall be submitted for approval to the LANL Welding Program Administrator via the Project
Manager.
Qualification guidance for Inspectors is contained in the LANL ESM (STD-342-100, Chapter 16,
Section IBC-TIA -- IBC Test and Inspection Agency Approval Process, Attachment B LANL IBC
Inspector Qualification Guidelines).

4.0

Roles and Responsibilities

LANL organizations and their subcontractors are required to comply with the LANL IBC Building Safety
Program. This section defines the roles and responsibilities for the participants in Special Inspection
Program activities for the LLW Subproject. The key participants include:

The Design Professional in Responsible Charge (DPIRC),
The LANL Project Manager,
The Construction Subcontractor and Fabricator, and
The Special Inspector(s).

4.1

LANL Project Owner

The LANL Project Owner (normally Project Manager or Facility Manager) is responsible for employing
one or more Special Inspector(s) to provide inspections during construction on the types of work listed
under Chapter 17, Section 1704 of the IBC, and for funding IBC Special Inspection Services. This
responsibility may be delegated in writing by the Project Owner to a registered design professional acting
as the Owners Agent.
Through the Subcontractor Technical Representative, the LANL Project Owner shall review a list of
planned submittals to ensure that additional tests and inspections required in the specifications, but not
contained herein, are provided by the Subcontractor.

4.2

LANL Building Official or Designee

The LBO is the Division Leader of Engineering Services. The LBO function is integral to enforcement of
the IBC, the IEBC, and other building-related LANL ESM requirements. The LBO does this enforcement
through design reviews and field inspections; those performing such work must be delegated by the
LBOs authority to act on the LBOs behalf. The LBO may delegate some of his/her duties to Deputy
Building Officials, Chief Inspectors, and others, as necessary.
The specific duties and responsibilities of the LBO relative to this Plan include the following:

Review construction documents for erection, alterations, demolitions, and moving of buildings
and structures for compliance with IBC requirements.

Approve the IBC Special Inspection Plan, which may require a preconstruction conference to
review the Plan with all applicable members of the Construction Team.

Approve Special Inspector/Inspection Agencies. The LBO is responsible for determining


competence of inspectors for the types of work they will be inspecting.

Approve field and laboratory test agencies and personnel used under the scope of the IBC, as well
as approval of offsite steel fabricators.

Monitor special inspection activities. The LBO shall inspect (as necessary) the premises and
enforce compliance with LANL requirements. The LBO (or authorized designee) shall monitor
the inspection activities at the jobsite to ensure that qualified Inspectors are performing their
duties when work requiring inspection is in progress.

Issue stop work orders, when required. As with any LANL employee, whenever the LBO finds
any work being performed in a manner dangerous or unsafe, the LBO is authorized to issue a stop
work order.

Review inspection reports. The LBO or authorized designee receives and reviews inspection
progress reports and final reports for conformance with the approved plans, specifications, and
workmanship provisions of the IBC.

Perform a final inspection on completion of the work. The LBO will perform any IBC-related
reviews deemed necessary before providing an approval for occupancy or beneficial use.

4.3

Design Professional in Responsible Charge

The engineer or architect of record is referred to as the DPIRC. The DPIRC has many duties and
responsibilities related to special inspections, tests, and structural observations, including the following:

Prepare Special Inspection and Test Plan. The DPIRC shall list the items for which inspection is
required, hold and witness points (shop or field), special inspections (per IBC and IEBC),
whether inspections are continuous or periodic, and the details of such inspection. The DPIRC
responsibilities include structural observations and any required seismic testing.

Submit approved Plan in native electronic file format to allow the Constructor to integrate with
scheduling data.

Submit Inspection Agencies to the LBO for approval, when the DPIRC is contracted by LANL to
perform special inspection duties.

Respond to field discrepancies. The DPIRC shall respond to nonconformance reports that are
dispositioned as Use-As-Is or Repair (not required on Rework or Reject).

Review shop and fabrication drawings, and submit revisions as approved design documents. The
DPIRC shall acknowledge and approve shop and fabrication drawings that may detail structural
information. The DPIRC shall submit the drawings to the LBO-designated design reviewers for
written approval of any deviations to the approved plans, drawings, or specifications, and shall
resubmit revised plans, drawings, and specifications that may be required because of the shop
drawings, etc. On approval, send copy to the LANL Construction Inspection Group.

LANL Project Manager

The LANL Project Managers responsibilities, with respect to this Plan, include the following:

Ensure that any designated work under the IBC purview does not proceed until they have
obtained LBO approval to proceed.

Ensure compliance to the IBC on the project down to the various subtier subcontractors and
suppliers. This includes instructing personnel and passing-down necessary controls to subtier
levels and ensuring that the LBO will be properly notified of nonconforming conditions on any
IBC-related work.

Ensure that applicable Fabricators, Test Agencies, and Inspection Agencies are appropriately
approved by the LBO.

Ensure that the LBO, or designee, will be properly notified of nonconforming conditions on any
IBC-related work.

Notify the LBO, or designee, as soon as possible when substandard construction has occurred on
the project (including work by testing agency or in fabrication and manufacturer shops).

Notify the LBO, on completion of the work, to request a final inspection review to support the
granting of an occupancy authorization.

4.5

Construction Subcontractor and Fabricator

The Constructors and Fabricators duties and responsibilities are detailed in the LLW Subproject
drawings, specifications, and other approved-construction documents. For the IBC-related work scope,
this includes, but is not limited to:

Complying with all applicable requirements of this Plan. Complete and submit to the LANL
Project Representative a Subcontractors Statement of Responsibility with Respect to Special
Test and Inspections form as found in Appendix B of this Plan.

Ensuring that necessary requirements related to the project special inspections, tests, and
structural observations, including hold and witness points, design documents, and IBC
requirements, are passed-down to their subtier subcontractors and suppliers.

Notifying the LBO and LANL Project Representative of any nonconformances or deficiencies
and any cases where the subcontractor does not honor inspection hold points, test hold points, or
witness points identified on the approved-Inspection Plan.

Special Inspector(s)

Approved IBC Inspectors (and Inspection Agencies) are those qualified by the LBO, either directly, or
through the LBOs approval of an Inspection Agency. The IBC Inspector or Inspection Agency shall not
be employed by the constructing subcontractor or material supplier. The work processes, authority,
duties, and reporting functions of the IBC Special Inspectors (and their agencies) shall comply with
LANLs ESM (STD-342-100, Chapter 16).

Specific roles and responsibilities for the Special Inspector include:

Review approved plans and specifications for inspection requirements. IBC Inspectors will
comply with the inspection requirements of LANL.

Signify presence at the jobsite. Special Inspectors shall notify the Construction Manager and the
Constructor of their presence and responsibilities at the jobsite location. If required by the LBO,
they shall sign in on the appropriate form posted by the construction subcontractor.

Observe assigned work. Special Inspectors shall inspect all work in the inspection program for
conformance with the LBO-approved drawings and specifications, applicable provisions of the
IBC (Chapter 17, Section 1704) on special inspection, and applicable workmanship provisions of
the IBC; approved shop drawings may be used only as an aid to inspection. Inspections include
special inspections in the steel fabricators shop when required and other inspections identified in
a Special Inspections, Tests, and Structural Observations Plan.

For continuous inspection, the Special Inspector shall be onsite at all times observing the work
requiring inspection designated as continuous. Periodic inspection is intended to mean that the
Inspector, at periodic times, inspects all work performed, but is not required to witness the
work being performed.

Notify the LANL Chief Inspector, immediately, in cases where project conditions or personnel do
not provide adequate notice for hold or witness points, inhibit the Inspectors ability to inspect, or
any conditions that present themselves that would prevent adequate inspection.

Filing of accurate and complete inspection reports to the LBO and other recipients.

Report nonconforming items, as well as bringing all nonconforming items to the immediate
attention of the subcontractor.

Provide timely reports, including written inspection reports for each inspection visit and a final
report addressing all items requiring IBC special inspection and testing.

The IBC Inspector shall notify the LBO (regardless of whether the Inspector reports directly to
the LBO, the Inspection Agencies, or another entity onsite) immediately when conditions on a
project, job, or other work under IBC causes the Inspector to be unable to perform their duties.
This can be due to lack of notification of hold or witness points, lack of funding, reduced funding,
or any conditions that impacts the Inspectors independence.

5.0

Special Inspections, Tests, and Structural


Observation Requirements

5.1

IBC Special Inspections

The following subsections provide a listing of the specific special inspection requirements from the IBC
(Chapter 17, Section 1704), that are determined by the responsible Structural Design Engineer to be
applicable to construction of the LLW Subproject. These special inspection requirements are in addition
to the construction inspections specified elsewhere, by the LLW Subproject construction documents, and
shall be carried out by Special Inspectors approved by the LBO.

5.1.1 Inspection of Fabricators


Where fabrication of structural load-bearing members and assemblies is being performed on the premises
of a fabricators shop, special inspection of the fabricated items shall be required by Chapter 17,
Section 1704.2 of the IBC, and as required elsewhere in the code, unless the Fabricator has been approved

by the LBO. Special inspection or shop approval is not required for burning and cutting of mild steel
[e.g., steel to Standard Specification for Carbon Structural Steel (ASTM A 36/A 36M)] [LANL ESM,
STD-342-100, Chapter 16, Section IBC-IP -- IBC Inspection Process, Appendix A Special Inspection
and Testing Summary (Sample)].
5.1.1.1 Fabrication and Implementation Procedures. The Special Inspector shall verify that the fabricator
maintains detailed fabrication and quality control procedures that provide a basis for inspection control of
the workmanship and the fabricators ability to conform to approved-construction documents and
referenced standards. The Special Inspector shall review the procedures for completeness and adequacy
relative to the code requirements for the fabricators scope of work. Exception: Special Inspections
Section shall not be required where the Fabricator is approved in accordance with Chapter 17,
Section 1704.2.2 of the IBC.
5.1.1.2 Fabricator Approval. Special Inspections required by this code are not required where the work is
done on the premises of a fabricator registered and approved to perform such work without special
inspection. Approval shall be based on review of the Fabricators written procedural and quality control
manuals and periodic auditing of fabrication practices by an approved Special Inspection Agency. At
completion of fabrication, the approved Fabricator shall submit a certificate of compliance to the LBO
stating that the work was performed in accordance with the approved-construction documents. (IBC,
Chapter 17, Section 1704.2.2). To clarify, a Fabricator approved in accordance with this section
eliminates the need for inspection of Fabricators required by the IBC (Chapter 17, Section 1704.2), and
use of an approved Fabricator also eliminates the need for special inspections for seismic resistance
required by Chapter 17, Section 1707. Because the Fabricator must be approved to perform the seismicresistance work, the LBOs approval should consider this (LANL ESM, STD-342-100, Chapter 16,
Section IBC-IP, Appendix A).

5.1.2 Steel Construction


The special inspections for steel elements of buildings and structures shall be as required by the IBC
(Chapter 17, Section 1704.3 and associated Table 1704.3). Where required, Special Inspection of steel
shall also comply with the IBC (Chapter 17, Section 1715).
5.1.2.1 Welding. Welding inspection shall be in compliance with the Structural Welding CodeSteel
(AWS D1.1/D1.1M) and the IBC (Chapter 17, Section 1704.3.1).
5.1.2.2 Details. The Special Inspector shall perform an inspection of the steel frame to verify compliance
with the details shown on the approved construction documents, such as bracing, stiffening, member
locations, and proper application of joint details at each connection (IBC, Chapter 17, Section 1704.3.2).
5.1.2.3 High-Strength Bolts. Installation of high-strength bolts shall be periodically inspected in
accordance with the American Institute of Steel Constructions (AISC) Specifications for Steel Buildings
(AISC 360-05) (IBC, Chapter 17, Section 1704.3.3).
5.1.2.3.1 General. While the work is in progress, the Special Inspector shall determine that the
requirements for bolts, washers, and paint; bolted parts; and installation and tightening, in such standards,
are met. For bolts requiring pre-tensioning, the Special Inspector shall observe the pre-installation testing
and calibration procedures when such procedures are required by the installation method or by project
plans or specifications; determine that all plies of connected materials have been drawn together and
properly snugged; and monitor the installation of bolts to verify that the selected procedure for installation
is properly used for tightening. For joints required to be tightened only to the snug-tight condition, the
Special Inspector need only verify that the connected materials have been drawn together and properly
snugged (IBC, Chapter 17, Section 1704.3.3.1).

5.1.2.3.2 Periodic Monitoring. Monitoring of bolt installation for pre-tensioning is permitted to


be performed on a periodic basis when using the turn-of-nut method with match-marking techniques, the
direct tension-indicating method, or the alternate-design fastener (twist-off bolt) method. Joints designed
as snug-tight need to be inspected only on a periodic basis (IBC, Section1704.3.3.2).
5.1.2.3.3 Continuous Monitoring. Monitoring of bolt installation for pre-tensioning, using the
calibrated wrench method or turn-of-nut method without match-marking, shall be performed on a
continuous basis (IBC, Chapter 17, Section 1704.3.3.3).

5.1.3 Concrete Construction


The special inspections and verifications for concrete construction shall be as required by the IBC
(Chapter 17, Section 1704.4 and associated Table 1704.4).

5.1.4 Masonry Construction


Masonry construction shall be inspected and evaluated in accordance with the requirements of the IBC
(Chapter 17, Section 1704.5). IBC Chapter 17, Section 1704.5.2 and Table 1704.5.1 apply to this project.

5.1.5 Soils
Special inspections for existing site soil conditions, fill placement, and load-bearing requirements shall
follow Chapter 17, Section 1704.7 and Table 1704.7 of the IBC. The approved soils report shall be used
to determine compliance.

5.1.6 Special Cases


Special inspections shall be required for proposed work that is, in the opinion of the LBO, unusual in its
nature, such as, but not limited to, the following examples (IBC, Chapter 17, Section 1704.15):
1.

Construction materials and systems that are alternatives to materials and systems prescribed by
the IBC;

2.

Unusual design applications of materials described in the IBC;

3.

Materials and systems required to be installed in accordance with additional manufacturers


instructions that prescribed requirements not contained in the IBC or in standards referenced by
the IBC; and

4.

Threaded rebar couplers, which shall comply with International Association of Plumbing and
Mechanical Officials (IAPMO) Evaluation Service/Evaluation Report on LENTON
Mechanical Splice System For Steel Reinforcing Bars In Concrete (ER-0129).

5.2

Special Inspection for Seismic Resistance

The following subsections identify special inspections for seismic resistance invoked by the IBC
(Chapter 17, Section 1707), based on the Seismic Design Category D classification established for the
LLW Treatment Building Structure. The special inspections itemized below are required for the
following:
1.

The Seismic-Force-Resisting Systems in structures assigned to Seismic Design Category D, as


determined in IBC Chapter 16, Section 1613.

2.

Designated seismic systems in structures assigned to Seismic Design Category D.

3.

Architectural, mechanical, and electrical components in structures assigned to Seismic Design


Category D that are required in IBC Chapter 17, Sections 1707.7 and 1707.8.

5.2.1 Architectural Components


Periodic special inspection during the erection and fastening of exterior cladding, interior and exterior
nonbearing walls, and interior and exterior veneer in structures assigned to Seismic Design Category D
(IBC, Chapter 17, Section 1707.6), except for:
1.

Special inspection is not required for exterior cladding, interior and exterior nonbearing walls,
and interior and exterior veneer 30 ft or less in height above grade or walking surface.

2.

Special inspection not required for exterior cladding and interior and exterior veneer weighing
5 psf or less.

3.

Special inspection is not required for interior nonbearing walls weighing 15 psf or less.

5.2.2 Mechanical and Electrical Components


Special inspection for mechanical and electrical equipment shall be as follows (IBC, Chapter 17,
Section 1707.7):
1.

Periodic special inspection is required during the installation of anchorage of other electrical
equipment in structures assigned to Seismic Design Category D and

2.

Periodic special inspection is required during installation of piping systems intended to carry
flammable, combustible, or highly toxic contents and their associated mechanical units in
structures assigned to Seismic Design Category D.

5.2.3 Designated Seismic System Verification


The Special Inspector shall examine designated seismic systems requiring seismic qualification in
accordance with Chapter 17, Section 1708.4 of the IBC and verify that the label, anchorage, or mounting
conforms to the certificate of compliance (IBC, Chapter 17, Section 1707.8).

5.3

Structural Testing for Seismic Resistance

The following subsections identify applicable special structural testing requirements to ensure seismic
resistance in accordance with the IBC (Chapter 17, Section 1708).

5.3.1 Testing for Seismic Resistance


The tests specified in Sections 5.3.2 and 5.3.3 are required for the following (IBC, Chapter 17,
Section 1708.1):
1.

The Seismic-Force-Resisting Systems in structures assigned to Seismic Design Category D, as


determined in IBC Chapter 16, Section 1616.

2.

Designated seismic systems in structures assigned to Seismic Design Category D.

3.

Architectural, mechanical, and electrical components in structures assigned to Seismic Design


Categories D, E, or F that are required in IBC Chapter 17, Sections 1707.6 and 1707.7.

5.3.2 Reinforcing and Prestressing Steel


Certified mill test reports shall be provided for each shipment of reinforcing steel used to resist flexural,
shear, and axial forces in reinforced concrete intermediate frames, special moment frames, and boundary

elements of special reinforced concrete or reinforced masonry shear walls. Where the Standard
Specification for Deformed and Plain Billet-Steel Bars for Concrete Reinforcement
(ASTM A 615/A 615M), reinforcing steel is used to resist earthquake-induced flexural and axial forces in
special moment frames and in wall boundary elements of shear walls in structures assigned to Seismic
Design Category D, as determined in Section 1616, the testing requirements of the Building Code
Requirements for Structural Concrete and Commentary (ACI 318-08/318R-08), shall be met. Rebar is not
to be welded.

5.3.3 Mechanical and Electrical Equipment


The registered DPIRC shall state the applicable seismic qualification requirements for designated seismic
systems on the construction documents. Each manufacturer of designated seismic system components
shall test or analyze the component and its mounting system or anchorage and submit a Certificate of
Conformance for review and acceptance by the DPIRC for design of the designated seismic system and
for approval by the LBO. Qualification shall be by actual test on a shake table, by three-dimensional
shock tests, by an analytical method using dynamic characteristics and forces, by the use of experience
data (i.e., historical data demonstrating acceptable seismic performance), or by more rigorous analysis
providing for equivalent safety (IBC, Chapter 17, Section 1708.5).

5.4

Structural Observations

Where required by the provisions of IBC Chapter 17, Sections 1710.2 or 1710.3, the Owner shall employ
a registered design professional to perform structural observations as defined in IBC Chapter 17,
Section 1702.
Structural observations are the responsibility of the structural Engineer of Record unless otherwise stated
in the Subcontract. The Engineer of Record must subcontract observations if he/she is in the same
company as the prime Subcontractor, and the LBO must approve observation performance by persons
other than the structural Engineer of Record.
At the conclusion of the work included in the permit, the Structural Observer shall submit to the LBO a
written statement that the site visits have been made and identify any reported deficiencies that, to the best
of the Structural Observers knowledge, have not been resolved. Structural observations may be made at
significant construction stages and/or at completion of a structural system. Significant construction stages
include:
1.

Prior to pouring the concrete and after the rebar has been set in place for all primary reinforced
concrete structural components and

2.

Following installation of primary steel structural components and before enclosure of the steel
beams and columns with architectural cladding, fire proofing, and/or insulation.

The following subsections identify the minimum structural observation requirements from the IBC
(Chapter 17, Section 1710), which are applicable to construction of the LLW Subproject.

5.4.1 Seismic Considerations


Structural observations shall be provided for those structures included in Seismic Design Category D (as
determined in the IBC, Chapter 16, Section 1616), where one or more of the following conditions exist
(IBC, Chapter 17, Section 1710.2):
1.

The structure is classified as Occupancy Category III or IV in accordance with IBC Chapter 16,
Section 1604.5;

2.

The height of the structure is greater than 75 ft above the base;

3.

When so designated by the registered DPIRC of the design; and

4.

When such observation is specifically required by the LBO.

Per LANLs ESM (STD-342-100, Chapter 5, Section II, Commercial Design and Analysis
Requirements, Paragraph 1.1), all PC-1 structures are to be classified as Occupancy Category II,
therefore, structural observation for seismic consideration is not required since the LLW Treatment
Building is not greater than two stories in height.

5.4.2 Wind Considerations


Structural observations shall also be provided for those structures sited where the basic wind speed
exceeds 110 mph (determined from IBC, Chapter 16, Figure 1609), where one or more of the following
conditions exist (IBC, Chapter 17, Section 1709.3):
1.

The structure is classified as Occupancy Category III or IV in accordance with IBC Chapter 16,
Section 1604.5;

2.

The height of the structure is greater than 75 ft;

3.

When so designated by the registered DPIRC of the design; and

4.

When such observation is specifically required by the LBO.

Per LANLs ESM (STD-342-100, Chapter 5, Section II, Paragraph 1.1), all PC-1 structures are to be
classified as Occupancy Category II, therefore, structural observation for wind consideration is not
required since the height of the LLW Treatment Building is less than 75 ft.

6.0

Written Work Products

6.1

Inspection Reports

Special inspections, tests, and structural observations shall include those identified in Appendix A.
Special Inspectors shall keep records of all inspections, tests, and structural observations performed. The
Special Inspector shall complete written inspection reports for each inspection and provide the reports on
a timely basis as determined by the LBO. Reports shall indicate whether the work inspected was done in
conformance to approved construction documents. The Special Inspector shall furnish these reports
directly to the LANL Chief Inspector, DPIRC, and others as designated by the Project Manager. The
reports should be generated on a daily basis, as applicable, and may be submitted weekly at the option of
the Chief Inspector. In these reports, Inspectors should, as a minimum:

Describe inspections and tests made with applicable locations;


Indicate how nonconforming items were resolved;
List unresolved items, parties notified, and time and method of notification;
Itemize changes authorized by DPIRC if not included in nonconforming items; and
Identify stop work notices, nonconformance reports, or other documentation vehicles that provide
information regarding the quality of the IBC-related work.

6.2

Nonconformance Reports

Special Inspectors shall bring all nonconforming items to the immediate attention of the Contractor. If
any such item(s) is not resolved in a timely manner, or is about to be incorporated into the work, the LBO
and DPIRC should be notified immediately and the item noted in the Special Inspectors written report.
The Special Inspector shall write a separate report regarding noted discrepancies which should contain, as
a minimum, the following information about each nonconforming item:

Description and exact location of the nonconforming item(s),


Reference to applicable design detail,
Name and title of each individual notified and method of notification, and
Resolution or corrective action taken.

Inspector Notifications

The Special Inspector shall notify the LBO (regardless of whether the Special Inspector reports directly to
the LBO, the Inspection Agencies, or other entity onsite) immediately when conditions on the project
cause the Inspector to be unable to perform their duties. This can be due to lack of notification of hold or
witness points, lack of funding, reduced funding, or any conditions that impacts the Special Inspectors
independence.

6.4

Final Report

A final report documenting all the required IBC special inspections, tests, and structural observations, and
correction of any discrepancies noted in these activities shall be prepared by the Special Inspector or
Inspection Agency engaged in conducting the work. The final report shall include all applicable
inspection and test reports and other supporting data as evidence of the special inspections, tests, and
structural observations performed. The Special Inspector, or Inspection Agency employed, shall submit
the final signed report to the LBO, Chief Inspector, and DPIRC stating that all items requiring IBC
inspection and testing were fulfilled and reported and, to the best of their knowledge, in conformance with
the approved drawings, specifications, and the applicable provisions of the IBC. Items not in
conformance, unresolved items, or any discrepancies in inspection coverage, missed inspections, periodic
inspection (when continuous inspection was required), etc., should be specifically itemized in this report.
This report will be included in the official LLW Subproject records to support facility and process
operational readiness activities for the LLW Subproject.

7.0

Schedule

The schedule for performing special inspections, tests, and structural observations will be developed at a
later stage in project execution, based on the detailed construction schedule of the LLW Subproject.
