DEFINITION OF THE TERM:

These are services that help people to receive care at home, rather in a hospital or long
term care facility, and to live as independently as possible in the community.

Home and community care is delivered by regulated health care professionals (eg.
nurses), non regulated workers, volunteers, friends and family care givers

Community healthcare has a number of benefits. Most importantly, treating patients in


their homes and in the communities where they are comfortable helps us provide a more
holistic, well-rounded and bespoke healthcare service to each and every one of our
patients. Improvements in treatments and technology mean more people than ever before
can be effectively treated in their own home. Not only does this provide faster and more
convenient care for our patients, it also comes at lower cost to the NHS. Our services
range from district nursing, health visiting and home-based rehabilitation programmes, to
NHS walk-in centres, specialist diabetes services and educational nutrition and dietetics
sessions.

COMMUNITY HEALTHCARE SERVICES ARE DESIGNED TO:


1. keep people out of hospital unnecessarily
2. treat people in their homes and community instead of hospital
3. keep people as independent as possible, for as long as possible
4. help people recover more quickly from their hospital stays
5. increase each patient's quality of life by effectively treating their long term conditions
6. work with new mothers to ensure their babies get the best start in life possible
7. help children develop to their full potential in their early years.

Assist people in remaining as independent as possible,

Support families in coping with a family members need for care


Help people stay at or return home and receive needed treatment, rehabilitation or
palliative care, and
Provide informal/ family caregivers with the support they need

DEFINITION OF THE TERM:

Family planning is the planning of when to have children, and the use of birth control and other
techniques to implement such plans. Other techniques commonly used include sexuality
education, prevention and management of sexually transmitted infections, pre-conception
counseling and management, and infertility management.

Family planning is sometimes used as a synonym or euphemism for the use of birth control,
however, it often includes a wide variety of methods, and practices that are not birth control. It is
most usually applied to a female-male couple who wish to limit the number of children they have
and/or to control the timing of pregnancy (also known as spacing children). Family planning may
encompass sterilization, as well as abortion.

PURPOSE OF FAMILY PLANNING

The purpose of family planning is to make sure that any couple, man, or woman who has the
desire to have a child has the resources that are needed in order to complete this goal. With these
resources a couple, man or women can explore the options of natural birth, surrogacy, artificial
insemination, or adoption. In the other case, if the person does not wish to have a child at the
specific time, they can investigate the resources that are needed to prevent pregnancy, such as
birth control, contraceptives, or physical protection and prevention.
