In view hereof we wish to renew our 3- in-1 accreditation (PCB, TB DOTS, MCP and NCP
package).Attached are the requirements for the said renewal however we have not yet secured
BEMONC Certificate due to some problems which pertain to the schedule of the Monitoring and
Assessment of our facility which will be conducted by the Department of Health in correlation
with PHO and CVMC.
In lieu of this may we appeal to your good office to please consider our renewal inspite of our
lacking requirement. We promise to comply the soonest possible time.
