Filling in the IELTS

Application Form

Centre stamp: Leave blank. The appropriate British Council office will stamp this box.

Candidate’s recent photograph: Attach two recent coloured passport sized


photographs. (Please refer to page iv of the IELTS Application form for specifications).

Please fill in the form in INK and BLOCK LETTERS

01. Preferred Date of Test: Pencil in the first 2 dates of your choice which the
appropriate British Council office has to offer (Visit British Council website
http://www.britishcouncil.org/pakistan-exams-ielts-calendar.htm for IELTS test date
schedule).

02. Family Name: Enter your family name/surname/father’s name/last name (this is
your surname as is applicable). Please enter this name exactly as it appears in your
passport/NIC.

For example: If your full name is Hassan Malik Your family name will be Malik
If your full name is Muhammad Usman Khan Your family name will be Khan
If your full name is Syeda Fatima Bokhari Your family name will be Bokhari

03. Circle the appropriated designation. Qualified doctors must circle only Dr.

04. Other Names: Enter your first name (followed by your middle name, if you wish).
Please enter these names exactly as they appear on your passport/NIC.

For example: If your full name is Hassan Malik Your other name will be Hassan
If your full name is Muhammad Usman Khan Your other name will be Muhammad Usman
If your full name is Syeda Fatima Bokhari Your other name will be Syeda Fatima

05. Address for Correspondence: Write your complete address along with the post
code. (We will courier your venue letter and IELTS result to this address).

06. Telephone/Mobile Number: Write your STD Code followed by your telephone
number. You may also provide your mobile number.

07. E-mail: Write your email address for quicker correspondence with you.

08. Date of Birth: Day first, month second and year last, all in numerals (e.g. 22/11/67).
Please enter your date of birth as it appears on your passport/ NIC.

09. Gender: Circle as appropriate.

10. Passport or National Identity Card Number: State your passport/ National
Identification Card Number. Please indicate the number of the ID document that you
have submitted at the time of IELTS registration along with the IELTS Application form.

11. Nationality: See the code list on page ii of the IELTS Application Form. For Pakistan
the code is 149.

12. First Language: See the code list on page iii of the IELTS Application Form. Your
first language is your mother tongue.

13. Occupation Sector and Level: Look for closest description of your occupation on
page iv of the IELTS Application Form. Level: Put the appropriate number in this
column. See the code list on page iv of the IELTS Application Form. If you are still a
student enter 08 (education) for sector and 07 for level (student).

14. Why are you taking the test: See the code list on page iv of the IELTS Application
Form.

15. Which country are you applying to: Circle as appropriate. If you are applying to
more than one country, circle the country that is your first choice

16. Which IELTS test format are you taking: Circle as appropriate. Doctors proceeding
to the UK for the PLAB test must take the Academic module. All students going for
further studies must also take the Academic module. Potential immigrants to New
Zealand, Australia and Canada may opt for the General Training module. It is advisable
to check with the university/ institute/ immigration authority where you have applied to
confirm which module of the IELTS test they require applicants to take.

17. Which IELTS test are you taking: Tick pen and paper test. (Computerised IELTS test
is not yet available).

18. Have to taken IELTS before: Tick as appropriate.

19. Most recent IELTS test details: If your answer to question 18 is ‘No’, please skip
question 19. If it is ‘Yes’, please refer to the Test Report Form of your previous attempt
and fill in the exact details. Please also attach a copy of the Test Report Form of your
previous attempt to the IELTS Application Form. If this is your third or more attempt,
please submit the copy of the Test Report Form of your most recent IELTS test.

20. Where are you currently studying English: Write as appropriate.

21. What level of education have you completed: Tick as appropriate.

22. How many years have you been studying English: Tick as appropriate.

23. Do you have any special needs: Please enter the relevant information, if applicable.

24. Name of the College/university/institute/Consulates you have applied for admission


or immigration. Enter the name and the address of the institute you require us to send
your scores (i.e., IELTS Test Report Forms). Candidates applying for immigration
should enter the name and the address of the respective consulates where the scores
require to be sent. It is the centre’s responsibility to furnish receiving institutions with
the Test Report Forms. Receiving Institutions expect this to be the case. When
candidates send copies, the receiving institutions waste a great deal of time verifying
the results that will only hold up your application.

Please read the declaration on page 4 of the IELTS Application Form and sign and date
it. Please note that if the declaration is not signed the IELTS Application Form will not
be processed further.
