2. Above references pertains to the directive of the Chief, DHSU to conduct House
Visitation to SPO1 Ramil S Hachero, PO2 Mark L De Paz, PO2 Ebonn O Decatoria, PO2
Weavin B Masa, PO2 Jerome C Datinguinoo, who were all under Restrictive Custody of the
Chief, DHSU.
3. Please be informed that the following personnel were visited in their last known
addresses recorded in the DPHRDDs Personnel Data Profile and the result of such visitation, to
wit:
a. SPO1 Ramil S Hacheros last known address was at 1157 Abad Santos Ext.,
Tondo, Manila was served with the Notice to Report/Return to Work Order last June 5, 2015.
However, as per Marivic S Hachero, a sister, he was no longer residing nor visiting the given
address since June 3, 2015. In the picture taken during the said activity, the notice was tendered
to his sister who signed and received such notice.
b. PO2 Mark L De Paz was visited at his last known address located at 79
Tagdali St., Manreza, Quezon City. But upon visitation, the visiting team found out that there
was no PO2 Mark L De Paz residing in the given address and situated therein is the factory of
Boysen Paint Inc.
c. PO2 Ebonn O Decatorias is not a resident and is unknown in the address
reflected in his Police Personnel File which is 17 St. Dominic Drive, Quirino Highway, Baesa,
Quezon City. As per Lady Guard Rowena A Padernas, she is unaware of any person having a
name of PO2 Decatoria, let alone a resident of their community.

d. PO2 Weavin B Masa who is a resident of 1021 Sitio Gitna Caybiga, Caloocan
City is no longer residing in the given address. But the Notice to Report/Return to Work Order
was duly received by his cousin, Mila Zepeda on June 8, 2015.
e. PO2 Jerome C Datinguinoo is no longer a resident of 179-D M. De Leon St.,
Santolan, Pasig City after the undersigned went to the same address and met his uncle, Arvin A
Camba last June 5, 2015 who also received the Notice to Report/Return to Work Order.
4. Both undersigned exerted effort to locate the above police officers as proven by the
photographs and received notices attached herein but the same proved to be futile to locate to
whereabouts as of this date.

2. Above reference pertains to the issuance of a warrant of arrest against SPO1 Ramil S
Hachero, PO2 Mark L De Paz, PO2 Ebonn O Decatoria, PO2 Weavin B Masa, PO2 Jerome C
Datinguinoo by the Mandaluyong City Regional Trial Court and their absence from their
respective duties at the DHSU, QCPD.
3. In this connection, you are hereby directed to conduct home visitation on the said
personnel in the addresses indicated beside their names, to wit:

2. This pertains to the directive of the Chief, District Directorial Staff, QCPD to locate
the whereabouts of SPO1 Ramil S Hachero, PO2 Mark L De Paz, PO2 Ebonn O Decatoria, PO2
Weavin B Masa, PO2 Jerome C Datinguinoo, who were all placed under Restrictive Custody of
the Chief, DHSU.
3. Please be informed that upon receipt of the verbal instruction of the CDDS, QCPD to
locate the whereabouts and the possible apprehension of the above mentioned police officers
after the issuance of the Warrant of Arrest by the Mandaluyong City Regional Trial Court, the
undersigned immediately directed the Chief Clerk and the RSPNCO of this Unit to conduct
Home Visitation in their last known addresses recorded in the DPHRDDs Personnel Data
Profile. The results of such visitation are as follows:
a. SPO1 Ramil S Hacheros last known address was at 1157 Abad Santos Ext.,
Tondo, Manila was served with the Notice to Report/Return to Work Order last June 5, 2015.
However, as per Marivic S Hachero, a sister, he was neither longer residing nor visiting the given
address since June 3, 2015. In the picture taken during the said activity, the notice was tendered

was no PO2 Mark L De Paz residing in the given address and situated therein is the factory of

4. Administrative charges for neglect of duty has been filed with the Chief, DIDMD,
QCPD and recommended that they be placed on AWOL status to cut-off the payment of their
respective salaries from their issued salary ATM Cards. Currently, a standing order was also
given to the Unit ESPO, Shift In-Charge and Chief Clerk, DHSU to conduct continuous
monitoring, accounting and possible location these personnel, however, this Unit, through the
undersigned exerted efforts to locate the above police officers as proven by the photographs and
received notices attached herein but the same proved to be futile in determining their
whereabouts as of this date. Rest assured that DHSU will continuously conduct coordination and
cooperation with the CIDU, Mandaluyong Police Station, EPD and other law enforcement units
for the possible apprehension and bringing into justice the live bodies of all accused personnel.
