Subject: Coroner's Inquest
I am the father of late Makibi Olayuk Timilak. Me and my Common-law spouse Neevee Akesuk have full
knowledge and fully support our friend Gwen Slade for a request of Coroner's inquest in the preventable death
of our son on the night of April 5, 2012.
We are upset that you have denied this request for a inquest not once but twice. We are asking Gwen to add
our letter to her response of your last denial.
We need to know the circumstances of his death because the autopsy report says his death is unclassified.
Until his death is classified, the investigation is not over. The circumstances of our son's death needs to be
determined in order for his death to be classified. We know a virus caused his death, we know the method by
which he died, we need to know why our first born son died. He died when he could have been saved.
We need a Coroner's inquest to determine if our first born son died of negligence, misconduct, or malpractice.
As the parents of our first born child who is now deceased, we have the right to ask for a inquest.
In your reports, you admitted that the nurse did not follow the standard policies and guidelines expected by the
government. We need a inquest to determine if this was a negligence death, if there was misconduct, or
malpractice.
We are aware that our friend Gwen Slade is asking for a inquest on behalf of our families. We as parents are
also requesting a inquest to be conducted.

Good afternoon Mr. Qaumagiaq,
On behalf of the Office of the Chief Coroner, I offer my deepest sympathies for your loss. I am
aware of the circumstances surrounding the death of your son and I will clarify your concerns
and questions in this case.
I understand that Ms. Slade is your friend and she is advocating on your best interest. However, I
have my limitations for her request. We can only accept inquest requests from the families
directly related to the deceased. Friends of the deceased family do not have the authority to
request the inquest.
Now that you are directly in contact with me, we will clarify the circumstances in which the
death happened in complete detail. I will keep you posted in the near future with your concerns
as to wanting an inquest into this death.
