Instructions for Use: This form should be completed by the LPA RC and provided to the contractor to identify the items that need to be addressed
prior to closing out the project. These items will be rechecked until all are resolved. This form may include deficiency in documentation regarding
certifications, payrolls, test results, etc. The LPA RC is responsible for adding a copy of the completed checklist to the project correspondence log in
SiteManager.
